TDT måling og analyse
To former for apparatet (bord og hodetelefonen), to metoder for stimulans presentasjon (trapp og tilfeldig), og to tilnærminger til dataanalyse (tradisjonell og distribusjon) har blitt presentert illustrere hvordan måle og kvantifisere en person's timelige diskriminering evne. Bærbar headsettet gir et behagelig alternativ som sikrer konsekvens i avstand og vinkler mellom deltakeren og LED-lyskilder samtidig dataene som samles i ethvert praktisk sted. ...

Timelige diskriminering beskriver en persons evne til å diskriminere eller oppfatte, raske endringer i miljøet. Timelige diskriminering terskelen (TDT) er det korteste tidsintervallet der en person kan oppfatte at to sekvensiell sensoriske stimuli er asynkron. Timelige diskriminering har vist seg å være unormalt langvarig i lidelser påvirker det basal Ganglion, inkludert Dystoni1,2,3,4,5,6 , 7.
Dystoni er den tredje vanligste nevrologiske bevegelighet lidelsen - etter Parkinson's sykdom og viktig Tremor. Det er preget av vedvarende eller uregelmessig muskelsammentrekninger forårsaker unormale, ofte repetitiv, bevegelser eller postures8. Dystoni kan påvirke alle deler av kroppen. Når den påvirker en kroppsdel kalles fokal Dystoni8. Dystoni påvirker nakkemusklene som kalles livmorhalsen Dystoni, og er den vanligste fenotypen av voksen utbruddet idiopatisk isolert fokal Dystoni. 9 , 10 patogenesen av cervical Dystoni er ukjent; anses å være en genetisk lidelse med autosomal dominant arv og markert redusert penetrance. Miljøfaktorer også anses viktig i forhold til sykdom penetrance og uttrykk.
Overlegen colliculus, en sensorimotor struktur i dorsal mellomhjernen, er viktig for rask påvisning av miljømessige stimuli under skjult attentional orientere2,11,12. Visuelle stimuli tilgang overlegen colliculus raskt gjennom retino-tectal magnocellular veien. I TDT er et enkelt, objektive mål antatt å representere behandling av visuelle (og andre sensoriske stimuli) i overfladisk lagene på overlegen colliculus. I TDT har blitt studert hos personer med cervical Dystoni, upåvirket slektninger og sunn kontroll deltakere. I forhold til alder og kjønn-matchet kontroll deltakere, en unormal TDT har høy følsomhet (97%, 36 av 37 pasienter) og spesifisitet (98-100%) i cervical Dystoni1. En unormal TDT har blitt funnet i 50% av upåvirket første-graders slektninger av pasienter med cervical Dystoni (14 av 25, i alderen 48 år eller eldre), demonstrere alder - og sex-relaterte penetrance med autosomal dominant arv13, 14. en unormal TDT i upåvirket slektninger av cervical Dystoni pasienter (i forhold til slektninger med normal TDTs) er assosiert med økt putaminal volum (ved voxel-baserte morphometry)15 og redusert putaminal aktivitet (med fMRI)4 . Overlegen colliculus regnes som en betydelig node i nevrale nettverk, som er dysfunksjonelle i cervical Dystoni12. Vurdering av timelige diskriminering regnes som gir viktige ledetråder til pathomechanisms underliggende cervical Dystoni.
Målet med denne artikkelen er å presentere to metoder for å måle og analysere timelige diskriminering, samt demonstrere bruk av denne metoden for å studere i Patofysiologien ved cervical Dystoni.

Timelige diskriminering terskelen (TDT) er det korteste tidsintervallet som observatør kan diskriminere to sekvensiell stimuli som asynkrone (vanligvis 30-50 ms). Det har vist seg å være unormalt (lengre) i nevrologiske lidelser, inkludert cervical Dystoni, en fenotypen av voksen utbruddet idiopatisk isolert fokal Dystoni.
