Erratum: In vivo Mouse Model of Spinal Implant Infection

Het doel van deze methode is om een nieuw muismodel van spinale implantaatinfectie (SII) te beschrijven. Dit model is ontworpen om een goedkoop en nauwkeurig hulpmiddel te bieden om het effect van gastheer-, pathogeen- en/of implantaatvariabelen in vivo flexibel te beoordelen. Het testen van potentiële therapieën en behandelingsstrategieën voor spinale implantaatinfecties in dit model is gericht op het begeleiden van onderzoeksontwikkeling voorafgaand aan toepassing in grotere diermodellen en klinische proeven.
Implantaatgerelateerde infectie na een wervelkolomoperatie is een verwoestende complicatie en komt helaas voor bij ongeveer 3-8% van de patiënten die een electieve wervelkolomoperatie ondergaan 1,2,3,4,5 en tot 65% van de patiënten die een multilevel- of revisieoperatie ondergaan 6. Behandeling van spinale implantaatinfecties vereist vaak meerdere ziekenhuisopnames, meerdere operaties en langdurige antibioticatherapie. SII's voorspellen slechte resultaten voor de patiënt, waaronder neurologische compromittering, invaliditeit en een verhoogd risico op sterfte. Het beheer van SII is extreem duur en kost meer dan $ 900,000 per patiënt7.
Staphylococcus aureus is de meest voorkomende virulente ziekteverwekker van SII 8,9,10,11. Bacteriën kunnen de hardware direct tijdens de operatie zaaien, via de wond tijdens de postoperatieve periode of later via hematogene verspreiding. In aanwezigheid van metalen implantaten vormt S. aureus een biofilm die de bacteriën beschermt tegen antibioticatherapie en immuuncellen. Hoewel het verwijderen van geïnfecteerde hardware kan helpen om een infectie effectief uit te roeien, is dit vaak niet haalbaar in de wervelkolom zonder destabilisatie te veroorzaken en neurologische compromittering te riskeren12.
Bij gebrek aan explantatie van geïnfecteerde hardware zijn nieuwe benaderingen nodig om SII te voorkomen, op te sporen en te behandelen. Historisch gezien zijn er beperkte diermodellen van SII om de veiligheid en werkzaamheid van nieuwe therapieën efficiënt te beoordelen. Eerdere diermodellen van SII vereisen grote aantallen dieren en het verzamelen van datapunten die euthanasie vereisen, waaronder het tellen van kolonies, histologie en cultuur13,14,15. Bij gebrek aan longitudinale in vivo monitoring bieden deze modellen slechts één datapunt per dier en zijn daarom duur en inefficiënt.
Eerder werk dat een muismodel van knieartroplastiekinfectie bestudeerde, stelde de waarde en nauwkeurigheid vast van niet-invasieve in vivo optische beeldvorming om de infectielast longitudinaal te bewaken16. De detectie van bioluminescentie maakt het mogelijk om de bacteriële belasting over een longitudinaal tijdsverloop in een enkel dier humaan, nauwkeurig en efficiënt te kwantificeren. Bovendien hebben eerdere studies een hoge correlatie aangetoond tussen in vivo bioluminescentie en CFU's die aan implantaten hechten17. Het vermogen om infecties in de loop van de tijd te volgen, heeft geleid tot een meer genuanceerd begrip van implantaatgerelateerde infecties. Bovendien heeft het op deze manier monitoren van longitudinale infecties het mogelijk gemaakt om de effectiviteit van antibioticatherapie en nieuwe antimicrobiële stoffen nauwkeurig te beoordelen16,17,18.
Met behulp van deze hulpmiddelen hebben we een model van postoperatieve spinale implantaatinfectie ontwikkeld en gevalideerd. In de gepresenteerde methode gebruiken we een inoculum van bioluminescente S. aureus Xen36 om een in vivo muismodel van SII op te stellen om de bacteriële belasting longitudinaal te monitoren16,17,18. Dit nieuwe model biedt een waardevol hulpmiddel om potentiële detectie-, preventie- en behandelingsstrategieën voor SII efficiënt te testen voordat ze worden toegepast in grotere diermodellen en klinische onderzoeken.

in vivo mouse model of spinal implant infection

Infecties van wervelkolomimplantaten voorspellen slechte resultaten, aangezien de diagnose een uitdaging is en chirurgische uitroeiing op gespannen voet staat met de mechanische stabiliteit van de wervelkolom. Het doel van deze methode is het beschrijven van een nieuw muismodel van spinale implantaatinfectie (SII) dat is gemaakt om een goedkoop, snel en nauwkeurig in vivo hulpmiddel te bieden om potentiële therapieën en behandelingsstrategieën voor spinale implantaatinfecties te testen.
In deze methode presenteren we een model van spinale chirurgie in de posterieure benadering waarbij een roestvrijstalen k-draad wordt gefixeerd in het L4-processus spinosus van 12 weken oude C57BL/6J wildtype muizen en wordt geïnoculeerd met 1 x 103 CFU van een bioluminescente stam van Staphylococcus aureus Xen36-bacteriën. Muizen worden vervolgens longitudinaal in beeld gebracht voor bioluminescentie in vivo op postoperatieve dagen 0, 1, 3, 5, 7, 10, 14, 18, 21, 25, 28 en 35. Bioluminescentiebeeldvorming (BLI)-signalen uit een gestandaardiseerd gezichtsveld worden gekwantificeerd om in vivo de bacteriële belasting te meten.
Om bacteriën te kwantificeren die zich hechten aan implantaten en peri-implantair weefsel, worden muizen geëuthanaseerd en worden het implantaat en het omliggende zachte weefsel geoogst. Bacteriën worden losgemaakt van het implantaat door middel van sonicatie, 's nachts gekweekt en vervolgens worden kolonievormende eenheden (CFU's) geteld. De resultaten van deze methode omvatten longitudinale bacterietellingen zoals gemeten door in vivo S. aureus-bioluminescentie (gemiddelde maximale flux) en CFU-tellingen na euthanasie.
Terwijl eerdere diermodellen van geïnstrumenteerde wervelkolominfectie invasieve, ex vivo weefselanalyse hebben omvat, maakt het muismodel van SII dat in dit artikel wordt gepresenteerd, gebruik van niet-invasieve, real-time in vivo optische beeldvorming van bioluminescente bacteriën om statisch weefselonderzoek te vervangen. Toepassingen van het model zijn breed en kunnen bestaan uit het gebruik van alternatieve bioluminescente bacteriestammen, het opnemen van andere soorten genetisch gemanipuleerde muizen om tegelijkertijd de immuunrespons van de gastheer te bestuderen, en het evalueren van huidige of het onderzoeken van nieuwe diagnostische en therapeutische modaliteiten zoals antibiotica of implantaatcoatings.

De hier gepresenteerde procedure werd gebruikt om de werkzaamheid van antibioticaregimes te beoordelen in een in vivo muismodel van SII. In het bijzonder werd de werkzaamheid van combinatietherapie met vancomycine- en rifampicine-antibiotica vergeleken met monotherapie met vancomycine en onbehandelde geïnfecteerde controles.
