I denne protokollen introdusere vi en prosedyre for kirurgisk behandling av BPH. HoLEP kan benyttes i daglig praksis. Som nevnt, omfatter utvikling av BPH kirurgi tre landemerker. Benign prostatahyperplasi (BPH), som er hyperplasia av prostate kjertel, oppstår i eldre menn. Dette starter på ca 40 y gamle og sykelighet kan nå 50% - 60% for menn deres 60-tallet og 80-90% for menn i deres 70 og 80-tallet1. BPH forårsaker progressiv obstruksjon, lavere urin spor tegn, oppbevaring og komplikasjoner og med nyresvikt, alle lavere livskvalitet eldre menn. Etter medisinsk behandling brukes kirurgisk behandling ofte som den mest effektive behandlingen for symptomatisk BPH.
Ulike typer kirurgi er utviklet for å behandle symptomene av BPH, inkludert den første åpne prostatektomi introdusert i 18942 og transurethral endoskopisk kirurgi (TURP) et århundre senere. Siden sin utvikling, har TURP vært ansett som gullstandarden å kirurgisk lindre prostata utvidelse.
Holmium laser opererer på en bølgelengde på 2120 nm, som ligner på det til vev vann3, som induserer fordamping av vann. Vev penetrasjon når 0.4 mm, og koagulering kan også betjenes med den. Prosessen ble først introdusert for BPH i 19944. I 1998 brukes Peter Gilling enheten til å utføre enucleation program5. Som foreslått av Peter Gilling et al. i 1990-årene4brukes holmium laser enucleation av prostate (HoLEP) til å behandle BPH ved å kombinere de to tidligere typene kirurgi, inkludert den åpne enucleation og den TURP4. Med utviklingen av hyperplasi, er prostatic perifere sonen foregir av sonen overgangsreglene (TZ) for å danne en såkalt kirurgisk kapsel. HoLEP tillater fjerning av den hyperplasic delen av prostata, TZ, som skiller prostata kapselen. surgical treatment for benign prostatic hyperplasia: holmium laser enucleation of the prostate (holep).

Benign prostatahyperplasi (BPH) forekommer ofte blant eldre menn. Tilstanden forårsaker symptomer i lavere urin-område, og dermed redusere kvaliteten på livet. BPH inneholder 2 store behandlinger: medisin og kirurgi. Kirurgisk behandling er ofte mest effektive og endelige intervensjon. Holmium laser enucleation av prostate (HoLEP), en av de mest effektive kirurgiske prosedyrene for BPH, er gjennomført transurethrally. Under kirurgi er den viktigste og vanskeligste delen å finne kirurgisk kapselen av prostata, som er konsekvent til ideen om anatomiske kirurgi. Ulike ferdigheter kan være nødvendig i behandling av ulike deler av prostata. HoLEP tendens til å være effektiv og trygg med gode hemostatic egenskaper, og kunne behandle blæren kalkulus, som kan være komplikasjoner av BPH. Teknikken er spesielt egnet for prostata varierende volumer og størrelser. Faktisk presenterer HoLEP også visse ulemper, som lange læringskurve og kostbart utstyr. Uansett, kan denne metoden være en "ny standard" og utmerket metoden for kirurgisk behandling av BPH.

Retrospektive data, samlet fra 2015 til 2016, ble innhentet fra klinisk databasen vår avdeling. Totalt 100 pasienter som behandles for BPH ble inkludert i studien. Perioperative resultater ble inkludert: operasjonstid (OT), hemoglobin nedgang etter kirurgi, Na+ variasjon, sykehus opphold varighet, blære vanning varighet og følgende dataene.
