膵臓十二指腸切摘術(PD)は、高度な外科的処置と考えられている。死亡率は過去数十年で減少しているが、罹患率は依然として高い1である。最も頻繁に、恐れられている合併症は、術後膵瘻(POPF)であり、手術2、3、4、5の20%以上で起こる。POPFは、術後の切出血、胃の空の遅延、腹腔内膿瘍および敗血症などの後続の合併症を引き起こす可能性がある。入院費の増加、長期入院、癌患者における化学療法の開始の遅延は、劇的な社会経済的影響を及ぼし、2、3、4.
POPFの頻度および重症度を減らすには、過去数十年で異なる外科的アプローチが導入されている。そのうちの一つは、胆道および膵臓アナストモスのための単離されたRoux-en-Yループを伴う二重ループ再構成(DLR)である。胆汁から膵液を分離することは、膵臓jejunosmy(PJ)に対する相互活性化分泌物の有害な影響を減少させると考えられている。文献の結果は6、7、8を奨励している。しかし、手術時間はDLR4、9で有意に延長される。
ここでは、従来のシングルループ再構成と比較して胆汁から膵分泌を分離するためにPD中に修飾されたシングルループ再構成(mSLR)の技術を紹介する(SLR、図1)。この方法では、単一の長い小腸ループが肝術術(HJ)およびPJの構築に用いられる。HJのアフェレント四肢と発泡性四肢の間の左右の解剖学は胆汁および膵臓分泌物の隔離された流れを促進する。リスクの高い膵臓患者の死亡率と罹患率の低下を確認した最初の有望な結果は、2018年10月に当社グループによって報告されました。
この方法は、二重ループ再建と比較して手術期間を大幅に延長することなく膵液と胆汁を分離することにより、POPFの重症度を低減する。
