На первых этапах любой чрезвычайной гуманитарной помощи доступ к чистой воде, санитарии и гигиене имеет решающее значение для выживания пострадавших. Поэтому мониторинг качества воды является приоритетной задачей для предотвращения вспышек воды. Хорошо известно, что загрязненная вода часто является источником заболеваний, но часто бывает трудно определить источники вирусных вспышек, таких как вирус гепатита Е (HEV), даже при наличии обычных лабораторных методов. Контроль качества воды основан на количественной оценкеFIB 1, 2,3,4. Тем не менее, было широко документировано, что нет никакой корреляции между отсутствием FIB и наличие вирусных патогенов, передающихся через воду, таких как ротавирус (RoV), норовирус (NoV), или HEV5,6. Таким образом, использование критериев качества воды, основанных на FIB, может привести к недооценке рисков, связанных с наличием вирусных патогенов, передающихся через воду. Наблюдение за вирусами индикаторов, такими как аденовирусы человека (HAdV), или конкретные патогенные микроорганизмы будут полезны в определении воздействия вирусных патогенов и выявлении потенциального источника инфекции человека7,8, 9,10 и в проверке эффективности санитарных мер11.
До сих пор выявление вирусов в этих сценариях опиралось на квалифицированный персонал и сложную логистику. VirWaTest (virwatest.org) направлена на разработку простого, доступного и портативного метода концентрации и последующего обнаружения вирусов из проб воды в момент использования.
Концентрация вируса основана на принципе органической флоккуляции 10 Л проб воды, с помощью которых вирусы восстанавливаются в меньших объемах12,13. Флоксы собираются и добавляются в буфер, который лизирует вирусы и предотвращает деградацию нуклеиновых кислот при их комнатной температуре не более 2 недель.
Метод извлечения нуклеиновой кислоты основан на использовании магнитных частиц, к которым адсорбируются нуклеиновые кислоты. Они могут быть переданы из одного буфера стирки в другой и, наконец, в буфер elution с помощью магнитной пипетки, к которой прикрепляются частицы. Полученные вирусные нуклеиновые кислотные суспензии могут быть отправлены в референтную лабораторию, где обнаружение может быть выполнено с использованием молекулярных методов, основанных на ПЦР. Для каждой добычи нуклеиновой кислоты, два различных количества тестируются, чтобы исключить ферментативное ингибирование, возникшее в образе. Кроме того, при минимальной доступности оборудования, ПЦР-тесты могут быть запущены в точке использования. Весь процесс предназначен для выполнения независимо от источника питания(рисунок 1).
Количественный ПЦР-аналитика для обнаружения HAdV, выведенного человеком и обнаруженного в пробах сточных вод в высоких концентрациях, был адаптирован для запуска в точке применения. HAdV используются в качестве человека фекальные вирусные показатели. ПЦР для количественной оценки бактериофага MS2 также был адаптирован, так как MS2 используется в VirWaTest в качестве управления процессом. Метод может быть настроен для обнаружения любого вируса, интересуемого.
После разработки метод VirWaTest был применен пользователями в двух различных условиях в Центральноафриканской Республике (RCA) и Эквадоре, обеспечивая обратную связь о применении протокола в реальных ситуациях.
Насколько нам известно, это первая процедура, которая позволяет концентрации и обнаружения вирусов в точке использования, независимо от какого-либо источника питания, большого оборудования и условий замораживания/охлаждения. virwatest, a point-of-use method for the detection of viruses in water samples

Вирусы, выделяемые людьми и животными, могут загрязнять источники воды и представлять опасность для здоровья человека, когда эта вода используется для питья, орошения пищевых продуктов, мытья и т.д. Классический индикатор фекальных бактерий не всегда проверяет наличие вирусных патогенов, поэтому обнаружение вирусных патогенов и вирусных показателей имеет отношение к принятию мер по снижению риска, особенно в гуманитарных сценариях и в районах где часто происходят вспышки вирусной инфекции, передаваемой через воду.
В настоящее время для тестирования в точке использования доступно несколько коммерческих тестов, позволяющих количественно определить бактерии фекальных индикаторов (FIB). Однако такие коммерческие тесты недоступны для обнаружения вирусов. Обнаружение вирусов в пробах воды в окружающей среде требует концентрации нескольких литров в меньших объемах. Кроме того, после сосредоточения, обнаружение вирусов опирается на такие методы, как добыча нуклеиновой кислоты и молекулярное обнаружение (например, полимераза цепной реакции (PCR основе анализов) вирусных геномов.
Описанный здесь метод позволяет концентрацию вирусов из 10 L проб воды, а также извлечение вирусных нуклеиновых кислот в точке использования, с простым и портативным оборудованием. Это позволяет проводить тестирование проб воды в точке применения нескольких вирусов и полезно в гуманитарных сценариях, а также в любом контексте, где не имеется оборудованной лаборатории. Кроме того, метод позволяет концентрировать вирусы, присутствующие в пробах воды, и отгружать концентрат в лабораторию при комнатной температуре для дальнейшего анализа.
