Sammenlignet med konvensjonelle positive blodkultur diagnostiske metoder, den raske anskaffelse av en bakteriell pellet ved hjelp av ammoniumklorid lysis sentrifuge tilnærming reduserer tiden for å rapportere identifisering av 16 til 24 timer, og tiden til å rapportere AST etter 24-48 timer (figurene 1 og 3). Blodet infeksjoner og sepsis i innlagte pasienter er en viktig årsak til sykelighet og dødelighet. Dermed er dødelighet relatert til blodbanen infeksjoner observert hos ca 14% til 37% av innlagt pasient og kan øke til 35% i intensivavdelinger pasienter 1-3. Den hurtige identifiseringen av infeksjonsmidlet er avgjørende for å lede optimal antimikrobiell behandling, og for å øke det vellykkede utfall av antimikrobiell terapi 4,5. Den raske analyse av Gram flekker fra positiv blodkultur har allerede en betydelig innvirkning på tilpasning av antimikrobiell behandling 6,7 men nøyaktig identifisering av smittestoffet er nødvendig for å gi best tilpasset antibiotikabehandling til pasientene. For eksempel ulike antibiotika behandlingsregimer må gjennomføres etter bakteriemi med enterokokker og streptokokker som er vanskelig å skille fra Gram farging. Tilsvarende identifikasjon på arten umel er nødvendig for å påvise Gram negative enterobakterier som koder for et kromosomalt gen som AmpC gi en øket motstand mot β-laktamer 8. Med en positiv blodkultur, er den konvensjonelle diagnostiske tilnærming til subkultur smittestoffet på forskjellige agar-plater, som krever flere timers ytterligere inkubering før identifikasjon med forskjellige tilnærminger inkludert biokjemiske tester, vekst på forskjellige selektive medier og automatiserte mikrobiell identifikasjonssystemer. Tiden til resultatene av en konvensjonell diagnostisk metode er av omtrent 1 til 3 dager. Fremveksten av matrisen-assistert laser desorpsjon / ionisering time-of-flight massespektrometri (MALDI-TOF) teknologi for rask identifisering av mikroorganismer har gitt et nytt verktøy for å raskt identifisere mikroorganismer fra koloniene dyrket på agar plater men også direkte fra positivt blod kulturer (figur 1) 9-12. The bruk av MALDI-TOF å identifisere en smittsom agent fra blodkulturer har redusert tid til resultatene til noen få minutter i stedet for timer og dager som kreves av tradisjonelle metoder. Som diskutert av Croxatto et al. 13. effektiviteten av MALDI-TOF-identifikasjon er avhengig av forskjellige parametre inkludert mikroorganismen renhet og mengde. Disse to kriterier er lett oppnådd fra diskrete kolonier dyrket på agarplater, men kreves en forhånds analytisk behandling av bakteriell anrikning og rensing fra komplekse prøver som blodkultur, som inneholder flere cellulære og protein-komponenter som kan forstyrre MALDI-TOF identifikasjon. Ulike mikroorganismer &#39;isolasjon metoder fra blodkultur har blitt brukt i en rekke studier inkludert saponin eller andre milde vaskemidler metode for bakteriell utvinning 9,14, serum separator metode 10, lyse sentrifugeringsmetoder 12 </sopp&gt; og kommersielt løsninger som sepsityper kit. Vår bakteriologi diagnostiske laboratorier har utviklet en enkel blod-kultur bakteriell pellet preparat basert på ammoniumklorid erytrocytt-lysering, som tillater hurtig identifisering av bakterier og gjær ved MALDI-TOF, og automatiske identifikasjonssystemer (figur 2) 15. Dette blod-kultur pellet forberedelse også gi et eksempel for andre direkte nedstrøms applikasjoner som Gram farging, automatisert PCR-baserte diagnostiske tester som POCT-PCRs for rask påvisning av meticillinresistente Staphylococcus aureus (MRSA), og antibiotika resistenstesting med AST automatiserte systemer og / eller ved å disk diffusjon Målinger på agarplater (figur 3). Blodet infeksjoner og sepsis er en viktig årsak til sykelighet og dødelighet. På samme måte kan resultatet av en hurtig PCR-basert assay bli kommunisert til klinikere mindre enn 2 timer etter rapport av en bakteriemi. Sammen utgjør disse resultatene viser at den raske utarbeidelse av en blodkultur bakteriell pellet har en betydelig innvirkning på identifisering og AST behandlingstid og dermed på det vellykkede resultatet av pasienter som lider av blodet infeksjoner.

I undersøkelsen utført av Prod&#39;hom et al. 15, ble bakterie pellets fremstilt ved ammoniumklorid lysis sentrifugering av 122 positive blodkultur fra 78 pasienter analyseres med MALDI-TOF MS. Ut fra 122 positive blodkultur, 95 (77,9%) ble korrekt identifisert på artsnivå og en (0,8%) ved slekten nivå. De resterende 26 (21,3%) av blodkultur pellets ga ingen sikker identifikasjon av MALDI-TOF. Blant dem, var 21 gram positive bakterier hvorav 13 streptokokker og 5 koagulase-negative stafylokokker...
