Er is dringend behoefte aan betaalbare CD4-telling aan HIV-ziekte te controleren. CD4-telling buiten bereik is in gebieden met beperkte middelen de regio&#39;s als gevolg van de tijd en temperatuur beperkingen, technische verfijning, en de kosten van reagentia, in het bijzonder monoklonale antilichamen tegen CD4 op bloedcellen, de enige op dit moment aanvaardbare methode te meten. Een veelgebruikt kostenbesparende en tijdbesparende laboratorium strategie is om te berekenen in plaats van maatregel bepaalde bloedwaarden. Bijvoorbeeld, LDL berekend met de gemeten concentraties van totaal cholesterol, HDL en triglyceriden 1. In de HIV-1-en vaatziekten, is α 1 PI geïnactiveerd als gevolg van ziekte-processen 4. In de vroege asymptomatische categorieën HIV-1 ziekte, werd actieve α 1 PI bevonden dan normaal in 100% van de onbehandelde met HIV-1 (mediane = 12 pM en normale te bereiken gedurende de symptomatische type 4, 5. Deze patroon is toegeschreven aan het immuunsysteem inactivering niet voldoende synthese proteolytische inactivering of zuurstof. Waargenomen werd dat in HIV-1 patiënten met&gt; 220 CD4-cellen / ul, CD4 gecorreleerd met serumconcentraties actieve α 1 PI (r 2 = 0,93, p &amp;lt; 0,0001, n = 26) en niet-actieve α 1 PI (r 2 = 0,91, p &lt;0,0001, n = 26) 5. Toediening van α 1 PI aan HIV-1 geïnfecteerde en niet-geïnfecteerde patiënten resulteerde in een dramatische toename van CD4-tellingen suggereren α 1 PI neemt deel in het reguleren van het aantal CD4 + T-cellen in het bloed 3. Met stimulatie, hele speeksel bevat voldoende sereuze exsudaat (plasma met eiwitmateriaal dat door middel van de bloedvatwanden gaat over in speeksel) om het meten van de actieve α 1 PI en de correlatie van deze meting is het bewijs dat het een accurate methode voor de berekening van CD4-tellingen. In het kort, sialogogues zoals kauwgom of citroenzuur stimuleren de afscheiding van serum in hele mond speeksel. Na stimuleren serum exudatie, wordt de activiteit van serum α 1 PI in speeksel gemeten door zijn vermogen om elastase activiteit remmen. Varkens pancreas elastase (PPE)een beschikbaar goedkope bron van elastase. PPE bindt aan een PI α die een-op-een complex PPE voorkomt splitsen zijn specifieke substraten, waarvan de colorimetrische peptide, succinyl-Ala-L-L-Ala-L-Ala-p-nitroanilide (SA 3 NA). Incuberen speeksel met een verzadigende concentratie PPE gedurende 10 min bij kamertemperatuur kan de binding van PPE alle actieve α 1 PI in speeksel. De resulterende remming van PPE door actieve α 1 PI kan worden gemeten door de PPE substraat SA 3 NA. (Figuur 1). Hoewel CD4 gemeten bloedvolume (CD4-cellen / pl), wordt de concentratie van α 1 PI in speeksel betrekking tot de concentratie van serum in speeksel, niet het volume van speeksel sinds deel aanzienlijk kan variëren overdag persoon tot persoon 6. Echter vrijwel alle eiwit in speeksel door serum inhoud en het eiwitgehalte van speeksel measurable 7. Zo wordt actief α 1 PI in het speeksel berekend als een percentage van het speeksel eiwitgehalte en wordt aangeduid als de α 1 PI Index. De resultaten die hierin aan te tonen dat de α 1 PI Index een accurate en nauwkeurige fysiologische methode voor de berekening van CD4-tellingen biedt.
