الصرع هو الاضطراب العصبي الرابع الأكثر شيوعا: حوالي 1 ٪ من السكان يعانون من الصرع ، وحوالي ثلث المصابين بنوبات متكررة. في معظم الحالات ، يمكن التحكم في النوبات بالادويه. ومع ذلك ، العلاج الدوائي يحتاج إلى تعيين لكل مريض علي حده ، حيث الجرعات المناسبة يمكن ان يستغرق سنوات للعثور علي1،2. بالاضافه إلى ذلك, معظم الدواء له اثار جانبيه خطيره التي تقلل من نوعيه الحياة3, 4,5,6,7. وأخيرا ، في 30 ٪ من الحالات المرضي مقاومه للادويه ، وفي حاله وجود مولد النوبات واحد ثابت ، فقط جراحه المخ والأعصاب المعدية يمكن ان تخفف من حدوث نوبات8. ولذلك ، فان المبادرة الرئيسية في أبحاث الصرع الحديثة هي اكتشاف استراتيجيات جديده يمكن ان تمنع النوبات المتكررة في المرضي المعرضين للخطر ، مع الحد من ضرورة العلاجات الدوائية القوية والجراحات الجراحية المعدية.
نوبات الصرع تحدث عندما يكون هناك خلل في الدوائر المثيرة والمثبطة اما في جميع انحاء الدماغ (الصرع المعمم) أو في جزء موضعي من الدماغ (الصرع البؤري), مثل ان الخلايا العصبية التفريغ بطريقه غير طبيعيه9 , 10 , 11. الادويه المضادة للصرع يمكن ان تعمل بطريقتين مختلفتين في الوقاية من المضبوطات: اما تقليل الاثاره أو تعزيز تثبيط12. علي وجه التحديد ، فانها يمكن اما تعديل النشاط الكهربائي للخلايا العصبية عن طريق التاثير علي قنوات أيون في غشاء الخلية13 أو القانون علي نقل المواد الكيميائية بين الخلايا العصبية عن طريق التاثير علي الناقل العصبي المثبط GABA أو مثير الغلوتامات في الاشتباكات العصبية14،15. بالنسبة لبعض الادويه ، فان طريقه العمل غير معروفه18. أيضا ، العلاجات الدوائية لها تاثير مستمر علي المرضي ولا يمكن التكيف مع ديناميات انتشار النوبات. ومن الناحية المثالية ، فان المخدرات مع أليات محدده للعمل تعمل علي عمليات الصرع الاساسيه. والعلاج الأمثل لا تلمس الدماغ البينية ولكن ستعمل علي الفور عندما يبدا النوبة النامية. وعلي النقيض من ذلك ، في جميع حالات الصرع ، يعني الدواء الآن العلاج المنهجي ، والتي تؤثر علي الدماغ كله والجسم كله من المريض9.
يمكن ان تظهر نوبات الصرع بعد سنوات عديده من الاهانه الاوليه مثل صدمه الدماغ. الفترة بين الاهانه الاوليه وحدوث النوبات العفوية الاولي تتميز باعاده تنظيمات جزيئيه وخلوية كبيره ، بما في ذلك موت الخلايا العصبية مع اختفاء اتصالات الشبكة العصبية و محواري sprouting/neosynaptogenesis مع ظهور اتصالات جديده19،20،21. وبمجرد ان تصبح المضبوطات متكررة ، يميل تواترها وشدتها إلى الزيادة ، بما يشمل مناطق أكثر من المخ. ومن المهم التمييز بين مواقع ظهور المضبوطات (المناطق الصرعة) من شبكات الانتشار ، حيث ان قواعد نشاه المضبوطات وانتشارها قد تختلف. وقد قدمت البحوث التي أجريت علي الانسجه البشرية والنماذج التجريبية من الصرع البيانات الهامه فيما يتعلق باعاده تنظيم الدوائر وقدرتها علي توليد المضبوطات20،21،22، 23-بيد انه من الصعب تحديد ما إذا كانت هذه المنظمات هي ردود تكيف أو ما إذا كانت مرتبطة بالابيليبتوجينيسيس أو بنشاه المضبوطات والانتشار12.
