AR CN analyse i serum og plasma representerer en ny, ikke-invasiv tilnærming for lagdeling av kastrering motstandsdyktig prostatakreft (CRPC) pasienter. Sirkulerende cellen gratis DNA (cfDNA) i blodet har vist seg for å være en optimal biomarkers for kreftdiagnose, prognosen, overvåking og prediksjon av behandling motstand1,2. Mange studier har vist en god samsvar mellom DNA endringer (mutasjoner, kopi nummer variasjoner, epigenetic endringer i vev), og i tilsvarende plasma prøver1, bekrefter at sirkulerende svulst DNA (ctDNA) er informativ for primære og metastatisk tumor vev endringer3. Muligheten for å studere ctDNA dermed lar for gjenoppbyggingen av genomisk rearrangements og kopi nummer varianter (CNVs) på bestemte oncogenes4, identifisere potensielt metastatisk klonal og subclonal celler. CtDNA har vist seg å være klinisk nyttig spesielt for kreft behandling overvåking som havner det spesifikke mutasjoner og CNVs, knyttet til bestemte målrettet terapi5,6. Det også overvinner behov for vev biopsier og lar resultater skal oppnås på ulike tidspunkter i løpet av en bestemt kreftbehandling i en ikke-invasiv måte.
Når det gjelder prostatakreft, en signifikant sammenheng mellom sirkulerende celle-fri androgen reseptoren (AR) CNVs og behandling respons på abiraterone og enzalutamide har vist, som angir AR genet antall kopier (CN) i cfDNA kan være en lovende biomarkør kan forutsi behandling motstand7,8,9,10,11. CNVs av bestemte gener i ctDNA kan evalueres med ulike tilnærminger med forskjellig følsomhet, kostnad og hurtighet (f.eks. sanntid, digitale PCR, og neste generasjons sekvensering).
Her beskriver vi en enkel og rask tilnærming, basert på dobbeltsidig analyser i sanntid PCR-teknologi, for å vurdere AR CN i cfDNA serum og plasma prøver7,8. Vi betraktet to forskjellige PCR analyser utformet på to genomisk regioner intron 5 av AR (Xq12) og to andre gener, som interne standardreferanse gener kjent for å ha statusen normale kopi i prostatakreft (RNaseP, ligger på 14q11; Siden1, ligger på 1 p 34). Vi valgte to referanse gener, snarere enn en, å øke presisjon og følsomhet av resultatene. En DNA mengde 60 ng ble forsterket for hver analysen kombinasjon (kombinert analysen for AR-assay_1 + RNaseP og AR-assay_2 + AGO1). Tre serum- eller DNA-prøver fra friske menn var samlet og brukt som en kalibrator. Vi vurderte konsentrasjon av &#62; 1.5 for AR gevinst og &#60; 0,5 for sletting. Serum og plasma celle gratis DNA (cfDNA) har vist som en informativ, ikke-invasiv biomarkers for kreftdiagnose, prognosen, overvåking og prediksjon av behandling motstand. Starter fra hypotesen at androgen reseptoren (AR) genet antall kopier (CN) få en hyppig hendelse i metastatisk kastrering motstand prostatakreft (mCRPC), foreslår vi å analysere denne begivenheten i cfDNA som en potensiell prediktiv biomarkør.
Vi vurdert AR CN i cfDNA bruker 2 forskjellige sanntid PCR analyser og 2 referanse gener (RNaseP og siden1). DNA mengden 60 ng ble brukt for hver kombinasjon av analysen. AR CN gevinst ble bekreftet ved hjelp av Digital PCR som mer nøyaktig. CN variant analyse allerede er vist for å være informativ for prediksjon av behandling motstand i innstillingen for mCRPC, men det kan være nyttig også til andre formål i ulike innstillinger for pasienten. CN analyse på cfDNA har flere fordeler: det er ikke-invasiv, rask og enkel å utføre, og den starter fra en liten mengde serum- eller materiale.

Totalt cfDNA konsentrasjon var kvantifiserbare av spectrophotometry for alle prøver analysert, viser en median på 6.12 ng/µL (område: 2.00-23.71 ng/µL) for serumprøver og en median på 3,21 ng/µL (område: 2,31-8.49 ng/µL) for plasmaprøver. Vi analyserte totalt 115 prøver av sanntids PCR eksperimenter.
