L. monocytogenes is een wijdverspreide en goed gekarakteriseerde menselijke ziekteverwekker, die in staat is om een aantal verschillende ziektemanifestaties te veroorzaken15. Listeria monocytogenes is een grampositieve intracellulaire ziekteverwekker die ernstige ziekten kan veroorzaken bij gevoelige populaties, waaronder ouderen, zwangere vrouwen, mensen met HIV/AIDS en personen die chemotherapiekrijgen 1. Infectie in deze populaties is vaak het gevolg van het innemen van besmette voedingsmiddelen, en de meeste infecties worden geassocieerd met grootschalige door voedsel overgedragen uitbraken2,3. Bij mensen en andere zoogdieren is L. monocytogenes in staat om zich over de epitheelrand van de dunne darm te verplaatsen en vervolgens naar de lever te worden getransporteerd4,5. Diermodellen suggereren dat ingeslikte bacteriën zich repliceren in de intestinale villi en via de poortader naar de lever gaan of zich via de mesenterische lymfeklieren in de bloedstroom verspreiden, wat leidt tot hematogene verspreiding naar de lever en milt6,7. In de lever en milt is de bacterie in staat om de opname in zowel professionele fagocyten als inwonende parenchymale cellen te bemiddelen en stelt snel infecties in deze organen vast. Naarmate de bacteriële belasting toeneemt, worden tal van bacteriën teruggespreid in het bloed, waar ze in staat zijn om gevoelige weefsels verder te koloniseren, waaronder het centrale zenuwstelsel en de placenta (indien aanwezig). Kolonisatie van deze sites sluit de meest voorkomende manifestaties van listeriose bij mensen uit, waaronder meningitis, encefalitis en foetale abortus2.
Van geselecteerde subpopulaties van L. monocytogenes is onlangs aangetoond dat ze een verbeterd vermogen hebben om binnen te dringen en zich te repliceren in hartweefsel8. Manifestaties van hartbetrokkenheid zijn gevarieerd en variëren van endocarditis en pericarditis tot fulminante myocarditis compleet met geleidingsafwijkingen9-13. Het totale aantal L. monocytogenes cardiale gevallen per jaar is laag, maar kan onder schatting zijn omdat dit facet van infectie niet algemeen goed wordt herkend. Kolonisatie van het hart door ziekteverwekkers vereist vaak gastheerpredisposities zoals reeds bestaande valvulaire schade of kunstmatige hartkleppen. Er zijn echter isolaten van L. monocytogenes geïdentificeerd die opmerkelijk zijn vanwege hun vermogen om de harten van geïnfecteerde dieren te koloniseren bij afwezigheid van hartschade en/of afwijkingen8.
Hierin worden in vitro en in vivo methoden beschreven voor het beoordelen van bacteriële kolonisatie van hartweefsel bij geïnfecteerde dieren met behulp van invasietesten in weefselkweek en levende dierinfecties. Deze methoden zijn niet alleen nuttig gebleken voor het identificeren van stammen met het potentieel om hartweefsel bij geïnfecteerde dieren te koloniseren, maar moeten ook nuttig zijn voor de identificatie van bacteriële genproducten die dienen om hartcelinvasie te verbeteren en / of te resulteren in veranderingen in hartpathologie. Deze methoden vergemakkelijken de vergelijking van cardiotropisme tussen meerdere stammen. Voor de hier beschreven methoden wordt L. monocytogenes 10403S gebruikt als een goed bestudeerde vertegenwoordiger van een niet-cardiotrope stam en wordt het klinische isolaat 07PF0776 gebruikt als een representatief voorbeeld van een cardiotrope stam. Deze twee stammen werden gekozen om een vergelijking te bieden voor bacteriële invasie van hartcellen in vitro en kolonisatie van harten van geïnfecteerde muizen in vivo. Het isolaat 07PF0776 is een klinisch isolaat dat is hersteld van een interventratriculair abces dat een fatale aritmie veroorzaakte bij een HIV+ patiënt8. L. monocytogenes isolaten kunnen variëren in hun virulentiepotentieel, en gezien de neiging voor Listeria om personen met immunosuppressie en zwangere vrouwen te infecteren, moeten personen binnen deze populaties voorzichtig zijn bij het beoordelen van verschillende klinische isolaten.

Listeria monocytogenes is een grampositieve facultatieve intracellulaire ziekteverwekker die ernstige invasieve infecties kan veroorzaken bij immuungecompromitteerde patiënten, ouderen en zwangere vrouwen. De meest voorkomende manifestaties van listeriose bij mensen zijn meningitis, encefalitis en foetale abortus. Een significant maar veel minder gedocumenteerd vervolg op invasieve L. monocytogenes infectie betreft het hart. Het sterftecijfer door hartaandoeningen kan oplopen tot 35% ondanks de behandeling, maar er is zeer weinig bekend over L. monocytogenes kolonisatie van hartweefsel en de daaruit voortvloeiende pathologieën. Bovendien is onlangs duidelijk geworden dat subpopulaties van L. monocytogenes een verbeterd vermogen hebben om binnen te dringen en te groeien in hartweefsel. Dit protocol beschrijft in detail in vitro en in vivo methoden die kunnen worden gebruikt voor het beoordelen van cardiotropisme van L. monocytogenes isolaten. Methoden worden gepresenteerd voor de infectie van H9c2-hartstilstandstheoblasten bij ratten in weefselkweek en voor de bepaling van bacteriële kolonisatie van de harten van geïnfecteerde muizen. Deze methoden zijn niet alleen nuttig voor het identificeren van stammen met het potentieel om hartweefsel bij geïnfecteerde dieren te koloniseren, maar kunnen ook de identificatie van bacteriële genproducten vergemakkelijken die dienen om hartcelinvasie te verbeteren en / of veranderingen in hartpathologie te stimuleren. Deze methoden voorzien ook in de directe vergelijking van cardiotropisme tussen meerdere L. monocytogenes stammen.

Geselecteerde isolaten van L. monocytogenes vertonen een verbeterde invasie van hartcellen in celkweek en in muismodellen van infectie. Figuur 1 toont een voorbeeld van hoe bacteriële kolonies kunnen verschijnen na het spotten van suspensies op agarmedia.
