Inflammatoire processen als gevolg van de afscheiding van oplosbare mediatoren door immuuncellen, leiden tot verschillende manifestaties in de huid, gewrichten en andere weefsels, alsmede gewijzigde cytokine homeostase. Het aangeboren immuunsysteem speelt een cruciale rol in het herkennen van ziekteverwekkers en andere endogene gevaar stimuli. Een van de belangrijkste cytokines vrijgegeven door aangeboren immuuncellen is Interleukine (IL) -1. Daarom maken we gebruik van een volbloed stimulatie test om de secretie van inflammatoire cytokines te meten en in het bijzonder van de pro-inflammatoire cytokine IL-1β 1, 2, 3. Patiënten met genetische disfuncties van het aangeboren immuunsysteem waardoor autoinflammatory syndromen vertonen een overdreven versie van volwassen IL-1β na stimulatie met LPS alleen. Met het oog op het aangeboren immuunsysteem component van de patiënten die zich presenteren met inflammatoire-geassocieerde aandoeningen te evalueren, gebruiken we een specifieke immunoassay om cellulaire immuunresponsen te detecteren pathogeen-geassocieerde moleculaire patronen (PAMPs), zoals de gram-negatieve bacteriële endotoxine, lipopolysaccharide (LPS ). Deze PAMPs worden herkend door pathogeen herkenning receptoren (PRRS), die worden gevonden op de cellen van het aangeboren immuunsysteem 4, 5, 6, 7. Een primaire signaal, LPS, in combinatie met een tweede signaal, ATP, noodzakelijk is voor de activering van de inflammasome, een multiprotein complex dat pro-IL-1β processen om haar rijpe, bioactieve vorm 4, 5, 6, 8, 9, 10. De volbloed-test vereist slechts een minimale steekproef manipulatie tot cytokine productie te beoordelen in vergelijking met andere methoden die arbeidsintensieve isoleren en kweken van specifieke celpopulaties vereisen. Deze methode onderscheidt zich van andere volbloed stimulatie testen, maar eerder dan verdunnen monsters met een verhouding van RPMI media, voeren we een witte bloedcel direct tellen van verdund volbloed en daarom, het stimuleren van een bekend aantal witte bloedcellen in cultuur 2. De resultaten van deze bijzondere bloed-test aantonen dat er een nieuwe techniek bruikbaar in het ontrafelen van de patiënt cohorten die zich presenteren met autoinflammatory pathophysiologies.
