I de senaste åren, har immunterapi vuxit fram som en mycket lovande behandling för många typer av cancer, inklusive RCC. Särskilt, immunterapi baserat på hämning av hämmande kontrollpunkter som PD-1 och CTLA-4 har rapporterats vara kliniskt effektivt1,2,3,4,5, 6. monoklonala antikroppar mot CTLA-4, PD-1 eller Program död Ligand 1 (PD-L1) redan har godkänts i flera cancerformer och leda till långvariga kliniska svaren i mer än 20% av patienterna7. Dock inte alla patienter responders, kostnaden för behandling är hög, och dessa behandlingar är giftiga, leder till potentiella allvarliga autoimmuna-liknande biverkningar. Utmaningen är därför att identifiera prediktiva markörer till dessa nya immunterapier. Graden av mutationer i tumören, uttrycket av PD-L1 eller nivåerna av intratumoral CD8+ T cell infiltration har rapporterats korrelera med kliniskt svar. Den här associeringen är dock fortfarande för svag för att rekommendera användning av dessa kliniska biomarkörer i klinisk praxis utom testet följeslagare för PD-L1 före administreringen av Pembrolizumab i icke-small cell lung cancer (NSCLC) patienter8 ,9,1011,12. Det har visats att samtidig uttrycket av många hämmande receptorer som PD-1, Tim-3, lagg-3, och CTLA-4, inducerar en cell utmattning fenotyp och resistens mot behandling13,14,15. Eftersom perifert blod inte är representativt för den tumör mikromiljö, är det av stort intresse att analysera de fenotypiska egenskaperna hos de celler i situ. PD-1 och Tim-3 Co uttrycker T-celler är kända för att vara funktionellt nedsatt celler i flera sammanhang13,16,17. I denna studie, den prognostiska effekten av samtidig uttrycket av de två hämmande receptorerna PD-1 och Tim-3 på CD8+ T celler bedömdes.
Upp förrän nu, studera samtidig uttrycket av flera markörer på tumören infiltrerar lymfocyter (TILs) har främst utförts av flödescytometri Flödesanalys, vilket gör det nödvändigt att arbeta på färska tumörer och därför att den utgör hinder retrospektiva analyser. I detta arbete, vi utvecklat en innovativ i situ multiparametric immunofluorescens metod med dator-inventering för att korrelera samtidig uttrycket av PD-1 och Tim-3 av tumören infiltrerar CD8+ T-celler med kliniska resultat i RCC. Denna teknik har flera fördelar, inklusive möjligheten att analysera på en enda cellnivå och flera markörer på samma gång använder en Multispektrala kamera som kan fånga begränsade intervall > 10 nm genom flytande kristall filter18. Förfarandet är dessutom automatisk som möjliggör en Inter operator reproducerbarhet och en förkortad analys jämfört med manuella tekniker19. I fältet cancer rapporterat flera studier övertygande flera färgningar av immun molekyler som PD-1, PD-L1 och CD8 i Merkel-cell carcinoma, lungcancer, och huvud och hals cancer20,21,22, 23. det automatisera celltalet är möjligt med utbildning av användaren (fenotypning steg). Fluorescensen mäts i olika cell fack (kärnor, cytoplasman och membran).
Här, olika undergrupper av tumör-infiltrera CD8+ T celler uttrycker PD-1 och/eller Tim-3 i en stor kohort av RCC räknades och resultaten var korrelerade med kliniska gravitation noter och överlevnad parametrar. Det var också möjligt att analysera membran fluorescensintensitet med den cellulära upplösningen med menar fluorescens intensitet (MFI) data som i flödescytometri. Såvitt vi vet, representerar detta första rapportering prognostiska studieresultaten använder denna Multispektrala bildteknik baserat antal.

multiplexed immunofluorescence analysis and quantification of intratumoral pd-1+ tim-3+ cd8+ t cells

Immunceller är viktiga komponenter i den tumör närmiljön och påverka tumörtillväxt och evolution i alla skeden av carcinogenes. Särskilt, är det nu väl etablerat att det immuna infiltrera i mänskliga tumörer kan korrelerar med prognos och svar på behandling. Analysen av det immuna infiltrera i den tumör närmiljön har blivit en stor utmaning för klassificering av patienter och behandlingssvaret.
Samtidig uttrycket av hämmande receptorer såsom Program Cell Death Protein 1 (PD1; även känd som CD279), cytotoxiska T-lymfocyter associerade Protein 4 (CTLA-4), T-Cell immunglobulin och Mucin som innehåller göra Protein-3 på (Tim-3; även känd som CD366) och lymfocyter Aktivering gen 3 (Lag-3; även känd som CD223), är ett kännetecken för T-cell utmattning. Vi utvecklat ett multiparametric i situ -immunofluorescens färgning för att identifiera och kvantifiera på cellnivå samtidig uttrycket av dessa hämmande receptorer. På en retrospektiv serie frysta vävnad av renal cell carcinom (RCC), använda en fluorescens Multispektrala bildteknik tillsammans med en bild analys programvara, det konstaterades att samtidig uttryck av PD-1 och Tim-3 på tumören infiltrerar CD8+ T celler korreleras med en dålig prognos hos RCC. Till vår kunskap, Detta representerar den första studien som visar att detta automatiserade multiplex i situ teknik kan ha viss klinisk relevans.
