Wijzigingen en probleemoplossing:
De kwaliteit van het weefsel is een belangrijke parameter; het kan gemakkelijk worden gecontroleerd door de haematoxyline en eosine-kleuring.

In de afgelopen jaren, heeft immunotherapie ontpopt als een zeer veelbelovende behandeling voor vele soorten van kanker, met inbegrip van de RCC. Bijzonder, immunotherapie, gebaseerd op de remming van remmende checkpoints als PD-1 en CTLA-4 is gemeld te worden klinisch effectief1,2,3,4,5, 6. monoklonale antilichamen tegen CTLA-4, PD-1, of programma dood Ligand 1 (PD-L1) zijn al goedgekeurd in verschillende kankers en leiden tot langdurige klinische reacties in meer dan 20% van de patiënten7. Echter niet alle patiënten responders, de kosten van de behandeling is hoog, en deze behandelingen zijn giftig, wat leidt tot potentiële ernstige bijwerkingen van de auto-immune-achtige. Daarom is de huidige uitdaging het identificeren van voorspellende markers aan die nieuwe immuuntherapie. Het tempo van de mutaties in de tumor, de uitdrukking van PD-L1 of de niveaus van intratumoral CD8+ T cel infiltratie gemeld te correleren met de klinische respons. Deze vereniging is echter nog steeds te zwak om te raden het gebruik van deze klinische biomarkers in de klinische praktijk met uitzondering van de test van de metgezel voor PD-L1 vóór de administratie voor Pembrolizumab in niet-kleincellige longkanker kanker (NKCLK) patiënten8 ,9,1011,12. Het is aangetoond dat de co uitdrukking van vele remmende receptoren als PD-1, Tim-3, Lag-3, en CTLA-4, induceert een cel uitputting fenotype en weerstand tegen therapie13,14,15. Aangezien perifeer bloed niet representatief zijn voor de communicatie van de tumor is, is het van groot belang voor het analyseren van de fenotypische kenmerken van de cellen in situ. PD-1 en Tim-3 mede waarin T-cellen zitten bekend voor zitten functioneel verminderde cellen in verschillende contexten13,16,17. In deze studie, de prognostische impact van de co uitdrukking van de twee remmende receptoren PD-1 en Tim-3 op CD8+ T cellen werd beoordeeld.
Omhoog tot nu toe, het bestuderen van de co uitdrukking van meerdere markeringen op tumor infiltreren lymfocyten (TILs) is voornamelijk door stroom cytometry analyse, waardoor het noodzakelijk wordt om te werken aan nieuwe tumoren en daarom uitschakeling retrospectieve analyses verricht. Met conventionele in situ kleuring, slechts één kleuring tegelijk kan worden uitgevoerd, en de karakterisatie van het celtype die mede de markers uitdrukt is niet mogelijk. Bijvoorbeeld, wordt PD-L1 uitgedrukt door veel celtypen van het tumoraal communicatie, waardoor het moeilijk te definiëren door conventionele immunohistochemistry analyse welke cellen uiten van PD-L1 relevanter voor oereenstemming studies zijn. In dit werk, ontwikkelden we een methode van multiparametric immunofluorescentie innovatieve in situ met computer-tellen om te correleren de co uitdrukking van PD-1 en Tim-3 door tumor infiltreren CD8+ T-cellen met klinische resultaten in de RCC. Deze techniek heeft meerdere voordelen, met inbegrip van de mogelijkheid om te analyseren op het niveau van een enkele cel en meerdere markeringen op hetzelfde moment met behulp van een multispectrale camera die beperkt intervallen vangen kan > 10 nm via vloeibare kristallen filters18. De procedure is bovendien automatische waardoor een inter exploitant reproduceerbaarheid en een verkorte analyse in vergelijking met manuele technieken19. Op het gebied van kanker, verschillende studies gerapporteerd overtuigend meerdere technieken van immuun moleculen zoals PD-1, PD-L1 en CD8 in Merkel-cel carcinoom, longkanker, en hoofd en nek kanker20,21,22, 23. de telling van de geautomatiseerde cel is mogelijk met de opleiding door de gebruiker (fenotypering stap). De fluorescentie wordt gemeten in de andere cel compartimenten (kernen cytoplasma en membraan).
Hier, de verschillende subgroepen van tumor-infiltreren CD8+ T cellen waarin PD-1 en/of Tim-3 in een grote cohort van RCC werden geteld en de resultaten werden gecorreleerd met klinische Ernst scores en overleving parameters. Het was ook mogelijk om het analyseren van membraan intensiteit van de fluorescentie bij de cellulaire resolutie met bedoel fluorescentie intensiteit (MFI) gegevens zoals in cytometry. As far as we know, vertegenwoordigt dit de eerste rapportage prognostische studieresultaten met behulp van deze multispectrale imaging gebaseerde graaf techniek.

multiplexed immunofluorescence analysis and quantification of intratumoral pd-1+ tim-3+ cd8+ t cells

Immune cellen zijn belangrijke componenten van de communicatie van de tumor en tumorgroei en evolutie beïnvloeden in alle stadia van carcinogenese. Met name nu ook vaststaat dat de immuun infiltreren in menselijke tumoren met de prognose en respons op therapie correleren kan. De analyse van de immuun infiltreren in de communicatie van de tumor is uitgegroeid tot een grote uitdaging voor de classificatie van patiënten en de reactie op behandeling.
De co uitdrukking van remmende receptoren zoals programma cel dood eiwit 1 (PD1; ook bekend als CD279), cytotoxische T-lymfocyten geassocieerde eiwit 4 (CTLA-4), T-cel Immunoglobulin Mucin bevattende eiwitten-3 (Tim-3; ook bekend als CD366) en lymfocyt Activering gen 3 (Lag-3; ook bekend als CD223), is een kenmerk van T cel uitputting. We ontwikkelden een multiparametric in situ immunofluorescentie kleuring te identificeren en te kwantificeren op cellulair niveau de co uitdrukking van deze remmende receptoren. Op een retrospectieve serie van bevroren weefsel van renaal cel carcinomen (RCC), met behulp van een fluorescentie multispectrale imaging technologie in combinatie met de software van de analyse van een afbeelding, bleek dat mede uiting van PD-1 en Tim-3 op tumor infiltreren CD8+ T cellen is gecorreleerd met een slechte prognose in RCC. Om onze kennis, dit is de eerste studie waaruit blijkt dat dit geautomatiseerd multiplex in situ technologie wellicht sommige klinische relevantie.

Met behulp van het algemeen protocol zoals hierboven beschreven, we gericht op het kwantificeren van intratumoral CD8+ T cellen samen uiting geven aan de remmende receptoren PD-1 en Tim-3 in bevroren weefsels van patiënten met RCC, en de resultaten correleren met klinische resultaten25.
