概要

脊椎手術の分野では、近年、低侵襲脊椎手術(MISS)技術が大きく進化し、開腹手術から顕微手術、顕微内視鏡手術、内視鏡手術へと移行しています。脊椎内視鏡手術は、脊椎疾患の治療と満足のいく臨床転帰の達成に広く使用されているMISS技術の高度な形態です¹。従来の開腹手術と比較して、組織の損傷が少なく、出血が少なく、回復が早く、術後の合併症が少ないという利点^{があります2。}

脊椎内視鏡手術は、1つまたは2つのポータルを使用して行うことができます。片側両門脈内視鏡(UBE)脊椎手術は、アルゼンチンで腰椎椎間板切除術を行うために最初に報告された革新的なタイプのMISS技術です。その後、韓国でも減圧手術や固定術を行うように改良されています³。UBE法の手術手順は、従来の開腹手術と同様です。ただし、UBE技術は侵襲性が低く、従来の開腹手術と比較してより明確な視野を提供します^4,5。

従来のUBE法は、片側減圧を可能とするだけでなく、脊柱管狭窄症の両側減圧も実現する^6,7

プロトコル

中山大学第三附属病院の治験審査委員会は、プロトコルを承認しました(ID:[2022]02-356-01)。対象となったすべての患者は、インフォームドコンセントフォームに署名する必要がありました。

1.外科的適応症

1. 対側凹部狭窄症または椎間板ヘルニアを有する腰椎脊柱管狭窄症の患者、または対側神経圧迫を有する患者を含める。

2.手術方法

1. UBE-ULBD技術による両側内視鏡的脊柱管減圧術とT-UBEによる内視鏡的椎間板切除術を行います。

3.術前手順

1. 麻酔と手術位置:
   1. 全身麻酔下で、患者を脊椎骨にうつ伏せに置き、不快感を避けます。腰椎を少し曲げて、層間空間を開きます。
      注:気管内挿管または脊椎と硬膜外麻酔を組み合わせた全身麻酔は、術中の痛みを最小限に抑えるための外科的処置に使用されます。
2. 希望のポータル位置(L4-5など):
   1. 標準的な前後(AP)X線図を取得し、手術台を調整して、X線側面図でターゲットの椎間腔を地面に対して垂直にします。
