Hier beschrijven we een experimentele procedure bij schapen, gericht op het nabootsen van vaginale dissectie en transvaginale meshinzetting van een implantaat met of zonder verankering van armen. De daaropvolgende stappen en instrumenten werden geïnspireerd door chirurgische procedures die werden uitgevoerd voor POP en stress urine incontinentie 15 , 16 , 19 , 20 . Pelvic orgaan prolaps wordt klinisch gediagnosticeerd bij de helft van de vrouwen die tenminste één vaginale aflevering hadden, maar ondervindt het de helft van de vrouwen over het algemeen 1 . De basis van de therapie is chirurgische reconstructie met gebruikmaking van inheemse weefsel- of implantaatmaterialen, maar elk van deze methoden heeft zijn beperkingen, waaronder herhaling of lokale complicaties 2 , 3 , 4 . Het ideale implantaat is nog niet geïdentificeerd; Daarom is er een voortdurende vraag naar productinnovatie en voor de ontwikkeling van een goede pijpleiding voor preklinische experimenten voor de introductie van nieuwe producten en technieken op de markt. Een van de stappen in deze track is experimentele evaluatie van geschikte diermodellen 5 , 6 . Ideaal gezien zouden ze de anatomische, biomechanische en biologische omgevingen moeten nabootsen. Als het gaat om thE experimentele evaluatie van nieuwe implantaten, worden ze typisch getest eerst in kleinere modellen, ook voor biocompatibiliteit of voor de reconstructie van buikwandafwijkingen. Dat soort experimenten zijn gekritiseerd, omdat de implantaten niet in het gebied van belang worden gebracht ( dwz de vagina) 7 . Vaginale chirurgie modellen zijn minder schaars, zeker als het doel van het experiment is om de biomechanische eigenschappen van explanten te documenteren. Om deze reden was er een beweging van konijnen naar schapen 8 . Volwassen ooien zijn grote dieren modellen met een redelijk grote en toegankelijke vagina. Ze kunnen gebruikt worden voor de tussentijdse evaluatie van nieuwe implantaten, en het is mogelijk om vaginale blootstellingen te reproduceren met bepaalde materialen 9 , 10 , 11 , 12 , 13 . Niet alleen de afmetingen en de anatomieVan de schapenvagina en de bekkenvloer zijn vergelijkbaar met die van de mens, maar ook het spontane voorkomen van prolaps, die zich voordoet bij 15% van de ooien. Prolapse risicofactoren zijn overlappend ( dwz multipariteit, eerdere geschiedenis van POP, verhoogde intra-abdominale druk geïnduceerd door een hoger lichaamsgewicht of bij grazen op heuvels en vergelijkbare effecten van (fyto) oestrogenen) 6 , 14 . In Europa zijn schapen het enige redelijke alternatief, omdat onderzoek naar niet-menselijke primaten bijna volledig verbannen is. Hier werd het model nog een stap verder genomen door de transvaginale insertie van implantaten na te bootsen met behulp van trocars en gidsen voor de spanningsvrije plaatsing van mazen in het recto-vaginale septum. Dit werd gevolgd door het implantaat te bevestigen door middel van verankering met armen door de spieren van de spieren, die kan worden beschouwd als equivalent aan de klinische praktijk 15 , 16 . Tot nu toe, deze techniekIs niet onderzocht, hoewel velen geloven dat specifieke complicaties kunnen optreden als gevolg van het gebruik van deze langere stroken en / of het doordringen van anatomische structuren. In een eerder gedetailleerde anatomische studie werd de bekkenbodem vergeleken met het vrouwelijke bekken 17 . Bij het verankeren van het implantaat hebben schapen geen sacrospinous ligament, maar ze hebben wel een zeer goed ontwikkelde en brede sacrotuberose ligament. De pudendale zenuw loopt ventraal over het, waardoor het onveilig is om dit landmerk als schorspunt te gebruiken. Omgekeerd zijn de coccygeusspier en zijn fascia, evenals het obturatormembraan, toegankelijk via de rectovaginale ruimte. Hier wordt de toegang en positie van de anatomische structuren voor de fixatie van verankeringsarms voorgesteld. De instrumenten die kunnen worden gebruikt om het netwerk te positioneren worden besproken. Ten slotte, de relatie van de armen of trocars tot aangrenzende anatomische structuren, zoals vaten en zenuwen, aZo goed als mogelijke intraoperatieve complicaties, worden ook beschreven.

transvaginal mesh insertion in the ovine model

Dit protocol beschrijft maasinvoeging in het rectovaginale septum in schaap met behulp van een enkele vaginale incisie techniek, met en zonder de trocar-geleide invoeging van verankeringsarms. Parous schapen ondergaan de dissectie van het rectovaginale septum, gevolgd door het inbrengen van een implantaat met of zonder vier verankeringsarmen, beide ontworpen om de schapenanatomie te passen. De verankeringswapens werden in gebruik genomen met behulp van een trocar en een &quot;buiten-in&quot; techniek. De kraniale armen werden door de obturator, gracilis en adductor magnus spieren geleid. De caudale armen werden bevestigd in de buurt van het sacrotuberale ligament, door de coccygeusspieren. Deze techniek zorgt voor het nabootsen van chirurgische ingrepen die worden uitgevoerd bij vrouwen die lijden aan baarmoederorgaan prolaps. De anatomische ruimten en elementen worden gemakkelijk geïdentificeerd. Het meest kritische deel van de procedure is de invoeging van de kraniale trocar, die gemakkelijk de peritoneale holte of de omliggende bekkenorganen kan binnendringen. ThiS kan worden voorkomen door een uitgebreide retroperitoneale dissectie en door de trocar meer zijdelings te begeleiden. Deze aanpak is uitsluitend bedoeld voor het experimenteel testen van nieuwe implantaten in grote dierenmodellen, aangezien de doorgevoerde insertie momenteel niet klinisch gebruikt wordt.

Beheer in een langere waarnemingsinstelling Na de chirurgische procedure kan vaginale verpakking (een zoutoplossing in de vagina onmiddellijk na de operatie ingebracht worden) 24 uur lang ingevoegd worden om de implantaatpositie te beveiligen.
