Nous avons été en mesure d’établir un protocole pour générer des sphéroïdes reproductibles de suspensions cellulaires, pour les deux lignées cellulaires et, dans des expériences préliminaires, les cellules tumorales humaines primaires. Tout d’abord, nous avons évalué deux méthodes précédemment décrites et identifiées la ULA-méthode, une méthode où sont utilisées des plaques de culture avec ultra basse adhérence, pour être la plus sûre et plus fiable pour la génération des sphéroïdes tridim...

Tête et cou carcinome épidermoïde (HNSCC) est le cancer le sixième plus répandu dans le monde entier avec une augmentation de l’incidence de la pathogenèse associée à une infection muqueuse virus du papillome humain (VPH), à côté de la majorité des cas causés par l’alcool et la nicotine excessive consommation 1,2. Tandis que les plus petites tumeurs et étapes pré-invasives sont en général bien traitables avec excision chirurgicale, généralement combinée avec dissection des ganglions cervicaux, traitement pour HNSCC avancée et récurrente demeure difficile en raison de l’invasion de tumeur agressive avec métastases et la résistance aux radiations et la chimiothérapie protocoles3,4,5,6,7,8. Des études récentes suggèrent une forte variabilité du phénotype cellulaire et caractérisation secondaire de circulation et les cellules tumorales disséminées vient de commencer9,10. La croyance antérieure d’une tumeur solide et uniforme masse devait être révisée à la lumière des études récentes dans le passé ans11,12,13,14. Les approches actuelles pour la caractérisation de la tumeur et l’identification de mutations clées pourraient identifier plusieurs gènes qui semblent être associés à la résistance de la thérapie, mais reste une démarche coûteuse. En outre, connaissance du génotype ne permet pas nécessairement une prévision fiable de phénotype et sa réponse au traitement.
Il y a eu quelques progrès dans l’amélioration de survie globale et indemne de la maladie pour la maladie avancée et récurrente. Pour la nicotine ainsi que carcinome associées aux virus, les options thérapeutiques actuelles en dehors de la chirurgie joindre rayonnement agressif et les chimiothérapies à base de platine. Il y a eu des répercussions sur les taux de réponse différents entre le carcinome HPV-négatif et positif ; Toutefois, cela n’a pas encore conduire à un changement en général directives de traitement. Résistance à la radiation et chimiothérapie est un phénomène répandu dans toutes les étapes de la tumeur et existe pour la chimiothérapie à base de platine aussi bien en ce qui concerne la thérapie ciblée (Anti-EGFR ; récepteur du facteur de croissance épidermique) et récemment émergentes checkpoint inhibition15. Chimiothérapie et la radiothérapie inefficace viennent à un coût élevé de morbidité importante de patients en termes de dysphagie, mucite, sécheresse de la bouche et risque de diminution de la fonction rénale ou cardiaque, entre autres. Prévoir avant l’intervention thérapeutique la décision d’un concept de thérapie générale pour chaque patient semble être l’objectif essentiel, empêchant les concepts de traitement inutile, les effets secondaires et les coûts.
Nous avons cherché à établir un modèle pour tester la sensibilité de traitement de chaque patient vers actuel chimio-rayonnement standard qui pourrait être intégrée dans l’algorithme de traitement oncologique régulière et contrôle de la qualité d’un point technique permanent. Le but lointain était d’utiliser le modèle sans utiliser des lignées de cellules fortement altérée et vieilli, car ils représentent mal les cellules tumorales humaines réelles sans leur variabilité et hétérogénéité comme nous le savons maintenant, alors que la mise en place du protocole a été faite dans diverses lignées cellulaires. Pour être indépendant qu’à partir de lignées cellulaires disponibles dans le commerce, nous avons généré récemment avec succès une lignée de cellules intermédiaire appelée « PiCa » de cellules primaires de HNSCC d’après des échantillons de tumeur humaine avec les marqueurs cellulaires conservées sur les passages de sa surfaces et limitées 16. lignée cellulaire PiCa ce devrait servir comme une préparation pour le développement du modèle sur la route à puis après essais avec des cellules cancéreuses humaines frais de biopsies tumorales. Il a été démontré que les cellules en trois dimensions cell cultures réagissent différemment et plus en vivo-comme à l’administration de médicaments contre le cancer que celles qui poussent dans les monocouches17,18,19,20 ,21, principalement en raison de la conservation des migrateurs et différenciation sous propriétés de certaines cellules des sous-ensembles22,23,24. Ici, les auteurs décrivent le protocole d’un modèle en trois dimensions, axée sur le sphéroïde de lignées de cellules intermédiaires et moyens et cellules humaines primaires carcinome spinocellulaire comment intègrent ce modèle dans le traitement du cancer de la tête et du cou chirurgien et oncologue ( Figure 1).

Les options thérapeutiques actuelles pour avancé et récurrente de tête et cou carcinome spinocellulaire (HNSCC) joindre rayonnement et approches de la chimio-rayons avec ou sans chirurgie. Alors que les chimiothérapies à base de platine actuellement représentent l’étalon-or en termes d’efficacité et sont indiquées dans la grande majorité des cas, les nouveaux schémas de chimiothérapie, à savoir l’immunothérapie font leur apparition. Toutefois, le taux de réponse et les mécanismes de résistance de thérapie pour chaque régime de chimio sont difficiles à prédire et restent insuffisamment compris. Grandes variations de chimiothérapie et radiation des mécanismes de résistance sont connues à ce jour. Cette étude décrit le développement d’un test standardisé, haut débit le in vitro pour évaluer réponse de lignée de cellules HNSCC à divers régimes de thérapie et j’espère que sur les cellules primaires de chaque patient comme un futur outil pour tumeur personnalisé thérapie. Le dosage est conçu pour être intégrée à l’algorithme standard de contrôle de la qualité pour les patients HNSCC à notre centre de soins tertiaires ; Toutefois, ce sera objet d’études futures. Faisabilité technique semble prometteuse pour les cellules primaires de biopsies de tumeurs de patients réels. Les échantillons sont ensuite transférées dans le laboratoire. Les biopsies sont séparées mécaniquement suivie par digestion enzymatique. Cellules sont ensuite cultivées en flacons de culture cellulaire ultra faible adhérence qui favorisent la formation reproductible, standardisée et spontanée des conglomérats de cellule en trois dimensions, en forme d’ellipsoïde. Sphéroïdes sont alors prêts à être exposés à des protocoles de chimio-rayons et protocoles d’immunothérapie au besoin. La taille de sphéroïde et de viabilité des cellules final sont des indicateurs de sensibilité de la thérapie et pourraient donc être tirées en considération à l’avenir pour évaluer la réaction probable de la thérapie des patients. Ce modèle pourrait être un outil précieux et rentable vers une thérapie personnalisée du cancer de la tête et du cou.

Therapy Testing in a Spheroid-based 3D Cell Culture Model for Head and Neck Squamous Cell Carcinoma

The overall goal of this in vitro method is to generate the three dimensional cell culture spheroids that enable the testing of current standard or experimental therapy regimens for head and neck squamous cell carcinoma. This method helps us to understand three dimensional tumor growth of head and neck cancer cells when exposed to radiation or chemo radiation treatment. It remains to be said that handling primary tumor cells is difficult and does not always lead to reliable three dimensional spheroid formation.Demonstrating the procedure will be Sabina Schwenk-Zieger, a technician from our laboratory. While using primary cells from a tumor specimen, place the specimen on a suitable and sterile surface and cut it thoroughly with a sterile, single-use scalpel into very small pieces. After sufficient mechanical separation of the primary tissue, transfer it into a vial containing collagenase 1 and 2 and incubate it for one hour at 37 degrees celsius.Next, sieve the mixture through a 70 micrometer Falcon cell strainer and wash the suspension with HBSS. After successful separation and subsequent washing, transfer the suspension containing one to two million cells into a T75 cell culture flask to grow to subconfluency at 37 degrees celsius for up to ten days. Under a microscope, confirm tumor cell growth.Count the cells in culture. Using an ultra-low adhesion 96-well plate with concave round bottoms, seed 5, 000 primary tumor cells, or 1, 000 to 2, 000 cells of intermediate cell line, in 200 to 300 microliters of media into the wells. Next, culture the cells at 37 degrees celsius.Perform media changes every other day. Pay attention not to aspirate the spheroid with the pipette during media changes. Culture the spheroids until the three dimensional spheroid-shaped cell conglomerates are observed.Keep in mind to exclude the wells with irregular or multiple spheroid formation from further investigation. Subsequently, change the media to media with chemo therapeutics and/or monoclonal antibodies at desired concentrations. Add Cisplatin at the concentrations of 2.5, five, or 10 micromolar or 5-Fluorouracil at 30 micromolar.In this procedure, measure the spheroid size after digital photo documentation on day six, day 10 and day 16 with a graphic software. After centrifugation of the plate at 520g for 2.5 minutes, remove the supernatant. Next, wash the cells with 1x PBS and centrifuge the plate again, followed by removing the supernatant.Next, add 100 microliters of enzymatic cell detachment solution to each vial to allow the spheroids to dissolve. Subsequently, incubate the plate for eight minutes at 37 degrees celsius. Check for successful dissolving of the spheroids under the microscope.Add 100 microliters of DMEM. Next, centrifuge the plate at 520g for 2.5 minutes. Then remove the supernatant and suspend the cells in 100 microliters of DMEM.Subsequently, if necessary, perform a commercially-available colorimetric proliferation assay and read the assay in an ELISA reader. All cell lines could generate reliable spheroids with differences in size and time of readout. Primary cells did form reproducible spheroids in ultra-low attachment plates.The Ki-67 staining reveals the periphery of the culture showing much higher proliferation rates than central cells, mimicking nutrient distribution in solid mucosal tumors that often show necrotic cores. Here, the effect of several chemo therapeutics and radiation on spheroid size. Radiation with two gray prior to treatment with Cisplatin, lead to a significant decrease in spheroid size compared to Cisplatin alone.With further establishment of generating spheroids from primary human cells, this is how the concept of therapy testing could be implemented. Spheroids would be generated after tumor biopsy, and then tested for molecular characteristics and therapy susceptibility. We were able to establish a protocol to generate reproducible spheroids from cell suspensions for both cell lines and primary human tumor cells.This multimodal assay is sufficiently sensitive to identify small differences between groups. In future, the assay could be used to first, assess individual response to standard and experimental therapy protocols. Second, further characterize the tumor cells from fresh specimens.And third, correlate therapy response to molecular patterns. The use of primary cells and the generation of spheroids would allow the preservation of as many characteristics of in vivo tumor growth as possible in combination with a cost-efficient assay for studying individual therapy response.
