Desde el advenimiento de la terapia antirretroviral, se ha producido una disminución sustancial de las muertes relacionadas con la monoinfección del VIH-1. Sin embargo, la enfermedad hepática ha surgido como una causa común de morbilidad en pacientes infectados por el VIH1,2. Las coinfecciones de virus de la hepatitis con infección por VIH-1 son más frecuentes, y representan entre el 10% y el 30% de las personas infectadas por el VIH en los Estados Unidos3,4,5.
La especificidad del huésped de los virus del VIH-1 y la hepatitis limita la utilidad de los modelos de animales pequeños para estudiar enfermedades infecciosas específicas del ser humano o para investigar múltiples aspectos de la patogénesis hepática asociada al VIH-1. Los ratones inmunodeficientes que permiten el injerto de células y/o tejidos humanos (llamados modelos de ratón humanizados) son modelos animales aceptables para estudios preclínicos6,7,8. Desde la introducción de ratones humanizados a principios de la década de 2000, múltiples estudios preclínicos de toxicidad hepática humana colestásica, patógenos específicos del ser humano, incluidos el VIH-1 y trastornos neurocognitivos asociados al VIH, el virus de Epstein Barr, hepatitis y otros enfermedades infecciosas, se han investigado en estos ratones6,9,10,11. Múltiples modelos de ratón para CD34+ HSCCP y/o trasplante de hepatocitos humanos han sido desarrollados durante mucho tiempo y han mejorado con el tiempo para estudiar la enfermedad patogénesis del virus de la hepatitis B (VHB) enfermedad hepática asociada12, 13 , 14. Varios modelos para el trasplante de HSPC y hepatocitos humanos (HEP) se basan en cepas, conocidas como NOG (NOD. Cg-Prkdcscid Il2rgtm1Sug/JicTac)8,13, NSG (NOD. Cg-Prkdcscid Il2rgtm1Wjl/SzJ)15, Balb/C-Rag2-/- c-/- (Rag2tm1.1Flv Il2rgtm1.1Flv/J)12, y fah-/- NOD rag1-/- il2r-null ratón16. Sin embargo, cada modelo tiene sus propias ventajas y limitaciones; por ejemplo, el fondo atravesado de dos ratones humanizados AFC8 para HEPs y células madre humanas (HSC) en un fondo Balb/C-Rag2-/- -/- permite el injerto exitoso de células inmunitarias y HSC, pero existe la ausencia de un fondo de células T y B específicos para antígenos respuesta en este modelo12. Las principales preocupaciones en la reconstitución de ratones humanizados dobles incluyen el injerto subóptimo, la falta de modelos adecuados para apoyar diferentes tejidos, condiciones no coincidentes, rechazo inmune, o enfermedad de injertofrentea huésped (GVHD), y técnicos dificultades, como manipulaciones de riesgo con recién nacidos y altas tasas de mortalidad debido a anomalías metabólicas13.
Aunque ratones humanizados se han utilizado para la investigación del VIH durante muchos años17,18,19, el uso de ratones humanizados para estudiar el daño hepático causado por el VIH-1 ha sido limitado20. Hemos informado previamente del establecimiento de un modelo de ratón TK-NOG humanizado dual y su aplicación en la enfermedad hepática asociada al VIH8. Este modelo muestra el injerto robusto de hígado y células inmunitarias y recapitula la patogénesis por infección por VIH. Esta discusión presenta un protocolo detallado, incluyendo los pasos más críticos en el trasplante de hepatocitos humanos. También se presenta una descripción de los HSCCP necesarios para un injerto exitoso de HEPs y el establecimiento de un sistema inmunitario funcional en ratones TK-NOG. Se detalla el uso de estos ratones para estudiar la inmunogénesis hepática asociada al VIH. Se utilizan ratones macho TK-NOG que llevan un transgén transgén transgén de virus del herpes simple tipo 1 específico del hígado. Las células hepáticas de ratón que expresan este transgén pueden ser fácilmente abladas después de una breve exposición a una dosis no tóxica de GCV. Las células hepáticas humanas trasplantadas se mantienen establemente dentro del hígado del ratón sin fármacos exógenos21. Los ratones también están preacondicionados con dosis no mieloablativas de treosulfano para crear un nicho en la médula ósea del ratón para las células humanas8. Los ratones inmunodeficientes TK-NOG se inyectan por vía intrasíptica con HEPs y HSPC multipotentes. A continuación, los ratones son monitoreados regularmente para la reconstitución de sangre y hígado mediante inmunofenotipado en sangre y mediciones de los niveles séricos de albúmina humana, respectivamente. Los ratones con una reconstitución exitosa de más del 15% tanto para las células inmunitarias humanas como para los HEP se inyectan por vía intraperitoneal con VIH-1. El efecto del VIH en el hígado se puede evaluar tan pronto como 4 - 5 semanas después de la infección. Es fundamental tener en cuenta que, debido a que se utiliza el VIH-1, se deben tomar todas las precauciones necesarias mientras se maneja el virus y se inyecta en ratones.

La citometría de flujo se realiza regularmente para evaluar el desarrollo de un sistema inmunitario funcional y para ver el efecto de la infección por VIH en las células inmunitarias.En ratones humanizados duales, el desarrollo de células inmunitarias funcionales puede variar del 15 al 90% de la puerta de linfocitos. Para la evaluación del injerto de hepatocitos humanos, ELIZA para los niveles de albúmina específicos para humanos se realiza mensualmente en el suero del ratón. Los ratones injertados con HSPC y HEPs muestran niveles de albúmina específicos para humanos que van desde aproximadamente siete microgramos por mililitro a 377 microgramos por mililitro a un mes, continuando creciendo en el tiempo de observación.El efecto de la infección por VIH en las células inmunitarias humanas en la sangre de ratones humanizados duales es monitoreado por citometría de flujo y sobre HEP en el hígado por albúmina específica para humanos ELIZA. A las cinco semanas, el VIH-1 causa una disminución en los niveles de albúmina humana en el suero y hay un agotamiento de los hepatocitos positivos CK18 humanos en las secciones hepáticas de ratones humanizados duales. Una menor proporción de CD4 a CD8 se observa típicamente en la sangre y el hígado de ratones infectados por el VIH, en comparación con los niveles observados en el mismo ratón antes de la infección.La sangre debe colocarse cuidadosamente en capas sobre el medio de separación de linfocitos para los aislamientos de la capa de color. Para la cirugía, sostenga suavemente el bazo e inserte la aguja en el polo inferior del bazo. Tras el aislamiento de las células madre hematopoyéticas, es crucial comprobar la pureza de las células madre hematopoyéticas positivas CD34 para evitar células positivas CD3 e inyecciones agudas de hepatocito de diferentes donantes.Como los ratones humanizados muestran el aspecto fisiológico del sistema inmunitario humano y el hígado, los ratones desarrollados podrían ser utilizados para estudiar la fisiopatogénesis del VIH y las infecciones físicas y cirrosis.
