Maladie de Crohn (MC) est une maladie inflammatoire chronique commune du petit et du gros intestin et est probablement le résultat d’une réponse inappropriée du système immunitaire hôte à microbes intestinaux7,8. Des études récentes ont montré que les murins et humains cellules souches mésenchymateuses (CSM) peut supprimer l’inflammation dans les modèles murins d’inflammation intestinale1,3,4,5. Il y a plusieurs essais cliniques en cours qui utilisent MSCs humaines dérivées de la moelle osseuse ou du tissu adipeux pour traiter les patients atteints de maladie intestinale inflammatoire (MII), qui comprend le CD9. Deux itinéraires pour la thérapie de la CSM ont été utilisées dans ces essais cliniques : l’un implique la perfusion systémique (c.-à-d., par voie intraveineuse) de MSCs pour EIA luminale (y compris les CD) et l’autre consiste en l’application localisée/injection de cellules souches dans la fistule tract de patients avec CD périanale. Dans une récente méta-analyse de thérapie MSC pour EIA, systémique (c.-à-d., par voie intraveineuse) thérapie MSC pour EIA luminale (y compris les CD) a été efficace chez jusqu'à 40 % (IC à 95 % : 7-79 %) des patients, alors que l’efficacité était beaucoup plus élevée, à 61 % ([IC95 : : 36-85 %), quand les MSCs ont été injectés localement à la malade de fistule CD9. Multicentrique récente phase III randomisée contrôlée contre placebo d’allogéniques cellules souches adipeuses injecté directement dans la fistule périanale de CD patients a montré des preuves cliniques et radiologiques statistiquement significative des fistules périanales, guérison, corroborant les résultats de la méta-analyse de10. Les raisons de la faible efficacité de la thérapie MSC administrée par voie intraveineuse pour CD luminal a été suffisamment examinés, mais une des raisons est peut-être le homing inadéquat de MSCs sur le site de l’inflammation. Murines études sur des modèles d’inflammation du côlon ont montré que seulement un faible pourcentage de MSCs (1-5 %) injecté par voie intraveineuse atteignent le côlon enflammé ; les MSCs restants sont filtrées par les poumons (effet de premier passage)1,2 ,5,11,12. Plusieurs études de recherche murins ont donc utilisé la voie intrapéritonéale (i.p.) pour l’administration de MSC dans les modèles animaux de colite4. Cependant, ils ont également démontré que seulement une petite fraction des cellules atteignent et greffer le côlon et que l’efficacité est liée à la sécrétion de facteurs solubles paracrine, comme des gènes inductibles par le facteur de nécrose tumorale 6 protéines (STG-6)2. Le mécanisme MSC de l’immunosuppression et la guérison implique une approche multiforme qui implique paracrine ; facteurs de proximité indépendant de cellules, comme STG-6 ; facteurs de proximité-dépendante, de la cellule comme programmé mort-ligand 1 (PD-L1) ; ou en escalier 1. Donc, localisation MSC sur le site de l’inflammation peut entraîner une augmentation efficacité9,13. En effet, une étude récente montre que MSCs directement implantés à l’endroit de la blessure du côlon dans la guérison en sécrétant l’angiogenèse favorisant vasculaire facteur de croissance endothéliale (VEGF). En revanche, un effet minime de guérison a été noté après injection intraveineuse de5. Pour augmenter leur localisation sur le site de l’inflammation (c.-à-d., l’intestin grêle chez les souris SAMP), cette technique d’injection intracardiaque guidée par échographie pour l’administration de MSC dans le ventricule gauche a été développée. Guidée par l’image injection assure une injection précise, ce qui conduit à un taux de réussite supérieur à diminué le taux de mortalité et de morbidité. Par ailleurs, l’injection de MSCs dans le ventricule gauche remet à la circulation artérielle, où ils peuvent atteindre l’intestin enflammé avant de devenir emprisonné dans les poumons.
Dans cette étude, MSCs de moelle osseuse humaines (CSM) ont été utilisées pour l’injection dans le modèle murin de SAMP-1/YitFc (SAMP) du CD14. SAMP est un modèle murin spontané bien caractérisé d’inflammation chronique qui développe l’inflammation intestinale-petit avec presque 100 % de pénétrance14. L’inflammation se développe en réponse à la microflore en l’absence de toute manipulation génétique, immunologique ou chimique et ressemble à humain CD11. Souris de la AKR/J (AKR) inflammation-free Sex - et appariés selon l’âge, les souris contrôle parental du SAMP, ont été utilisées dans cette étude.
Le CSM ont été isolés et élargi en laboratoire des échantillons de moelle osseuse (BM) provenant de donneurs normaux, non identifiés après que consentement éclairé à l’aide de validé et publié précédemment protocoles15,16. Après isolement et l’expansion, la capacité MSC en ostéogénique, adipocytaire, et différenciation chondrogéniques a été évaluée en laboratoire par plusieurs essais sur15. Le test fonctionnel ostéogénique a été réalisé par l’implantation des cubes en céramique d’une matrice hydroxyapatite/tricalcium phosphate contenant du CSM par voie sous-cutanée dans les sujets immunodéprimés de souris SCID CB17-Prkdc17. Le test de cube montre ostéogenèse et chondrogenèse potentiels et est considéré comme l’ultime épreuve d’évaluation individuelle MSC préparations17. Pour visualiser le CSM en vivo après l’injection, les lentivirus servait à transduce CSM avec construction de gène de journaliste triple qui se compose de la luciférase firefly (fl), protéine fluorescente rouge monomérique (RDRF) et l’herpès simplex thymidine kinase virale (ttk ), conduit par un sarcome myéloprolifératifs mis à jour le virus (mnd) promoteur18. La luciférase firefly dans le journaliste triple est une enzyme que luciférine injecté tectrices à oxyluciferin au CSM et produit des photons/blanc bleu. Ceci est détecté par la caméra sensible dispositif à couplage de charge (CCD) (bioluminescence) dans in vivo d’imagerie système, qui permet la visualisation du CSM direct chez la souris optique. Bioluminescence imaging (BLI) est une technique sensible qui peut être utilisée en série pour le suivi des cellules et pour l’analyse des ex vivo . L’utilisation d’un promoteur fort MINISTERIELLE entraîne l’expression continue de la construction de gène de journaliste triple fusion et permet l’imagerie des CSM injectée pendant plus de 16 semaines19. Le CSM est difficiles à transmettre et ont un rendement faible de transduction. En utilisant un protocole optimisé, l’efficacité de la transduction du CSM a été améliorée et l’expression du transgène a été renforcée18. À l’aide d’expression RDRF (un des gènes journaliste triple) sur la cytométrie en flux, la capacité à transmettre le CSM à rendement élevé jusqu'à 83 % a été démontrée. Différenciation des essais et des essais in vivo cube démontré la capacité de la CSM transduite à se différencier en chondrocytes, les adipocytes et les ostéocytes,17.

ultrasound-guided intracardiac injection of human mesenchymal stem cells to increase homing to the intestine for use in murine models of experimental inflammatory bowel diseases

Maladie de Crohn (MC) est une maladie inflammatoire chronique commune du petit et du gros intestin. Murins et humains cellules souches mésenchymateuses (CSM) ont un potentiel immunosuppresseur et auraient dû être divulgués pour supprimer l’inflammation dans les modèles murins d’inflammation intestinale, même si la voie d’administration peut limiter leur homing et efficacité 1 , 3 , 4 , 5. application locale de MSCs aux modèles de lésion colique a montré une efficacité supérieure à atténuer l’inflammation du côlon. Cependant, il y a peu de données sur les techniques pour améliorer la localisation de MSCs de moelle osseuse humaines (CSM) à l’intestin grêle, le site de l’inflammation dans le modèle de SAMP-1/YitFc (SAMP) maladie de Crohn expérimentale. Cet ouvrage décrit une nouvelle technique d’injection intracardiaque guidée par échographie de CSM chez les souris de la SAMP, un modèle bien caractérisé spontané de l’inflammation intestinale chronique. Sexe - et appariés selon l’âge, les souris AKR/J (AKR) exempte d’inflammation ont été utilisés comme témoins. Pour analyser la biodistribution et la localisation, les CSM ont été transduites avec un lentivirus contenant un journaliste triple. Le journaliste triple se composait de la luciférase firefly (fl), pour l’imagerie bioluminescente ; protéine fluorescente rouge monomérique (RDRF), pour le tri des cellules ; et tronquée de l’herpès simplex virus thymidine kinase (ttk), pour l’imagerie par émission de positrons (TEP). Les résultats de cette étude montrent que 24 h après l’administration intracardiaque, CSM localiser dans l’intestin des souris SAMP par opposition à exempte d’inflammation des souris AKR. Ce roman, injection guidée par ultrason du CSM dans le ventricule gauche de souris SAMP assure un taux de réussite élevé de livraison de cellule, permettant la récupération rapide des souris avec une mortalité et une morbidité minime. Cette technique pourrait être une méthode utile pour la localisation accrue de MSCs dans d’autres modèles d’inflammation intestinale grêle, tels que TNFΔRE,6. Les études à venir détermineront si la localisation accrue du CSM par voie intra-artérielle peut conduire à une augmentation efficacité thérapeutique.

The overall goal of this procedure is to use ultrasound-guided intracardiac human bone marrow derived mesenchymal stem cell or HMSC injection in a mouse model of experimental crohn's disease, to observe HMSC homing to the intestine. This method can help answer the key questions in the inflammatory bowel disease field, about the need for increased localisations of HMSC to small intestine during inflammation. The main advantage of this technique is that it facilitates a high HMSC delivery success rate with minimal mouse mobility and mortality.Generally, individuals new to this method can have trouble because multiple variables, they can affect the procedure outcome. Requiring a lot of trainee understanding of human mesenchymal stem cell biology. Demonstrating the procedures will be Paola Menghini, a post doc from our laboratory.In a specific pathogen free facility, set up the mouse echocardiogram ultrasound imaging system protocol and initialize the 30 megahertz transducer. Next, confirm the appropriate level of sedation by toe pinch and apply ointment to the animal's eyes. Use depilatory cream to remove the hair over the thorax and place the animal on the 37 degree celsius imaging table in the supine position.Tape the limbs to prevent movement during the procedure and disinfect the exposed skin of the thorax with 10%povidone iodine and 70%ethanol. Apply a thick layer of 37 degree celsius ultrasound gel to the skin and mount the transducer in the holder, adjusting the position of the transducer until the left ventricle is clearly visible in the field of view. Next, gently re-suspend 2 x 10 to the sixth transduced human mesenchymal stem cells in 150 microliters of sterile PBS to generate a homogenous cell suspension and load the cells into a one milliliter syringe equipped with a 28 gauge needle.Inject less than 150 microliters of cell suspension per mouse to avoid a volume overload and death. Secure the syringe in a syringe holder. Then, advance the syringe towards the mouse thorax using the ultrasound guidance to adjust the needle trajectory as necessary and penetrate the needle tip through the intercostal space into the left ventricle.Adequate visualization of the needle on the ultrasonographic image is critical for ensuring a proper trajectory and insertion of the needle into the left ventricle, a successful injection and the avoidance of cardiac dampening and death. The tip should be clearly visible within the left ventricle and proper placement can be further confirmed by fresh arterial blood outflow into the syringe. Apply a gentle pressure to the plunger to slowly inject the HMSC suspension over a period of two minutes.When all the cells have been injected, gently withdraw the needle and place the animal in a clean cage on a preheated pad with monitoring until full recovery. 24 hours after the injection, initialize the bioluminescence imaging system and open a new study in the in-vivo imaging software. To set the imaging parameters in the control panel, click the sequence set up.In the imaging mode, select luminescent and photograph. Set the exposure times from 0.5 seconds to 10 minutes, the bidding to medium and the f/stop to one. Set the excitation filter to block, the emission filter to open and the field of view to see for two mice.Then click on the acquisition control panel to add the sequence set up to the image wizard. Next, inject the first mouse with 300 microliters of d-luciferin. Apply ointment to the animals'eyes and transfer the mouse to the in-vivo imaging chamber.10 minutes after the d-luciferin injection, click acquire, to begin the image acquisition. After all the mice have been imaged, harvest the whole gastrointestinal tract, mesenteric lymph nodes, lungs, spleen and liver from each animal. Then image the organs as just demonstrated, for whole animal bioluminescence imaging.HMSC can be transduced with the triple reporter at a high efficiency preserving the stem cell properties and allowing their visualization in real time by bioluminescence imaging. Ex-vivo analysis of the intracardiac delivered stem cells, confirms that this root of administration facilitates HMSC homing to the inflamed small intestines of the experimental crohn's disease mice. HMSC is injected into control, inflammation free animals however, do not accumulate in the small intestine.Once mastered, this technique can be completed in less than 15 minutes per mouse if it is performed properly. While attempting this procedure, it's important to remember to use the ultrasound image to adjust the needle trajectory. A poor technique and an incorrect trajectory can lead to cardiac tamponade from hemorrhagic pericardial effusion and death.Following this procedure, other methods like histology and 3D studio microscopy can be performed to answer additional questions such as, does an increased homing of HMSC to the small intestine augment that therapeutic efficacy. This technique will allow researchers in the field of experimental IBD to explore all their mouse models like TNF trajectory, can be injected with HMSCs to enhance homing to the small intestine. After watching this video, you should have a good understanding of how to perform ultrasound guided intracardiac injection of HMSC in mice and of how to use bioluminescent imaging to analyze a bad distribution and homing.Don't forget that working with human MSCs can be hazardous and that precautions such as, using gloves and a face mask with a shield, should always be taken while performing this procedure.
