Cet article a décrit un protocole complet pour l’enregistrement simultané de MEG et EEG pendant la livraison gaz anesthésique N2O et Xe. Un tel protocole seront utile pour étudier les corrélats neurones électromagnétiques des réductions induite par l’anesthésie en conscience. Le protocole devrait également de généraliser à la livraison des autres gaz anesthésiques tels que sévoflurane ou l’isoflurane. Il y a bon consensus entre les preuves neuroscientifiques pré-cliniques et cliniques suggérant que le phénomène de la conscience humaine dépend de l’intégrité des circuits neuraux explicites. L’observation que ces circuits sont systématiquement influencées par la descente dans l’inconscience a justifié la nécessité pour les techniques de neuroimagerie être utilisés pendant l’anesthésie et permettre « navigation » les corrélats neurones de la recherche conscience. À l’exception peut-être du sommeil, anesthésie représente le seul moyen par lequel on peut, de façon contrôlée, réversible et reproductible, perturber et donc disséquer, les mécanismes qui servent sous la conscience, en particulier à l’échelle macroscopique de dynamique du cerveau global. Cliniquement, l’anesthésie générale peut être défini comme un état d’hypnose/inconscience, immobilité et analgésie et demeure l’une des interventions médicales les plus abondamment utilisées et la plus sûres. Malgré la clarté et l’efficacité dans le résultat final, il reste grande incertitude quant aux mécanismes d’action des différents types d’agents donnant naissance à l’inconscience induite anesthésique1.
Les anesthésiques peuvent être divisés en agents intraveineux notamment propofol et les barbituriques ou des agents volatils/gazeux comme sevoflurane, isoflurane, l’oxyde nitreux (N2O) et le xénon (Xe). La pharmacologie de l’anesthésie a été bien établie avec plusieurs cibles cellulaires identifiés comme lié à l’action anesthésique. La plupart des agents a étudié à date acte principalement par l’intermédiaire de l’agonisme de γ-Amino-Butyric-Acid-(GABA) l’activité médiée par le récepteur. En revanche, la kétamine agents dissociatifs, Xe et N2O sont censés exercer leurs effets en ciblant principalement les N-Methyl-D-Aspartate-(NMDA) glutamatergique récepteurs2,3. Autres cibles pharmacologiques importantes incluent les canaux potassiques, l’acétylcholine et les récepteurs de glutamate de vestige, AMPA et kaïnate, toutefois l’étendue de leur contribution à l’action anesthésique demeure insaisissable (pour un examen complet, voir ( 4).
La mesure de la variabilité dans le mécanisme d’action et les effets physiologiques et neurones observés des différents types d’agents complique la dérivation des conclusions générales sur leur influence sur le traitement conscient. Perte de conscience (LOC) induite par les agents GABAergiques est généralement caractérisée par un changement global dans l’activité cérébrale. Cela est évident dans l’émergence de grande amplitude, basse fréquence delta (δ, 0,5 à 4 Hz) vagues ainsi que la réduction à haute fréquence, activité gamma (γ, 35 à 45Hz) à l’électroencéphalographie (EEG), similaire à onde lente dorment5,6 ainsi que la réduction généralisée des sanguin cérébral flux et glucose métabolisme5,6,7,8,9,10,11,12 . Boveroux et al. 13 ajouté à ces observations, en démontrant une diminution significative au repos la connectivité fonctionnelle État sous anesthésie de propofol à l’aide d’imagerie de résonance magnétique fonctionnelle (IRMf). En revanche, les anesthésiques dissociatifs donnent un moins désactivez le profil des effets sur l’activité cérébrale. Dans certains cas, ils sont associés à des augmentations sanguin cérébral flux et glucose métabolisme14,15,16,17,18,19, 20,21 , tandis que des études réalisées par Rex et ses collègues22 et Laitio et ses collègues de23,24 en regardant les effets de la Xe fourni la preuve des deux augmente et diminue le cerveau activité. Une irrégularité similaire peut être vu dans les effets sur les EEG signaux25,26,27,28. Johnson et al. 29 a démontré une augmentation de la puissance totale du basse fréquence bandes delta et theta ainsi que dans le gamma de bande fréquence plus élevée dans une forte densité étude EEG de l’anesthésie Xe tandis que des observations défavorables ont été faites pour N2O dans le delta, thêta et bandes de fréquences alpha30,31 et Xe dans le plus élevé de fréquences32. Cette variabilité dans les effets du Xe sur l’activité électrique de cuir chevelu peut être observée dans l’alpha et les gammes de fréquences bêta avec les deux augmente également33 et réductions34 rapportées.
Malgré les écarts mentionnés ci-dessus, l’image commence à devenir plus cohérente dans l’ensemble des agents lorsqu’on essaie de regarder les altérations dans la connectivité fonctionnelle entre les zones du cerveau. Ces mesures, cependant, ont été principalement limités aux modalités qui nécessairement faire des concessions en ce qui concerne la résolution spatiale ou temporelle. Bien que les études utilisant l’EEG semblent révéler clairement et dans une certaine mesure cohérente, changements dans la structure topologique des réseaux fonctionnels au cours de l’anesthésie/sédation par propofol35, sévoflurane36 et N2O37, le largement espacées capteur niveau données EEG ont une résolution spatiale insuffisante utilement définir et délimiter les sommets des réseaux fonctionnels correspondants. À l’inverse, des études utilisant la résolution spatiale supérieure d’IRMf et par émission de positrons (TEP), trouver des altérations topologiques semblables en connectivité fonctionnelle à grande échelle à celle de l’EEG13,38,39 ,40,41, possèdent cependant une résolution temporelle insuffisante pour caractériser la phase-amplitude de couplage dans la bande alpha de (8 à 13 Hz) EEG et d’autres phénomènes dynamiques qui émergent comme signatures importantes de action anesthésique12,42. En outre, ces mesures n’évaluent pas directement l’activité neurale électromagnétique43.
Par conséquent, afin de véritablement faire progresser la compréhension des processus macroscopiques associée à l’action des anesthésiques, les limitations des enquêtes mentionnées précédemment doivent être abordés ; la couverture restreinte des agents anesthésiques et la résolution spatio-temporelle insuffisante des mesures non invasives. Sur cette base, les auteurs décrivent une méthode pour enregistrer simultanément magnetoencephalogram (MEG) et l’activité EEG chez des volontaires sains qui a été développée pour l’administration des agents anesthésiques dissociatifs gazeux, Xe et N2O.
Le MEG est utilisé comme c’est la technique non invasive seulement neurophysiologique autres que l’EEG qui a une résolution temporelle de l’ordre de la milliseconde. EEG a le problème de flou de champs électriques par le crâne, qui agit comme un filtre passe-bas sur activité corticalement généré, tandis que MEG est beaucoup moins sensible à cette question et la question du volume de conduction44. On peut affirmer que MEG a plus spatiale et source de précision de la localisation que EEG 45,46. EEG ne permet pas de vrai enregistrement sans référence37,47, mais fait de MEG. Systèmes MEG a aussi généralement enregistrent l’activité corticale dans une gamme de fréquences beaucoup plus large que l’EEG, y compris les gamma élevés48(généralement 70-90 Hz), qui ont été proposés d’être impliqués dans les effets hypnotiques des agents anesthésiques notamment Xe29 et N 2 O28. Le MEG propose activité neurophysiologique qui complimente qui véhiculé par EEG, comme l’activité EEG est liée à des courants électriques extracellulaires tandis que MEG reflète principalement les champs magnétiques générés par des courants intracellulaire46, 49. en outre, MEG est particulièrement sensible aux activité électrophysiologique tangentielle au cortex, tandis que l’EEG enregistre principalement l’activité extracellulaire radiale pour le cortex49. Fusionnant ainsi données MEG et EEG a avantages Super additif50.
Les agents dissociatifs gazeux Xe et N2O ont été choisis pour les raisons de principe suivante : ils sont sans odeur (Xe) ou essentiellement inodore (N2O) et donc peut facilement être utilisés en présence de conditions de contrôle lorsqu’ils travaillent à concentrations infracliniques. En outre, ils sont bien adaptés pour l’administration à distance et le contrôle dans un environnement de laboratoire en raison de leurs faibles effets dépresseurs cardio-respiratoires61. Xénon et à un moindre degré N2O, retenir au moins relativement faible-alvéolaires - concentration-(MAC)-éveillé à laquelle 50 % des patients répondait à une commande verbale avec des valeurs de 32,6 ± 6,1 %51 et 63,3 + - 7,1 %52 respectivement. Malgré les Xe et N2O fois étant antagonistes des récepteurs NMDA, ils modulent l’EEG différemment - Xe semble se comporter plus comme un agent GABAergique typique lorsque surveillées à l’aide de le Bispectral Index33,53,54 (l’une des différentes approches utilisées pour surveiller electroencephalographically profondeur de l’anesthésie). En revanche, N2O produit un effet électro-encéphalographiques beaucoup moins évident en ce que c’est mal, si, surveillées à l’aide de l' Bispectral Index26. Parce que Xe a des propriétés différentes de technologues en électroencéphalographie signalées aux autres agents dissociatifs, mais possède des caractéristiques similaires aux agents GABAergiques plus communément étudiés, son étude électrophysiologique a le potentiel pour révéler important caractéristiques relatives aux corrélats neurones de la conscience et les changements correspondants de réseau fonctionnel. Agents qui agissent sur les récepteurs de NMDA sont susceptibles de révéler plus sur les réseaux de cerveau qui favorisent la conscience normale et altérée, étant donné le rôle essentiel que médiée par le récepteur NMDA activité joue dans l’apprentissage et de mémoire et de son rôle impliquée dans une gamme de troubles psychiatriques qui incluent la schizophrénie et la dépression80.
Cet article se concentre principalement sur la procédure de collecte données exigeant et complexe, associée à la livraison des agents anesthésiques gazeux en milieu non hospitalier tout en enregistrant simultanément MEG et EEG. Analyse des données de base au niveau du capteur est exposé et données exemple sont fournies qui illustrent que les enregistrements haute fidélité peuvent être obtenus avec mouvement minime de la tête. Les nombreuses méthodes potentielles de source ultérieure d’imagerie et/ou fonctionnelle connectivité analyse qui serait généralement effectuée à l’aide de ce type de données ne sont pas décrits, car ces méthodes sont bien décrites dans la littérature et de démontrent les différentes options pour analyse55,56.

L’anesthésie fournit sans doute une des façons seulement systématiques pour étudier les corrélats neurones de conscience/inconscience globale. Mais à ce jour la plupart neuro-imagerie ou études neurophysiologiques chez l’humain ont été limités à l’étude des anesthésiques γ-Amino-Butyric-Acid-(GABA)-receptor-agonist-based, alors que les effets de la dissociation N-Methyl-D-Aspartate-(NMDA)- anesthésiques récepteur dotés d’antagoniste de la kétamine, l’oxyde nitreux (N2O) et le xénon (Xe) sont en grande partie inconnus. Cet article décrit les méthodes qui sous-tendent l’enregistrement simultané de la magnétoencéphalographie (MEG) et de l’électroencéphalographie (EEG) d’hommes en bonne santé pendant l’inhalation des agents anesthésiques gazeux N2O et Xe. En combinant les données MEG et EEG permet l’évaluation de l’activité cérébrale électromagnétique lors d’une anesthésie à haute résolution spatiale, temporelle et modérée. Nous décrivons ici un protocole détaillé, affiné au cours de plusieurs sessions d’enregistrement, qui inclut le recrutement des sujets, l’installation de matériel d’anesthésie dans la salle de scanner de MEG, la collecte et l’analyse de la base de données. Dans le présent protocole, chaque participant est exposé à différents niveaux de Xe et N2O dans un design reglable de mesures répétées. Suite de base pertinente enregistrements participants sont exposés à l’augmentation de lavage inspiré des concentrations de Xe et N2O de 8, 16, 24 et 42 % et 16, 32 et 47 % respectivement, au cours de laquelle leur degré de réactivité est suivi avec un auditif tâche continue de la performance (aCPT). Résultats sont présentés pour un certain nombre d’enregistrements pour mettre en évidence les propriétés au niveau du capteur de données brutes, la topographie spectrale, la minimisation des mouvements de la tête et les effets dépendants niveau sans équivoque sur les réponses évoqués auditifs. Ce paradigme décrit une approche générale de l’enregistrement des signaux électromagnétiques associés à l’action des différents types de gazeuses anesthésiques, qui peuvent être facilement adaptées pour être utilisé avec des agents anesthésiques volatils et par voie intraveineuse. Il est prévu que la méthode décrite peut contribuer à la compréhension des mécanismes macro-échelle de l’anesthésie par l’activation des extensions méthodologiques imagerie spatiale de source et d’analyse de réseau fonctionnel.

Cette section utilise les données provenant d’un sujet afin de démontrer les caractéristiques typiques des enregistrements simultanés et le potentiel d’une telle information afin de contribuer à une meilleure compréhension de l’anesthésique induit par États modifiés de conscience. The purpose of this experiment is to characterize changes in brain electromagnetic activity in response to the administration of the two gaseous dissociative agents xenon and nitrous oxide to healthy participants in a two-way repeated measures crossover design. Due to the complexity of the experiment, it is important that the majority of equipment setup occurs before participant arrival. The most important of these is the configuration of the EEG system in the magnetically shielded room or MSR.The EEG amplifier is placed in the MSR as far as practical away from the participant, and needs to be configured to run on DC power using either a built-in battery or an external shielded battery box. Fiber optic coupling from the amplifier to the data-logging computer should be checked to be operational. Finally, a video camera in the MSR should be configured such that its field of view clearly captures the participant's face and upper thorax such that the physician anesthesiologist can clearly monitor the respiratory and airway status of the participant.In order that consistent and comparable data is acquired, participants are first interviewed to confirm their eligibility for the study. Participants should be healthy males of 20 to 40 years of age with no psychiatric or neurological conditions, no recent history of drug or alcohol use, and have no respiratory or cardiac conditions. Participants should arrive clean-shaven on the day of the experiment, and having fasted for at least six hours.To ensure high quality EEG recordings, it is recommended that the subject's hair has been recently washed without conditioner, and that their scalp has been firmly brushed to remove any dried or dead skin. An MEG-compatible high-density EEG cap is applied with the appropriate use of conductive gel. Head position indicator coils are then firmly attached at predefined locations on the EEG cap.All auxiliary biopotential electrodes are then applied at the designated locations on the subject's face and body. For subsequent analysis, it is important that all electrodes and HPI coil positions are accurately registered along with digitization of the outlines of the orbits, nasal ridge and cheeks for subsequent coregistration with structural MRI. Two electrodes are placed submentally to record resting muscle tone.Electrooculogram or EOG recordings are made by placing two further electrodes above and below the eye. And finally, a three-lead ECG derivation is configured by electrodes on the left and right wrist flexor surfaces. EEG electrode impedances should then be verified as being below five kiloohms in order to yield high-fidelity recordings.The duration of the experiment as well as the sedative effects of the administered gases makes minimization of head movement vital. A memory foam head brace is applied in order to reduce lateral head movement, and a jute harness is tied around the body to minimize any vertical movement. Eyes-closed resting state and baseline auditory continuous performance task recordings are now performed.It is of vital importance that the administration of anesthetic gases is supervised by a physician anesthesiologist at all times. As well as patient monitoring, the role of the physician anesthesiologist includes ensuring the adequate configuration of the anesthetic gas delivery unit, specialized in this case to deliver and monitor xenon. The physician anesthesiologist will test the anesthesia machine, check that the mask fits comfortably and does not leak, adjust gas flows, insert a cannula, administer antiemetics, as well as commencing denitrogenation with 100%oxygen with the participant seated in the MSR before the final post-antiemetic baseline data collection and the start of the gas administration protocol.At this stage the participant will be comfortably seated in the MSR having been denitrogenated and having been administered dexamethasone and ondansetron to minimize the risk of any adverse nausea or vomiting due to gas administration. Before the MSR door is closed, the anesthetic nurse or other suitably trained medical professional is seated next to the participant. A final check is made that the blood pressure cuff is connected, the button response boxes are comfortably positioned, and the pulse oximeter on the toe or finger is securely fastened.The protocol then proceeds to deliver four Equi-MAC awake progressively increasing stepwise levels of xenon gas, or three stepwise nitrous oxide levels over a period of 15 minutes each in order to assure blood-brain-gas equilibration. Each level should contain a five-minute steady state when the desired gas concentration is reached and should be followed by a 10-minute washout period. During all levels, oxygen, carbon dioxide, and xenon or nitrous oxide levels are continuously monitored and electronically logged.At the end of each level, participants are briefly assessed before progressing to higher levels. Because at the highest xenon level, in which loss of responsiveness is achieved, airway obstruction can readily and unexpectedly occur, and subjects need to be closely monitored and continuously assessed by qualified personnel, both inside and outside the MSR for all necessary action to be taken immediately. At the conclusion of the experiment, the physician anesthesiologist will enter the MSR and debrief with the patient.The participant is then brought outside the MSR where the apparatus is removed and they are administered standard postoperative anesthetic care. Before the participant leaves, ensure they are safe to travel home with a relative or friend. In the next 24 hours, the participant will be required to fill in the questionnaires regarding the procedure and any after effects.This video has been primarily prepared to illustrate the methods involved in delivering gaseous anesthesia while recording simultaneous EEG and MEG data, therefore, only very basic data analysis and representative results are presented to give the viewer a clear idea of what the recorded data should look like. The basic results illustrate examples of head movement, gas concentration timecourse and auditory task accuracy;filtered artifact-free EEG and MEG data;EEG and MEG spectra and power topography;and auditory evoked responses for both baseline and during gas administration. To create the basic results, the following pipeline is employed using a combination of data analysis software platforms and toolboxes.The raw MEG and EEG data is stored as fif and cnt file formats respectively. Head movements are computed using the MaxFilter software and a script written by the authors, both used to convert head movements from the cortonian space into Cartesian coordinates. Auditory task accuracy and reaction time are then computed and visualized, time-aligned with the head movement and gas concentration time series, in order to assess data quality before further processing the EEG and MEG data.Time alignment of the data is based on recorded trigger times. Typically, head movement will be minimal during baseline conditions, while more movement tends to occur at the higher gas levels associated with more dissociated or sedated states. This can be seen here for example in the 42%xenon recording where the top figure shows head movement, and the bottom, gas concentration and auditory task accuracy.Note that loss of responsiveness occurs between 7.5 and 12.5 minutes. Absolute head movement of less than five millimeters is considered acceptable for source imaging purposes. Data segments with movement larger than five millimeters are ignored.Preprocessing of the MEG and EEG data initially involves selection of bad channels. This is followed by the use of the tSSS algorithm to remove artifacts from the MEG recording due to external sources. The rest of the processing pipeline involves the use of data analysis software platforms and associated toolboxes for analysis.Band-pass and notch filtering is applied to both the MEG and EEG data to focus analysis on the frequency band of interest. Time periods in the data containing artifacts are rejected based on visual inspection, in addition to removing periods involving excessive head movement changes. Filtered artifact-free MEG and EEG data can then be visualized and related to anesthetic-induced changes.Amplitude spectra for each sensor and spectral band power topographies across all sensors are then computed for both MEG and EEG data. Visualization of the amplitude spectra at different sensor locations facilitates observation of the effects of different anesthetic levels on signal spectra. Visualization of sensor level power topography in different frequency bands, in this case the alpha band, enables the observation of the effects of different anesthetic levels on the spacial pattern of activity.The filtered artifact-free MEG and EEG data are then epoched using triggers linked to the time of auditory tone delivery. Average auditory evoked responses are then constructed through time on sample averaging. Visualization of sensor level auditory evoked responses allows for the characterization of the effects of anesthetics on the brain's responsiveness.In particular, awakening of the initial evoked response and the disappearance of the later evoked response peaks results from high gas concentrations. There are many other potential interesting analysis steps that can be performed subsequent to this basic analysis. Given that there are many options presented in the literature, we do not cover them in detail here.Such options include MEG and EEG source imaging using beamforming or minimum-norm estimation to gain a better understanding of the relationship between brain activity and brain anatomy. Sensor or source-level functional connectivity analysis can unravel changes in brain networks linked to anesthetic action. There is also the option to either focus analysis on the sensor or source-level auditory evoked responses, or to look at continuous data independent of the auditory stimulus in order to identify any anesthetic-induced changes in global brain dynamics.The macroscale mechanisms of anesthetic-induced reductions in consciousness are not well understood. Evidence from literature has strongly implicated a breakdown in functional connectivity underlying anesthetic-induced reductions in consciousness. Although anesthetics have a diverse range of molecular targets of action and different macroscopic effects on the EEG and MEG, it is still not resolved if there exists a universal or common macroscale mechanism across all anesthetics.Here we have outlined a protocol for performing gaseous anesthesia using the putative NMDA receptor antagonists xenon and nitrous oxide while simultaneously recording MEG and EEG data. This method will facilitate the study of different gaseous and volatile agents on simultaneously recorded MEG and EEG data, and thus the search for universal mechanisms of anesthetic-induced reductions in consciousness.
