Traumatische Rückenmarksverletzungen (SCI) ist eine verheerende Zustand führt zu erheblichen Beeinträchtigungen in motorischen, sensorischen und autonomen Funktionen. Bisher wurde noch keine Therapie seiner Effizienz bei Patienten nachgewiesen. Für solche Grund ist es wichtig, neue Techniken, die die Bewertung der potenziellen Behandlungen zu verbessern und kann weiter aufzuklären Verletzungen pathiophysiology 1 zu identifizieren. SCI ist in zwei aufeinanderfolgenden Phasen unterteilt, die als primäre und sekundäre Verletzungen. Der primäre Verletzung entspricht der Ausgangs mechanische Beleidigung. Die sekundäre Verletzungen Gruppen eine Kaskade von verschiedenen biologischen Ereignisse (wie Entzündung, oxidativer Stress und Hypoxie), die auf die fortschreitende Expansion des ursprünglichen Läsion, Gewebeschäden und damit neurologisches Defizit 2,3 beizutragen. Bei der akuten Phase der SCI werden neuroprotektive Therapien zur Verringerung der Sekundärverletzung Pathologie und sh gerichtetOuld entsprechend verbessern neurologischen Folgen. Unter den vielen sekundären Verletzungen Ereignisse, spielt eine entscheidende Rolle Ischämie 4,5. Auf der Ebene des SCI Epizentrum, die beschädigten parenchymal Mikrogefäßen behindern effektive Rückenmark Blutfluss (SCBF). Außerdem SCBF wird ebenfalls deutlich in der Region rund um das Epizentrum Verletzungen, einen Bereich speziell als &quot;ischämischen Penumbra Zone&quot; bekannt reduziert. Wenn SCBF nicht schnell in diesen Regionen wieder hergestellt werden kann, um Ischämie Zusatz parenchymal Nekrose und weitere Nervenschäden Gewebes führen. Als auch nur die geringste Gewebekonservierung kann erhebliche Auswirkungen Funktion haben, ist es von großem Interesse, um Medikamente und Therapien, die Ischämie post-SCI zu reduzieren entwickeln. Um dieses Phänomen zu markieren, hat früheren Arbeiten, dass die Erhaltung der nur 10% der markhaltigen Axonen gezeigt war genug, um zu Fuß in Katzen erlauben post-SCI 6. Obwohl mehrere Techniken beschrieben worden, um SCBF, die beurteileny alle haben erhebliche Einschränkungen. Zum Beispiel kann die Verwendung von radioaktiven Mikrosphären 7,8 und C14- iodopyrine Autoradiographie 9 erfordert nachfolgende Tieropfer und kann nicht zu späteren Zeitpunkten wiederholt werden. Die Wasserstoff-Clearance-Technik 10 ist abhängig von der Einfügung intraspinal Elektroden, die weiter beschädigen können das Rückenmark. Während Laser-Doppler-Bildgebung, Photoplethysmographie 14,15 und in-vivo-Lichtmikroskopie 16 haben eine sehr begrenzte Tiefe / Bereich der Messung 11-13. Unser Team hat bereits gezeigt, dass kontrastverstärkten Ultraschall (CEU) Bildgebung verwendet werden, um in Echtzeit zu bewerten und in-vivo die SCBF Veränderungen in der Rattenrückenmark Parenchym 17. Es ist wichtig anzumerken, dass eine ähnliche Technik wurde von Huang et al aufgetragen. In einem Schweinemodell SCI 18. CEU legt eine spezifische Form der Ultraschall-Bildgebung, die in Graustufen morphologischen im verknüpfen erlaubtAlter (nach dem herkömmlichen B-Modus erhalten wird) mit der räumlichen Verteilung der Blutfluss 19. Die SCBF Bildgebung und Quantifizierung beruht auf intravaskuläre Injektion von Echokontrastmittel. Das Kontrastmittel besteht aus Schwefelhexafluorid Mikrobläschen gebildet (mittlerer Durchmesser von etwa 2,5 um und 90% mit einem Durchmesser von weniger als 6 &amp; mgr; m), die durch Phospholipide stabilisierte. Die Mikrobläschen reflektieren die Ultraschallstrahl durch die Sonde somit eine Verbesserung Blut Echogenität und Kontrasterhöhung der Gewebe entsprechend ihrer Blutfluss emittiert. Es ist daher möglich, den Blutfluss in einem bestimmten interessierenden Bereich entsprechend der Intensität des reflektierten Signals zu beurteilen. Die Mikrobläschen sind auch sicher und sie sind klinisch beim Menschen angewendet. Die Schwefelhexafluorid wird schnell abgeräumt (mittlere terminale Halbwertszeit beträgt 12 min) und mehr als 80% der verabreichten Schwefelhexafluorid wird in der ausgeatmeten Luft innerhalb von 2 Minuten nach der Injektion gewonnen. Reduziert Rückenmark Blutfluss (SCBF) (dh Ischämie) spielt eine Schlüsselrolle bei der traumatischen Rückenmarksverletzungen (SCI) Pathophysiologie und ist daher ein wichtiges Ziel für neuroprotektive Therapien. Obwohl mehrere Techniken beschrieben worden, um SCBF bewerten, sie haben alle signifikante Einschränkungen. Um letzteres zu überwinden, schlagen wir die Verwendung von Echtzeit-kontrastverstärkten Ultraschall-Bildgebung (CEU). Hier beschreiben wir die Anwendung dieser Technik in einem Rattenmodell der Prellung SCI. A Jugularkatheter zunächst für die wiederholte Injektion von Kontrastmittel, einer Natriumchloridlösung von Schwefelhexafluorid eingekapselten Mikrobläschen implantiert. Der Dorn wird dann mit einem maßgeschneiderten 3D-Rahmen stabilisiert und das Rückenmark Dura mater durch eine Laminektomie bei THIX-ThXII belichtet. Die Ultraschallsonde wird dann an der Hinterseite der Dura mater (mit Ultraschall Gel beschichtet) positioniert ist. Zum Ausgangswert SCBF, eine einzelne intravenöse Injektion (400 ul) contra bewertenst Mittel angelegt wird, um seinen Durchgang durch die intakte Rückenmarks microvasculature aufzuzeichnen. Ein Gewichtsfangvorrichtung wird anschließend verwendet, um eine reproduzierbare experimentelle Quetschung Modell SCI generieren. Kontrastmittel wieder injiziert 15 Minuten nach der Verletzung, um post-SCI SCBF Veränderungen zu beurteilen. CEU ermöglicht die Echtzeit und in-vivo-Bewertung der SCBF Veränderungen nach SCI. In der unverletzten Tier zeigte Ultraschall-Bildgebung ungleichmäßige Blutfluss entlang der intakten Rückenmark. Außerdem, 15 min post-SCI, gab es kritische Ischämie auf der Ebene des Epizentrums während SCBF blieb in den abgelegeneren Gebieten intakt bewahrt. In den Bereichen neben dem Mittelpunkt (beide rostral und kaudalen) SCBF signifikant reduziert wurde. Dies entspricht dem vorher beschriebenen &quot;ischämischen Penumbra zone&quot;. Dieses Tool ist von großem Interesse für die Beurteilung der Auswirkungen von Therapien zur Begrenzung Ischämie und die resultierende Gewebsnekrose im Anschluss an SCI ausgerichtet.

Mit der vorstehend beschriebenen Protokoll ist es möglich, die SCBF entlang einer longitudinalen Rückenmarks sagittalen Segment zuzuordnen. Im intakten Rückenmark scheint es SCBF Unregelmäßigkeiten im Parenchym (Figur 12) sein.
