Hyperplasie bénigne de la prostate (HBP), qui est une hyperplasie de la prostate, se manifeste chez les personnes âgées. Cette affection débute à environ 40 ans et la morbidité peuvent atteindre 50 % - 60 % pour les hommes de 60 ans et 80 % - 90 % pour les hommes dans les années 70 et 80,1. HBP provoque l’obstruction progressive, symptômes du bas appareil urinaire, rétention et ses complications et une insuffisance rénale même, qui tous baisser la qualité de vie de l’homme âgé. Après un traitement médical, le traitement chirurgical est souvent utilisé comme le traitement plus efficace de l’HBP symptomatique.
Différents types de chirurgie ont été développés pour traiter les symptômes de l’HBP, y compris la première prostatectomie ouverte introduite en 18942 et chirurgie endoscopique transurétrale (TURP) un siècle plus tard. Depuis son développement, TURP a été considéré comme l’étalon-or pour atténuer chirurgicalement hypertrophie de la prostate.
Le laser holmium fonctionne à une longueur d’onde de 2120 nm, ce qui est similaire à celle des tissus eau3, qui provoque l’évaporation de l’eau. Pénétration tissulaire atteint 0,4 mm et coagulation peut également fonctionner avec elle. Le processus a été introduit pour l’HBP en 19944. En 1998, Peter Gilling a utilisé l’appareil pour effectuer une énucléation demande5. Tel que proposé par Peter Gilling et ses collaborateurs dans les années 19904, énucléation laser holmium de la prostate (HoLEP) est utilisée pour traiter l’HBP en combinant les deux types précédents de chirurgie, y compris l’énucléation ouverte et la RTUP4. Avec le développement du processus d’hyperplasie, la zone périphérique prostatique est ont entouré par une zone de transition (TZ) pour former une capsule dite chirurgicale. HoLEP permet l’élimination de la partie hyperplasique de la prostate, la TZ, qui sépare la capsule de la prostate. Ainsi, la HoLEP, comme la chirurgie anatomique, pouvez enucleate la zone de la prostate élargie de la séparation formée entre TZ et capsule. Comparée à la méthode chirurgicale traditionnelle, HoLEP peut être plus sûr, plus efficace et approfondie avec des résultats durables. Par rapport à la RTUP, holmium réduit la durée, temps d’irrigation et l’heure de cathéter de la procédure et l’hospitalisation réduite. 6 , 7 , 8 cette technique peut également être réalisée en toute sécurité sur des patients qui sont sous traitement anticoagulant ou avec saignement troubles9. Bien qu’il existe différents types de procédures pour HoLEP, la procédure principale est le même que celui mentionné dans le texte suivant. Nous présentons également nos propres expériences et l’innovation pour cette méthode afin de rendre plus facile à accepter et à raccourcir la courbe d’apprentissage. La procédure est particulièrement adaptée pour les grandes prostates et les combinés avec les calculs de la vessie, étant donné que le laser holmium peut également être utilisé pour la lithotritie8. Cet article présente cette technique et la procédure détaillée.

L’hyperplasie bénigne de la prostate (HBP) se produit généralement chez les hommes âgés. Cette affection provoque des symptômes dans les voies urinaires basses, ce qui réduit la qualité de vie. HBP comprend 2 traitements majeurs : médicaments et chirurgie. Le traitement chirurgical est souvent l’intervention plus effective et définitive. Énucléation laser holmium de la prostate (HoLEP), une des procédures chirurgicales plus efficaces contre l’HBP, est menée transurétrale. Pendant la chirurgie, la partie plus importante et le plus difficile est de localiser la capsule chirurgicale de la prostate, qui est conforme à l’idée de chirurgie anatomique. Différentes techniques peuvent être nécessaires dans le traitement des différentes parties de la prostate. HoLEP tend à être efficace et sécuritaire avec de bonnes propriétés hémostatiques, et capable de traiter les calculs de la vessie, qui peuvent être les complications de l’HBP. La technique est particulièrement adaptée pour la prostate de volumes et de tailles différentes. En effet, HoLEP présente aussi certains inconvénients, comme une longue courbe d’apprentissage et un équipement coûteux. Malgré tout, cette méthode peut être la « nouvelle norme » et la méthode excellente pour le traitement chirurgical de l’HBP.

Les données rétrospectives, recueillies entre 2015 et 2016, proviennent de la base de données clinique de notre département. Un total de 100 patients traités contre l’HBP ont participé à l’étude.
