Vi har beskrevet og validert en metode for å isolere levedyktige myocytes fra kirurgiske prøver av menneskelig ventrikkel myokard. Disseksjon av de elektrofysiologiske egenskapene til hjertemuskelen har utviklet seg betydelig etter utvikling av teknikker for enkelt hjerte myocyte isolasjon. Nye fremskritt i forståelsen av hjerte eksitasjon kontraksjon kopling (EC-Coupling) har også blitt gjort mulig ved evnen til å isolere levedyktige enkelt cardiomyocytes som beholder alle de fysiologiske egenskapene til intakt vev. Patch klemme metoder blir rutinemessig brukt for å studere funksjonen og farmakologisk modulering av hjerte sarcolemmal ion strømninger. Humant atrielle vev er lett tilgjengelig som atriale vedheng blir ofte kasseres under kirurgiske prosedyrer. Innledende kvantitative studier av voksne menneskelige hjerteaksjonspotensialer og ionisk currents ansatt enzymatisk isolerte atrial celler 1-4. Fremskritt og raffinering avisoleringsprosedyre 10, tillates en klar karakterisering av ionisk basis av den økte arytmogene potensial i terminal hjertesvikt, som omfatter aksjonspotensial-forlengelse 11, forsinket etter depolariseringer 12 og økt morsom strøm 13 som fører til depolarisering diastolisk og premature slag. Voksen hjertemuskelceller er normalt isolert fra små dyr ved retrograd perfusjon av hele hjertet med ulike enzymblandinger, en teknikk som gir høy avkastning av Ca 2 +-tolerante celler 14. Isolering av hjertemuskelceller fra fragmenter av vev er iboende mindre vellykket trolig på grunn av den begrensede tilgangen av enzymer til individuelle myocytes sammenlignet med det som oppnås ved perfusjon av koronararteriene. På grunn av den svært begrensede tilgjengeligheten av ubrukte donor hjerter, den eneste praktiske måten å få normale menneskelige ventrikulære celler på en jevnlig basis er ved enzymatisk digestion av de ofte svært små vev fragmenter skåret under elektive kirurgiske prosedyrer. Bare human sykdom modell som har blitt grundig karakterisert på cellenivå er terminal hjertesvikt, på grunn av tilgjengelighet til transplanterte hjerter. Imidlertid oppstår terminal hjertesvikt i et mindretall av pasientene og ofte innebærer en felles vei av alvorlig ombygging av hjerteinfarkt celler, noe som er relativt uavhengig av den underliggende årsaken 15. Hypertrofisk kardiomyopati (HCM) er et talende eksempel. Siden bare et mindretall av HCM pasienter utvikler seg mot terminal hjertesvikt og krever hjertetransplantert, HCM hjerter er svært sjelden tilgjengelig for celleisolasjon med standard metoder. Men minst 30% av HCM pasienter utvikle obstruktive symptomer på grunn av massive septal hypertrofi endring utstrømningen tarmkanalen blodstrømmen under systole (HCM) 18. Den mest effektive tilgjengelige terapeutisk alternativ for lindring av obstruksjon i HCM er kirurgisk septal myectomy: i løpet av denne kirurgiske prosedyren, er en variabel størrelse del av øvre septum fjernet av trans aortic tilnærming. Denne delen av hypertrophied septum er derfor tilgjengelig for celleisolasjon fra den friske vev. En fremgangsmåte for isolering av humant ventricular myocytes fra enkle, små transvenøse endomyokardbiopsi eksemplarer er tidligere utviklet og utgitt 19. Vi gjennomførte en metode for å isolere enkelt septal myocytes fra ventrikulære myokard prøver fra pasienter som gjennomgår hjertekirurgi, inkludert pasienter med HCM gjennomgår septal myectomy og pasienter som gjennomgår ventil erstatning prosedyrer. isolation and functional characterization of human ventricular cardiomyocytes from fresh surgical samples

Cardiomyocytes fra syke hjerter er utsatt for komplekse remodeling prosesser med endringer i cellestrukturen, eksitasjon kontraksjon kopling og membran ion strømninger. Disse endringene vil trolig være ansvarlig for den økte arytmogene risiko og kontraktile endringer som fører til systolisk og diastolisk dysfunksjon hos hjertepasienter. Men har mest mulig informasjon om de endringer av myocyte funksjon i hjertesykdommer kommer fra dyremodeller. Her beskriver vi og validere en protokoll for å isolere levedyktige myocytes fra små kirurgiske prøver av ventrikkel myokard fra pasienter som gjennomgår hjertekirurgi operasjoner. Protokollen er beskrevet i detalj. Elektrofysiologiske og intracellulære kalsium målinger er rapportert å demonstrere gjennomførbarheten av et antall enkeltcellemålinger i humane ventrikulære cardiomyocytes oppnådd med denne metoden. Protokollen rapporterte hanre kan være nyttig for fremtidige undersøkelser av cellulære og molekylære basis av funksjonelle forandringer av det menneskelige hjerte i nærvær av forskjellige hjertesykdommer. Videre kan denne fremgangsmåte anvendes for å identifisere nye terapeutiske mål på cellenivå, og for å teste effektiviteten av nye forbindelser på human cardiomyocytes, med direkte overføringsverdi.

Metoden beskrevet ovenfor ble anvendt for å karakterisere de funksjonelle forandringer av kardiomyocytter isolert fra septum av pasienter med hypertrofisk kardiomyopati (HCM) som gjennomgikk myectomy drift, sammenlignet med ikke-sviktende uten hypertrofiske kirurgiske pasienter 21.
