Deze techniek maakt gebruik van niet-invasieve observatie van de mechanische respons van zacht weefsel SC instorten kwantificeren. Toekomstige werkzaamheden met behulp van menselijke kadaver ogen nodig is om weefsel doorbuigingen kalibreren werkelijke weefsel stijfheid na dissectie. Maar, deze studies dezelfde beperkingen van eerdere uitstroom modellen lijden; bepaald dat de bijdragen van levend spier weefsel spanning niet aanwezig zal zijn. Glaucoom is wereldwijd de tweede belangrijkste oorzaak van onomkeerbare blindheid 1. Verhoogde intraoculaire druk (IOP) een belangrijke causale risicofactor voor de aanwezigheid en progressie van glaucoom 2-7. &#160; IOP wordt geregeld door het evenwicht tussen de vorming en de uitstroom van waterige humor 8. De locaties van de grootste uitstroomweerstand de juxtacanicular weefsel en de binnenwand van het kanaal van Schlemm (SC), het raakvlak tussen SC en het trabeculaire netwerk (TM) 9-11. Hoewel TM stijfheid kan bijdragen tot het voorkomen van SC ineenstorting tegenover IOP verhoging, Overby et al. 12 onlangs aangetoond dat genexpressie in glaucoom wordt veranderd, resulterend in verhoogde SC endotheliale verstijving, belemmeren de vorming van poriën, wat leidt tot verhoging van IOP 13 ogen met glaucoom. TM morfologie en stijfheid correleren met uitstroom faciliteit 14,15, met de nadruk thij nodig heeft om zijn biomechanische eigenschappen te meten. Atomic force microscopie metingen van de TM vertonen verhoogde stijfheid in ogen geschonken door glaucoompatiënten (81 kPa) in vergelijking met de ogen van donoren zonder glaucoom (4,0 kPa) 16, maar deze metingen zijn uitgevoerd ontleed ex vivo weefsel. De achterste TM is verankerd in de ciliaire spier via anterior pezen van de longitudinale spiercellen die voegen in de buitenste gelamelleerde en cribiform TM 17. Spieractiviteit Ciliaire (CM) kan TM tautness verhogen, het nabootsen van een verhoogde stijfheid TM 17. Het vermogen om veranderingen waarnemen in resistentie tegen SC instorting veroorzaakt door verstoringen van glad spierweefsel is aangetoond in een diermodel 18. We hebben de mogelijkheid om niet-invasief imago van de primaire waterige humor uitstroom systeem in levende menselijke ogen distale tot en met SC gebruik spectrale domeinnaam optical coherence tomography (OCT) toonden &lt;sup&gt; 19-21. Met deze techniek, hebben we de mogelijkheid om de morfometrische reactie van de TM en SC acute IOP hoogte 22 kwantificeren aangetoond. Het algemene doel van de hierin beschreven werkwijze is de reactie van de morfometrische levende uitstroomkanaal acute IOP verhoging in levend weefsel in situ gekwantificeerd. Deze techniek heeft het voordeel dat het onderzoek van de TM onder fysiologische condities, die bijdragen zowel contractiele vezels activiteit in de TM en CM te TM stijfheid omvat, vergeleken met gepubliceerde metingen in ontleed weefsel. De grondgedachte achter het toepassen van deze techniek aan de waarneming van de mechanische TM reactie is dat het ons anders niet beschikbaar inzicht in het mechanisch gedrag van de TM, waarvan we weten nu direct worden gekoppeld aan uitstroom weerstand en IOP regulering 13. Om de bijdrage van contractiele weefsel stijfheid, een coho onderscheidenrt van de proefpersonen werd onderzocht zonder en met onderdrukking van gladde spieractiviteit door toediening van tropicamide.

trabecular meshwork response to pressure elevation in the living human eye

De mechanische eigenschappen van het trabeculaire netwerk (TM) gekoppeld aan uitstroomweerstand en intraoculaire druk (IOP) regulering. De grondgedachte achter deze techniek is de directe waarneming van de mechanische reactie van de TM acute IOP hoogte. Voorafgaand aan het scannen, IOP wordt gemeten op baseline en tijdens de IOP hoogte. De limbus wordt gescand door spectrale domein optische coherentie tomografie op baseline en tijdens de IOP elevatie (ophthalmodynamometer (ODM) toegepast bij 30 g kracht). Scans worden verwerkt om visualisatie van de waterige humor uitstroom traject met behulp ImageJ verbeteren. Vasculaire monumenten worden gebruikt om overeenkomstige locaties in baseline en IOP hoogte scan volumes te identificeren. Schlemm kanaal (SC) dwarsdoorsnedeoppervlak (SC-CSA) en SC lengte van anterior naar posterior langs zijn lengteas worden handmatig gemeten op 10 locaties binnen 1 mm segment van SC. Mean binnen naar buiten wandafstand (korte lengte-as) wordt berekend als het oppervlak van SC gedeeld door delange lengte-as. Om de bijdrage van aangrenzende weefsels naar het effect IOP verhogingen onderzocht worden metingen herhaald zonder en met gladde spierrelaxatie met instillatie van tropicamide. TM migratie naar SC wordt tegengegaan door TM stijfheid, maar wordt versterkt door de ondersteuning van haar gehechtheid aan aangrenzende gladde spieren in het corpus ciliare. Deze techniek is de eerste die de levende humane TM reactie op drukverhoging in situ onder fysiologische omstandigheden in het menselijk oog te meten.

Met deze data acquisitie en beeldanalyse technieken worden de effecten van kleine en grote veranderingen in IOP op uitstroomkanaal morfologische parameters zoals SC dwarsdoorsnede verkregen (Figuur 1). We kunnen zien dat hoge niveaus van IOP toename produceren een waarneembare ineenstorting van SC, vertegenwoordigd door een sterke daling van de dwarsdoorsnede. Het oog lijkt te kunnen kleine stijgingen in IOP accommoderen, zoals blijkt uit het ontbreken van verandering in SC-CSA (figuur 1).</stro...
