TDT Measurement and Analysis
Twee vormen van apparatuur (tafelblad en headset), twee methoden van stimulans presentatie (trap en willekeurig), en twee benaderingen van data-analyse (traditionele en distributie) zijn aangebracht om te illustreren hoe te meten en kwantificeren van een persoon's temporele discriminatie vermogen. Temporele discriminatie beschrijving van iemands vermogen om te discrimineren, of waarnemen, snelle veranderingen in hun omgeving. De temporele discriminatie drempel (TDT) is de kortste tijdsinterval waarop een individu waarnemen kan dat twee opeenvolgende zintuiglijke prikkels asynchroon zijn. Temporele discriminatie is aangetoond dat abnormaal worden verlengd in aandoeningen op het gebied van de basale ganglia, met inbegrip van dystonie1,2,3,4,5,6 , 7.
Dystonie is de derde meest voorkomende neurologische bewegingsstoornis - na Parkinson's ziekte en essentiële Tremor. Het wordt gekenmerkt door duurzame of intermitterende spiercontracties veroorzaakt door abnormale, vaak herhaalde bewegingen of houdingen8. Dystonie kan beïnvloeden elk deel van het lichaam. Wanneer het van invloed is op een lichaamsdeel is het bekend als focale dystonie8. Dystonie beïnvloeden de nekspieren cervicale dystonie wordt genoemd, en is de meest voorkomende fenotype van volwassen begin idiopathische geïsoleerde focale dystonie. 9 , 10 de pathogenese van cervicale dystonie blijft onbekend; het wordt beschouwd als een genetische stoornis met autosomaal dominante overerving en sterk verminderde doordringendheid. Omgevingsfactoren zijn eveneens belangrijk in verband met ziekte doordringendheid en expressie.
De superieure colliculus, een sensomotorische structuur, gelegen in de dorsale veroorzaakt, is belangrijk voor de snelle detectie van milieu stimuli in het proces van heimelijke attentional oriënterende2,11,12. Visuele stimuli toegang krijgen tot de superieure colliculus snel via de blastoma-tectal magnocellular-pathway. De TDT is een maatregel van het eenvoudige, objectieve verondersteld te vertegenwoordigen van de verwerking van visuele (en andere zintuiglijke stimuli) in de oppervlakkige lagen van de superieure colliculus. De TDT is onderzocht bij personen met cervicale dystonie, hun onaangetast familieleden en gezonde controle deelnemers. Vergeleken met de leeftijd en geslacht-zoekwoorden controle deelnemers, een abnormale TDT heeft hoge gevoeligheid (97%, 36 van 37 patiënten) en specificiteit (98-100%) in de cervicale dystonie1. Een abnormale TDT is gebleken dat bij 50% van onaangetast eerstegraads vrouwelijke familieleden van patiënten met cervicale dystonie (14 25, van 48 jaar of ouder), demonstreren en seks-leeftijdsgebonden doordringendheid met autosomaal dominante overerving13, 14. een abnormale TDT in onaangetast familieleden van patiënten van de cervicale dystonie (vergeleken met familieleden met normale TDTs) wordt geassocieerd met verhoogde putaminal volume (door voxel gebaseerde morphometry)15 en putaminal activiteit (met fMRI) verminderd4 . De superieure colliculus wordt beschouwd als een belangrijk knooppunt in de neuronale netwerk, dat disfunctioneel in cervicale dystonie12 is. De beoordeling van de temporele discriminatie wordt beschouwd als het verstrekken van belangrijke aanwijzingen over de pathomechanisms onderliggende cervicale dystonie.
Het doel van dit artikel is te presenteren van twee methoden voor het meten en analyseren van temporele discriminatie, alsmede het demonstreren van de toepassing van deze methode op het bestuderen van de pathofysiologie van cervicale dystonie.

De temporele discriminatie drempel (TDT) is de kortste tijdsinterval waartegen een waarnemer onderscheid twee opeenvolgende prikkels als asynchrone (doorgaans 30-50 ms maken kan). Het is aangetoond dat abnormale (langdurige) in neurologische stoornissen, met inbegrip van cervicale dystonie, een fenotype van volwassen begin idiopathische geïsoleerde focale dystonie. De TDT is een kwantitatieve maat voor de mogelijkheid om het waarnemen van snelle veranderingen in de omgeving en wordt beschouwd als indicatie van het gedrag van de visuele neuronen in de superieure colliculus, een belangrijk knooppunt in het heimelijke attentional oriënteren. Dit artikel bevat methoden voor het meten van de MVision (inclusief twee hardwareopties en twee modi van stimuli presentatie). We verkennen ook twee benaderingen van data-analyse en TDT berekening. De toepassing van de beoordeling van de temporele discriminatie aan het begrijpen van de pathogenese van cervicale dystonie en volwassen begin idiopathische geïsoleerde focale dystonie wordt ook besproken.
