Aqui, descrevemos um procedimento experimental em ovelhas, com o objetivo de imitar a dissecção vaginal e inserção de malha transvaginal de um implante com ou sem braços de ancoragem. As etapas e instrumentos subsequentes foram inspirados por procedimentos cirúrgicos realizados para POP e incontinência urinária de estresse 15 , 16 , 19 , 20 . O prolapso do órgão pélvico é clinicamente diagnosticado na metade das mulheres que tiveram pelo menos um parto vaginal, mas subjetivamente, incomoda a metade das mulheres em geral 1 . O principal suporte da terapia é a reconstrução cirúrgica usando tecido nativo ou material de implante, mas cada um desses métodos tem suas limitações, incluindo recorrência ou complicações locais 2 , 3 , 4 . O implante ideal ainda não foi identificado; Portanto, existe uma demanda contínua de inovação de produtos e para o desenvolvimento de um oleoduto adequado para experimentação pré-clínica antes da introdução de novos produtos e técnicas para o mercado. Uma das etapas desta pista é a avaliação experimental em modelos animais adequados 5 , 6 . Idealmente, eles devem imitar os ambientes anatômicos, biomecânicos e biológicos. Quando se trata deE avaliação experimental de novos implantes, eles geralmente são testados primeiro em modelos menores, seja para biocompatibilidade ou para a reconstrução de defeitos da parede abdominal. Esse tipo de experimentos foi criticado, porque os implantes não estão inseridos na área de interesse ( ou seja, a vagina) 7 . Os modelos de cirurgia vaginal são mais escassos, certamente quando o objetivo do experimento é documentar as características biomecânicas dos explantes. Por esse motivo, houve um movimento de coelhos para ovelhas 8 . As ovelhas adultas são modelos de animais grandes com uma vagina de tamanho razoável e acessível. Eles podem ser usados ​​para a avaliação intercalar de novos implantes, e é possível reproduzir exposições vaginais com certos materiais 9 , 10 , 11 , 12 , 13 . Não só as dimensões e a anatomiaDa vagina ovina e do assoalho pélvico são comparáveis ​​aos humanos, mas também a ocorrência espontânea de prolapso, que ocorre em 15% das ovelhas. Os fatores de risco de prolapso são sobrepostos (por exemplo , multipartite, histórico prévio de POP, aumento da pressão intra-abdominal induzida por maior peso corporal ou quando pastoreia em colinas e efeitos comparáveis ​​de (fito) estrogênios) 6 , 14 . Na Europa, as ovelhas são a única alternativa razoável, já que a pesquisa sobre primatas não humanos foi quase completamente banida. Aqui, o modelo foi levado um passo adiante imitando a inserção transvaginal de implantes usando trocartes e guias para a colocação livre de tensão de malhas no septo recto-vaginal. Seguiu-se a fixação do implante usando ancoragem com os braços através dos ligamentos dos músculos, o que pode ser considerado equivalente à prática clínica 15 , 16 . Até agora, esta técnicaNão foi estudado, embora muitos acreditam que podem ocorrer complicações específicas devido ao uso dessas tiras mais longas e / ou ao piercing de estruturas anatômicas. Quando se trata de ancorar o implante, as ovelhas não possuem o ligamento sacrospinoso, mas possuem um ligamento sacrotubero bem desenvolvido e amplo. O nervo pudendo corre ventralmente sobre ele, tornando-o inseguro para usar esse marco como um ponto de suspensão. Por outro lado, o músculo coccygeus e sua fáscia, bem como a membrana obturadora, são acessíveis através do espaço rectovaginal. Este protocolo descreve a inserção de malha no septo rectovaginal em ovelhas usando uma única técnica de incisão vaginal, com e sem a inserção guiada por trocares de braços de ancoragem. As ovelhas parosas foram submetidas à dissecção do septo rectovaginal, seguido da inserção de um implante com ou sem quatro braços de ancoragem, ambos destinados a caber a anatomia ovina. Os braços de ancoragem foram colocados usando um trocar e uma técnica de &quot;fora-dentro&quot;. Os braços cranianos foram passados ​​através dos músculos obturador, gracilis e adutor magnus.
