Interpretation von Cytomegalovirus (CMV)-Infektion in klinischen Proben ist von entscheidender Bedeutung. CMV ist ein Mitglied der Unterfamilie Betaherpesvirinae von Herpesviren. Ähnlich wie bei anderen Herpesviren, hat die Fähigkeit, CMV dauerhafte oder latente Infektion zu etablieren, gekennzeichnet durch einen Mangel der aktiven Virusreplikation 1. Reaktivierung des Virus kann in Zeiten von Stress oder andere Immunsuppression auftreten, was zu aktiver Virusreplikation, gekennzeichnet durch Virion Mitteilung, die von Krankheitsmanifestationen 2-4 begleitet sein kann. Gastrointestinale (GI) CMV-Erkrankung Symptome sind häufig Bauchschmerzen und / oder Blut im Stuhl ein. Die Diagnose der CMV Gastroenteritis durch Histologie nutzt Hämatoxylin und Eosin (H &amp; E), um virale CMV Einschlüsse identifizieren. In Fällen, in denen der klinische Verdacht hoch noch keine offensichtlichen Einschlüsse beobachtet werden, ist der Immunhistochemie (IHC) häufig als Ergänzung Testverfahren verwendet. Jedoch IHCkann auch durch nicht-klassische selten auftretenden Zellfärbungsmuster (zweideutig) behindert werden, so dass die Interpretation erschweren (Abbildung 2) 5. Es wurde versucht, aus Formalin fixierten und in Paraffin eingebetteten (FFPE) GI Biopsiegewebe und quantitative Polymerase-Kettenreaktion (qPCR) auf CMV in GI Biopsien erkennen extrahierten DNA zu verwenden. Diese Technik hat sich gezeigt, wertvoll bei der Detektion von CMV in GI Biopsien werden und ist besonders nützlich in zweideutigen IHC Färbung Fällen 5,6. Auch hat qPCR auch gezeigt, dass auch mit zusätzlichen klinischen CMV Testdaten zu korrelieren, wenn es 6 ist verfügbar. Verwendung von qPCR bei der Aufdeckung von CMV kann auf frühere Diagnose und Behandlung in Fällen, in denen der klinische Verdacht auf CMV ist hoch führen, aber H &amp; E-und CMV-IHC negativ sind.

Es ist entscheidend, Cytomegalovirus (CMV)-Infektion im Magen-Darm-Trakt (GI) bei immunsupprimierten Patienten zu identifizieren, aufgrund ihrer höheren Risiko für schwere Infektionen. Viele Laborverfahren zum Nachweis von CMV Infektion entwickelt worden, einschließlich der Serologie, Viruskultur und molekularbiologischer Methoden. Daher wird der Nachweis von CMV Infektion in den GI-Trakt häufig durch herkömmliche Histologie der Biopsiegewebe gemacht. Hämatoxylin und Eosin (H &amp; E)-Färbung in Verbindung mit der Immunhistochemie (IHC) haben die Hauptstützen der Untersuchung dieser Biopsien geblieben. H &amp; E und IHC manchmal in atypischen (zweideutig) Färbungsmuster führen, was die Interpretation erschweren. Es wurde gezeigt, dass quantitative Polymerase-Kettenreaktion (qPCR) zur CMV erfolgreich auf Formalin-fixierten und in Paraffin eingebetteten (FFPE) Gewebebiopsie durchgeführt werden, für sehrhohe Sensitivität und Spezifität. Das Ziel des Protokolls ist es, zu demonstrieren, wie qPCR Tests für den Nachweis von CMV in FFPE Biopsiegewebe in einer klinischen Laborumgebung durchzuführen. Diese Methode ist wahrscheinlich von großem Nutzen für die Patienten in Fällen von zweideutigen Färbung für CMV in GI Biopsien sein.

Ein insgesamt 228 Gewebeblöcke wurden durch quantitative Polymerase-Kettenreaktion (qPCR), die von 91 Cytomegalovirus (CMV)-positiven Fällen mit zweideutigen CMV IHC bestand, wurde basierend auf positive Histologie und Immunhistochemie (IHC), 18, und 79 negative Kontrollen getestet.
