Genterapi har länge föreslagits som en potentiell behandling för genetisk hörselnedsättning, men framgång inom detta område har varit svårfångade 1. Hittills har viralt medierade metoder dominerade grund den teoretiska möjligheten att rikta specifika celltyper inom den relativt otillgängliga snäckan. Båda adenovirus (AV) och adeno-associerat virus (AAV) har använts för cochlea gentillförsel. AAV är fördelaktiga i snäckan för ett antal skäl. Snäckan är potentiellt ett idealiskt mål för genterapi av flera skäl: 1) dess liten volym nödvändiggör en begränsad mängd av viruset som behövs; 2) dess relativa isolering från andra organsystem gränser biverkningar; och 3) dess vätskefyllda kamrar underlättar virusleverans i hela labyrinten 10, 11,12,13,14, 15. Musmodeller av kongenital dövhet möjliggör en användning av många metoder för undersökning för att övervaka utvecklingen av innerörat på ett systematiskt, replikerbar sätt. Medan den lilla storleken hos mus hörselsnäckor medför några kirurgisk svårighet, tjänar musen som en extremt viktig modell i studien av genetisk hörselnedsättning, med flera experimentella fördelar jämfört med andra arter 16. Musmodeller tillåter bedömning av en rad egenskaper genom genetisk kopplingsanalys, samling av detaljerade morfologiska observationer, och simulerar patogena scenarier; som sådana, de är goda kandidater för viralt medierad genterapi. Omfattande genetiska studier på möss i kombination med tekniska framsteg har gjort det möjligt att generera genetiskt modifierade möss på ett reproducerbart sätt över laboratorier 17,18, 19, 20,21. Furthermore, finns det många modeller för både förvärvade och nedärvda hörselförlust fenotyper hos möss, vilket gör rigorösa tester i denna djurmodell 22, 23,24. Således korrigera hörsel använder viralt medierad genterapi i en musmodell är ett lämpligt första steg i sökandet efter ett botemedel för mänskliga sjukdomar. Vi har tidigare visat att transgena möss som saknar vesikulär glutamat transportör 3 (VGLUT3) föds döva på grund av brist på glutamatfrisättning vid IHC band synapsen 25. Eftersom denna mutation inte leder till en primär degeneration av sensoriska hörselceller, dessa muterade möss är potentiellt en utmärkt modell, i vilken för att testa cochlea genterapi för medfödd hörselnedsättning. Hittills har ett antal virala leveranstekniker för cochlea genterapi har beskrivits, inklusive runda fönstermembranet diffusion, runda fönstermembranet injektion, och tillförsel via en cochleostomy. Det finns potentaial fördelar och nackdelar med vart och ett av dessa tillvägagångssätt 9. Här rapporterar vi en kirurgisk metod för viralt medierad gentillförsel till VGLUT3 KO mouse innerörat genom det runda fönstermembranet (RWM). Den post aurikulär RWM injektion metod är minimalt invasiv med utmärkt hörsel konservering, och är relativt snabb. Som vi tidigare har publicerats, i ett försök att återställa hörseln i denna musmodell, var en AAV1 vektor bär VGLUT3 genen (AAV1-VGLUT3) införs i snäckan av dessa döva möss vid postnatal dag 12 (P! @), Vilket resulterar i restaurering av hörsel 26. Utfrågning i VGLUT3 KO möss verifierades med hörselhjärnstammen svar (ABR), medan transgen proteinuttryck verifierades med hjälp immunofluorescens (IF). Denna metodik visar således att viralt medierad genterapi kan korrigera en genetisk defekt som annars skulle resulterar i dövhet.

surgical method for virally mediated gene delivery to the mouse inner ear through the round window membrane

Genterapi, som används för att uppnå funktionell återhämtning från sensorineural dövhet, lovar lämna bättre förståelse av de bakomliggande molekylära och genetiska mekanismer som bidrar till hörselnedsättning. Införande av vektorer in i innerörat måste göras på ett sätt som har brett distribuerar medlet i snäckan och samtidigt minimera skador på de befintliga strukturerna. Detta manuskript beskriver en post-aurikulär kirurgisk metod som kan användas för mus cochlea terapi med användning av molekylära, farmakologisk och viral leverans till möss postnatal dag 10 och äldre via runda fönstermembranet (RWM). Denna kirurgisk metod möjliggör snabb och direkt leverans till scala tympani samtidigt minimera blodförlust och undvika djurdödligheten. Denna teknik innebär försumbar eller ingen skada på viktiga strukturer i inner och mellanörat samt nackmusklerna samtidigt helt bevara hörseln. För att demonstrera effekten av detta kirurgisk teknik, den vesikulära glutamåt transportör 3 knockout (VGLUT3 KO) möss kommer att användas som ett exempel på en musmodell av kongenital dövhet som återvinner höra efter leverans av VGLUT3 till innerörat med hjälp av en adeno-associerat virus (AAV-1).

För att verifiera de tekniska egenskaperna och nyttan av efter auricular strategi för cochlea molekylär terapi, var AAV1-VGLUT3, AAV1-GFP och AAV2-GFP levereras i P10-12 möss innerörat via RWM. Detta tillvägagångssätt visar framgångsrik transgenexpression inom inre hårcellerna (IHC) (VGLUT3 Figur 1 och GFP Figur 2 och GFP figur 3A), yttre hårcellerna (OHC) (GFP Figur 2) och stödjande celler (GFP Figur 2 och figur 3A)...
