Il nostro protocollo è stato progettato per aumentare il ritardo nel recupero dall'acidosi intracellulare per potenziare gli effetti cardioprotettivi del protocollo originale di postcondizionamento. Nessun agente farmacologico o attrezzatura speciale è necessario per questo metodo, quindi è molto sicuro e prontamente disponibile in qualsiasi laboratorio di cateterizzazione in tutto il mondo. La maggior parte dei pazienti potrebbe non essere in grado di riprodurre il postcondizionamento con procedure ematiche arricchite di lattato, o PCLeB, solo leggendo documenti di laboratorio.
Quindi la dimostrazione visiva può aiutare con l'implementazione. Dopo aver individuato la lesione oclusa mediante angiografia coronarica e aver terminato le procedure di cablaggio, spostare il catetere del palloncino in avanti e posizionare il palloncino sulla lesione oclusa. Per determinare se il palloncino è posizionato in modo appropriato alla lesione, fornire un mezzo di contrasto di grande volume nell'arteria coronarica colpevole.
Dopo aver controllato la posizione del palloncino, il mezzo di contrasto riempirà il lume dal collettore alla punta del catetere guida. Scollegare l'iniettore della siringa dal collettore e collegare una siringa da trenta millilitri. Quindi aspirare da venti a trenta millilitri della soluzione di Ringer allattata attraverso la linea collegata all'ingresso del collettore fino a quando la siringa non viene riempita.
Una volta riempita la siringa, gonfiare il catetere a palloncino per venti o trenta secondi a bassa pressione, quindi sgonfiare il palloncino e iniziare la riperfusione iniziale di dieci secondi. All'inizio della riperfusione, erogare immediatamente almeno quattro millilitri della soluzione di Ringer per spingere tutto il mezzo di contrasto dalla punta del catetere guida per visualizzare se il flusso coronarica è stato recuperato. Se il flusso coronarica viene recuperato, reintegrare la siringa con lo stesso volume di soluzione ringer lattata utilizzata per spingere fuori il mezzo di contrasto.
Mantenendo il palloncino all'occlusione, iniettare la soluzione di Ringer allattata direttamente nell'arteria coronarica colpevole attraverso il catetere guida per un periodo di cinque-sette secondi. Nel mezzo dell'iniezione, fare in modo che un operatore secondario inizi l'inflazione del palloncino in modo che l'inflazione sia completata poco prima del completamento dell'iniezione di soluzione di Ringer allattata. Durante il sessantadue secondo periodo di ischemia, ricaricare la siringa con quattro millilitri di mezzo di contrasto e caricare il mezzo nel catetere guida.
Quindi ricaricare la siringa con venti o trenta millilitri di soluzione ringer fresca allattata. Alla fine del periodo dell'ischemia, sgonfiare il palloncino e iniziare la successiva riperfusione, erogando tutto il mezzo di contrasto preriempato per riconfermare il flusso coronario. Quindi avviare l'iniezione di soluzione della suoneria allattata nell'arteria coronarica colpevole attraverso il catetere guida da quattro a cinque secondi prima della fine della riperfusione, facendo in modo che l'operatore secondario inizi l'inflazione del palloncino nel mezzo dell'iniezione di soluzione della suoneria allattata come appena dimostrato.

La riperfusione con PCLeB è stata avviata sessanta minuti dopo l'inizio del sintomo. La pressione sistolica aortica aumentò a circa settantacinque millilitri di mercurio alla fine del PCLeB e a ottanta millilitri di mercurio dopo l'intervento coronarica percutaneo.
La trombolisi nel flusso di infarto miocardico III è stata raggiunta dopo PCLeB, ed è stata ancora osservata dopo stenting e intervento coronarica percutaneo. L'elettrocardiografia registrata al momento dell'ammissione all'unità di terapia intensiva ha rivelato una risoluzione ST completa. La scintigrafia del tallio a riposo prima dello scarico ha rivelato una vitalità miocardica ben conservata.
Il passo più importante è intrappolare un volume maggiore di soluzione di Ringer lattato all'interno del miocardio ischemico in una forma meno diluita durante ogni breve periodo ischemico ripetuto. Lo stenting può essere tranquillamente preformato senza paura del fenomeno di non riflusso in contrasto con la teoria generalmente creduta, l'embolico distale potrebbe non essere responsabile del fenomeno di non riflusso.
