Bien que nous avons décrit comment utiliser CEU chez un rat SCI modèle de contusion, ce protocole peut être modifié pour tenir d&#39;autres objectifs expérimentaux ou modèles SCI. Lésion de la moelle épinière (SCI) est une maladie dévastatrice qui conduit à une altération significative du moteur, sensoriel et les fonctions autonomes. À ce jour, aucun traitement a démontré son efficacité chez les patients. Pour cette raison, il est important d&#39;identifier de nouvelles techniques qui permettront d&#39;améliorer l&#39;évaluation des traitements potentiels et peut encore élucider blessures pathiophysiology 1. SCI est divisé en deux phases successives, dénommé blessures primaires et secondaires. La blessure primaire correspond à l&#39;insulte mécanique initiale. Alors que les groupes de blessures secondaires une cascade d&#39;événements biologiques différents (tels que l&#39;inflammation, le stress oxydatif et l&#39;hypoxie) qui contribuent à la suite de l&#39;expansion progressive de la lésion initiale, des lésions tissulaires et donc le déficit neurologique 2,3. A la phase aiguë de la SCI, thérapies neuroprotectrices visent à réduire la pathologie de la lésion secondaire et should conséquence d&#39;améliorer les résultats neurologiques. Parmi les nombreux événements de blessures secondaires, l&#39;ischémie joue un rôle essentiel de 4,5. Au niveau de l&#39;épicentre SCI, les microvaisseaux du parenchyme endommagés entravent le flux sanguin de la moelle épinière efficaces (FERC). En outre, SCBF est également significativement réduite dans la région entourant l&#39;épicentre de blessure, une zone spécifiquement connu comme la «zone de pénombre ischémique». Si SCBF ne peut pas être rapidement rétablie dans ces régions, l&#39;ischémie peut conduire à une nécrose parenchymateuse supplémentaires et des dommages aux tissus plus nerveux. Comme même la préservation des tissus moindre peut avoir des effets importants de la fonction, il est d&#39;un intérêt majeur pour développer des médicaments et des thérapies qui peuvent réduire l&#39;ischémie post-SCI. Pour mettre en évidence ce phénomène, travaux antérieurs ont montré que la préservation de seulement 10% des axones myélinisés était suffisant pour permettre la marche chez les chats post-SCI 6. Bien que plusieurs techniques ont été décrites pour évaluer SCBF, lay ont tous des limitations importantes. Par exemple, l&#39;utilisation de microsphères radioactives 7,8 et C14-iodopyrine autoradiographie 9 nécessite ultérieure sacrifice animal et ne peut être répétée à plus tard points de temps. La technique de dégagement d&#39;hydrogène 10 dépend de l&#39;insertion d&#39;électrodes intraspinales, qui peut en outre endommager la moelle épinière. Alors que l&#39;imagerie laser Doppler, photopléthysmographie 14,15 et in vivo microscopie optique 16 ont une profondeur / zone très limitée de la mesure 11-13. Notre équipe a déjà montré que contraste amélioré ultrasons (CEU) imagerie peut être utilisée pour évaluer en temps réel et in vivo les changements de SCBF dans le parenchyme rat de la moelle épinière 17. Il est important de noter que une technique similaire a été appliquée par Huang et al., Dans un modèle porcin de 18 SCI. CEU applique un mode spécifique d&#39;imagerie par ultrasons qui permet d&#39;associer im morphologique niveaux de grisâges (obtenus par le mode B classique) avec la distribution spatiale du flux sanguin 19. L&#39;imagerie de SCBF et la quantification repose sur l&#39;injection intravasculaire de produits de contraste pour échographie. L&#39;agent de contraste est constitué de microbulles d&#39;hexafluorure de soufre (de diamètre moyen d&#39;environ 2,5 pm et 90% d&#39;un diamètre inférieur à 6 pm) stabilisées par des phospholipides. Les microbulles reflètent le faisceau d&#39;ultrasons émis par la sonde améliorant ainsi l&#39;échogénicité de sang et augmentant le contraste des tissus en fonction de leur flux sanguin. Il est donc possible d&#39;évaluer le débit sanguin dans une région d&#39;intérêt donnée fonction de l&#39;intensité du signal réfléchi. Les microbulles sont également sans danger et elles ont été appliquées cliniquement chez l&#39;homme. L&#39;hexafluorure de soufre est rapidement effacé (la demi-vie terminale est de 12 min) et plus de 80% de l&#39;hexafluorure de soufre administrée sont retrouvés dans l&#39;air expiré dans les 2 minutes après l&#39;injection. Ce protocole fournit un moyen simple d&#39;utiliser CEU imvieillissement à évaluer les changements SCBF chez le rat.

Réduction du flux sanguin de la moelle épinière (SCBF) (c.-à-ischémie) joue un rôle clé dans une lésion traumatique de la moelle épinière (SCI) pathophysiologie et est donc une cible importante pour les thérapies neuroprotectrices. Bien que plusieurs techniques ont été décrites pour évaluer SCBF, ils ont tous des limitations importantes. Pour surmonter ce dernier, nous proposons l&#39;utilisation du temps réel contraste amélioré échographie (CEU). Ici, nous décrivons l&#39;application de cette technique dans un modèle de contusion de rat de SCI. Un cathéter jugulaire est d&#39;abord implanté pour l&#39;injection répétée d&#39;agent de contraste, une solution de chlorure de sodium de l&#39;hexafluorure de soufre microbulles encapsulées. La colonne vertébrale est alors stabilisé avec un 3D-cadre sur mesure et de la moelle épinière dure-mère est exposée par une laminectomie au THIX-ThXII. La sonde à ultrasons est ensuite positionné à la partie postérieure de la dure-mère (revêtue de gel d&#39;ultrasons). Pour évaluer SCBF base, une seule injection intraveineuse (400 pi) de contrest agent est appliqué pour enregistrer son passage dans le système microvasculaire de la moelle épinière intacte. Un dispositif à chute de poids est ensuite utilisé pour générer un modèle de contusion expérimentale reproductible de SCI. agent de contraste est ré-injecté 15 minutes après la blessure pour évaluer les changements post-SCI SCBF. CEU permet en temps réel et in vivo évaluation des changements de SCBF suivante SCI. Chez l&#39;animal indemne, échographie a montré le flux sanguin inégale le long de la moelle épinière intacte. En outre, 15 min post-SCI, il y avait ischémie critique au niveau de l&#39;épicentre tandis SCBF resté conservé dans les zones les plus reculées intacte. Dans les régions voisines de l&#39;épicentre (à la fois rostrale et caudale), SCBF a été significativement réduite. Cela correspond à la «zone de pénombre ischémique» décrit précédemment. Cet outil est d&#39;un intérêt majeur pour évaluer les effets des thérapies visant à limiter l&#39;ischémie et la nécrose tissulaire résultant à la suite de SCI.

Avec le protocole décrit ci-dessus, il est possible de mapper le SCBF le long d&#39;un segment de la moelle épinière sagittal longitudinal. Dans la moelle épinière intacte, il semble y avoir des irrégularités de SCBF au sein du parenchyme (Figure 12).
