Perte d’audition neuro-sensorielle (SNHL) découle principalement de dommages ou d’une déficience dans la cochlée. Causes courantes de la SNHL comprennent vieillissement (presbyacousie,par exemple ), des anomalies génétiques, l’exposition à un bruit fort, infection et médicaments ototoxiques1. L’Organisation mondiale de la santé (OMS) estime que plus 360 millions de personnes, ce qui représente 5,3 % de la population mondiale, souffrent de plaidoirie perte2. On estime aussi que 1 à 900 à 1 à 2 500 nouveaux-nés ont modéré, sévère et surdité congénitale permanente profonde et environ un sur trois adultes de plus de 65 ans ont un certain degré de perte d’audience3. Cependant, il n’y a aucun traitement efficace clinique de perte auditive chez ces patients.
Recherche d’audience a longtemps été effectuée en utilisant des rongeurs ou des modèles de cobaye, et plusieurs approches, telles que la thérapie génique et de thérapie régénérative, ont été suggérés comme nouveau traitement pour la perte auditive. Cependant, il y a une grande différence entre les humains et les rongeurs en ce qui concerne le système auditif, et il est très difficile à traduire des modèles animaux à des applications humaines. L’Ouistiti commun (c. jacchus), un singe du nouveau monde originaire de l’Amazonie, est devenu un modèle attrayant de primates non humains pour diverses études de recherche fondamentale pour plusieurs raisons. Tout d’abord, l’anatomie et la physiologie sont plus semblables à ceux de l’être humain plutôt que de rongeurs. En second lieu, l’information de base biologique sur cette espèce, y compris les maladies, les réseaux neuronaux, le comportement et le génome, est bien caractérisée. Traitement sonore et vocal, le codage cortical de la périodicité, la représentation de la hauteur et les interactions de l’auditif-vocal des ouistitis ont également été signalés4,5,6,7 , 8 , 9 , 10. dans de récentes études histologiques, chercheurs identifiés des profils d’expression distinctifs de 20 gènes de la surdité et l’échangeur d’anions dans la cochlée de l’Ouistiti commun et a constaté que cinq gènes qui sont responsables de surdité progressive et trois échangeur d’anions, avaient des modèles d’expressions différentes de celles des rongeurs11,12. Ces profils nous amènent à penser que l’Ouistiti commun est un outil puissant pour la recherche de l’audience.
Les attributs plus marquées de l’Ouistiti commun comme un animal expérimental sont comme suit :
<ul><li>Une manipulation facile avec une petite taille par rapport à certaines espèces de vieux singes du monde : ouistitis adultes pèse de 300 à 400 g et sont environ 60 cm de hauteur, qui ressemble à des rats.</li>
	<li>Primate hautement reproductif : ouistitis atteignent la maturité sexuelle à l’âge de 18 mois et sont capables de supporter deux fois l’an et produire une progéniture de 4 à 6 par an.</li>
	<li>Modifications génétiques sont disponibles : Sasaki E et coll. a réussi à créer des transgéniques13 et knock out14 primates à l’aide d’ouistitis de modifications génétiques impliquées dans les troubles neurologiquespar exemple, maladie de Parkinson de ( la maladie et la maladie d’Alzheimer).</li>
	<li>Les cellules souches embryonnaires (ES) et cellules souches (iPS) pluripotentes induites ont été établis15,16. S’il est plutôt difficile de garder le même nombre d’ouistitis comparée aux rongeurs, la disponibilité de cellules souches ou cellules iPS fournit des tests in vitro qui permettra de réduire le nombre d’in vivo les essais requis.</li>
</ul>Afin de faciliter le mieux la recherche translationnelle dans le domaine de la surdité et sa thérapie potentielle, nous avons établi un protocole d’imagerie de l’étude à l’aide de la CT et IRM, l’anesthésie générale et des tests d’audition à l’aide de réponses auditives du tronc cérébral (PEATC). Ces systèmes expérimentaux peuvent nous fournir de meilleures chances d’obtenir la preuve préclinique d’études de concepts qui sont essentiels pour réduire les écarts entre les essais cliniques et études sur les rongeurs. Nous rapportons ici une méthode chirurgicale d’administrer des médicaments à l’oreille interne de l’Ouistiti commun à travers la membrane de la fenêtre ronde. Pour obtenir une vue claire autour de la fenêtre ronde, sans manipulation de la membrane tympanique qui pourrait causer une perte auditive de conduction, il est utile à l’approche de la cavité mastoïde. Cliniquement, cette approche, appelée « tympanotomie postérieure » est bien établie et généralement utilisé pour la chirurgie Cholestéatome implantation cochléaire. Nous croyons que tympanotomie postérieure nous permet d’effectuer l’administration précise de médicaments sans provoquer de perte d’audition.

Plaidoirie de recherche a longtemps été facilitée par les modèles de rongeurs, bien que dans certaines maladies, les symptômes humaines ne peuvent être récapitulées. L’Ouistiti commun (Callithrix jacchus) est un singe de nouveau monde petit, facile à manipuler, qui a une anatomie similaire de l’OS temporal, y compris les chaînes osselets de l’oreille moyenne et l’oreille interne pour les humains, que par rapport à celle des rongeurs. Nous rapportons ici une approche chirurgicale reproductible, sûre et rationnelle à la niche de la fenêtre ronde cochléaire pour l’administration de médicaments à l’oreille interne de l’Ouistiti commun. Nous avons adopté tympanotomie postérieure, une procédure utilisée cliniquement en chirurgie humaine, pour éviter la manipulation de la membrane tympanique qui peuvent causer une perte auditive de conduction. Cette intervention chirurgicale n’a pas entraîné toute perte auditive importante. Cette approche a été possible en raison de la structure de bulla grande du Ouistiti commun, bien que le canal semicirculaire latéral et la partie verticale du nerf facial doivent être examinées attentivement. Cette méthode chirurgicale nous permet d’effectuer l’administration sûre et précise des médicaments sans perte d’audition, qui est très important dans l’obtention de preuve préclinique du concept de recherche translationnelle.

Tympanotomie postérieure s’est déroulée sans aucune complication tels que l’infection de site opératoire, saignement avec lésion vasculaire ou un dysfonctionnement vestibulaire, qui se produit parfois avec la manipulation de l’oreille moyenne. A Surgical Procedure for the Administration of Drugs to the Inner Ear in a Non-Human Primate Common Marmoset Callithrix jacchus

The overall goal of this surgical procedure is to evaluate the effects of specific drugs of interest on the hearing function of non-human primate common marmoset through their safe administration into the inner ear without inducing hearing loss. This method can help answer key questions about inner ear drug delivery, including how to open the master cavity and where to deliver the drug. The main advantage of this technique is that the inner ear can be a plus without complications, which is critical for evaluating the efficiency of inner ear treatments in primates.The implications of this technique extend towards the study of potential therapies for human deafness, as the auditory system of the marmoset is quite similar to that of humans. The masteration of this method is critical and the posture and anatomy is difficult to learn and require that small window to be created in the narrow space without damaging the facial knob or the ossicular chain. Confirm the appropriate level of sedation by toe pinch and place the animal in the supine position on a heating mat with the head and neck extended.Fix maxilla to the table with nylon thread and pull out the tongue. Insert an endotracheal tube along the laryngeal surface of the epiglottis. Check the entitle carbon dioxide level in the endotracheal tube to confirm that the tube has been properly placed within the trachea and secure the tube to the animal's face.Incline the head to the 30 to 45 degree angle toward the non-operative side and shave the postauricular region of the operative side of the animal Then disinfect the postauricular region and the external auditory canal with povidone iodine and cover the animal with a surgical drape sheet with a round opening over the surgical area. Before opening the skin inject 1%of lidocaine hydrochloride subcutaneously to the incision site. Then, using a surgical microscope and a number 15 round shaped blade, make an incision into the postauricular skin and apply pressure to the bleeding area with a gauze soaked in one to 5, 000 diluted epinephrine.Open the soft tissue of the postauricular region so that the auricular cartilage and the postauricular muscle can be identified and expose the external auditory canal bone deep within the cartilage. To confirm the depth of the tympanic membrane, expose the bony part of the tympanic rim and peel the postauricular muscles toward the caudal aspect to separate the muscles from the temporal bone. Expose the lateral surface of the temporal bone and use a one millimeter diamond burr to make a hole approximately five millimeters behind the external auditory canal.Drill further to enlarge the hole as appropriate and locate the horizontal semi-circular canal, which bulges toward the mastoid cavity. Then drill and remove any bone septa as necessary to obtain a clear view of the facial recess, located at the center of the triangular area between the facial nerve and the corded tympany. Using a 0.6 millimeter diameter diamond burr at 1, 000 rpm, drill a small hole between the vertical section of the facial nerve, which appears as a pink line through the thinned bone and the tympanic rim.Stop the drilling just before the nerve is exposed as a white line structure and confirm that the round window niche can be visualized through the hold. Using a 25 gauge or thinner needle, deliver the drugs of interest into the niche and cover the niche with an appropriate miscues solution to ensure the drug is retained. Confirm that hemostasis is securely controlled and use six o absorbent threads to close the epidermal and dermal layers with simple, interrupted sutures.Then place a piece of gauze onto the surgical wound, fix the gauze in place with a bandage, and allow the marmoset to recover in an intensive care unit at 37 degrees Celsius in 30%oxygen until complete arousal. In these graphs, the auditory brain stem responses of a seven year old common marmoset before and two months after the delivery of one microliter of PBS to the round window niche, as just demonstrated, are shown. Comparison of these wave forms and thresholds to a previous study demonstrated no changes in these parameters before or after the surgery, confirming that posterior tympanotomy is a safe and useful procedure for drug administration to the round window niche of the common marmoset.Once mastered, this technique can be completed in an hour, including the anesthesia and induction, if it's performed properly. While attempting this procedure, it's important to remember not to damage the ossicle or the facial knob which can result in conductive hearing loss or facial paralysis respectively. After its development, this technique paved the way for researchers in the field of the translation research to test potential new therapies for hearing loss.After watching this video, you should have a good understanding of how to approach the inner ear of the common marmoset safely and to administer the drug into round window niche properly.
