Das Ziel dieser Methode ist es, ein neuartiges Mausmodell der spinalen Implantatinfektion (SII) zu beschreiben. Dieses Modell wurde entwickelt, um ein kostengünstiges und genaues Werkzeug zur flexiblen Bewertung der Wirkung von Wirts-, Erreger- und/oder Implantatvariablen in vivo bereitzustellen. Die Erprobung potenzieller Therapeutika und Behandlungsstrategien für Wirbelsäulenimplantatinfektionen in diesem Modell zielt darauf ab, die Forschungsentwicklung vor der Anwendung in größeren Tiermodellen und klinischen Studien zu steuern.
Implantatbedingte Infektionen nach Wirbelsäulenoperationen sind eine verheerende Komplikation und treten leider bei etwa 3–8 % der Patienten auf, die sich einer elektiven Wirbelsäulenoperation unterziehen 1,2,3,4,5 und bis zu 65 % der Patienten, die sich einer Mehrebenen- oder Revisionsoperation unterziehen 6. Die Behandlung von Wirbelsäulenimplantatinfektionen erfordert oft mehrere Krankenhausaufenthalte, mehrere Operationen und eine längere Antibiotikatherapie. SIIs deuten auf schlechte Patientenergebnisse hin, einschließlich neurologischer Beeinträchtigungen, Behinderungen und eines erhöhten Mortalitätsrisikos. Die Behandlung von SII ist extrem teuer und kostet mehr als 900.000 US-Dollar pro Patient7.
Staphylococcus aureus ist der häufigste virulente Erreger von SII 8,9,10,11. Bakterien können die Hardware direkt während der Operation, durch die Wunde während der postoperativen Phase oder später durch hämatogene Ausbreitung aussäen. In Gegenwart von Metallimplantaten bildet S. aureus einen Biofilm, der die Bakterien vor einer Antibiotikatherapie und Immunzellen schützt. Während die Entfernung infizierter Hardware dazu beitragen kann, eine Infektion effektiv auszurotten, ist dies in der Wirbelsäule häufig nicht möglich, ohne eine Destabilisierung zu verursachen und eine neurologische Beeinträchtigung zu riskieren12.
Da infizierte Hardware nicht explantiert werden kann, sind neue Ansätze zur Vorbeugung, Erkennung und Behandlung von SII erforderlich. In der Vergangenheit gab es nur begrenzte Tiermodelle für SII, um die Sicherheit und Wirksamkeit neuartiger Therapien effizient zu bewerten. Bisherige Tiermodelle der SII erfordern eine große Anzahl von Tieren und die Erfassung von Datenpunkten, die Euthanasie erfordern, einschließlich Koloniezählung, Histologie und Kultur13,14,15. Da es keine longitudinale In-vivo-Überwachung gibt, liefern diese Modelle nur einen Datenpunkt pro Tier und sind daher teuer und ineffizient.
Frühere Arbeiten zur Untersuchung eines Mausmodells für Knieendoprothetikinfektionen erwiesen den Wert und die Genauigkeit der nicht-invasiven optischen In-vivo-Bildgebung zur longitudinalen Überwachung der Infektionslast16. Der Nachweis von Biolumineszenz ermöglicht es, die bakterielle Belastung über einen longitudinalen Zeitverlauf in einem einzelnen Tier human, genau und effizient zu quantifizieren. Darüber hinaus haben frühere Studien eine hohe Korrelation zwischen In-vivo-Biolumineszenz und an Implantaten haftenden KBE gezeigt17. Die Fähigkeit, Infektionen im Laufe der Zeit zu verfolgen, hat zu einem nuancierteren Verständnis von implantatbedingten Infektionen geführt. Darüber hinaus hat die Überwachung der longitudinalen Infektion auf diese Weise eine genaue Bewertung der Wirksamkeit der Antibiotikatherapie und neuartiger antimikrobieller Mittel ermöglicht16,17,18.
Mit Hilfe dieser Werkzeuge haben wir ein Modell der postoperativen Wirbelsäulenimplantatinfektion entwickelt und validiert. In der vorgestellten Methode verwenden wir ein Inokulum von biolumineszierendem S. aureus Xen36, um ein in vivo Mausmodell der SII zu etablieren, um die bakterielle Belastung longitudinal zu überwachen16,17,18. Dieses neuartige Modell bietet ein wertvolles Werkzeug, um potenzielle Erkennungs-, Präventions- und Behandlungsstrategien für SII effizient zu testen, bevor sie in größeren Tiermodellen und klinischen Studien angewendet werden.

Infektionen mit Wirbelsäulenimplantaten deuten auf schlechte Ergebnisse hin, da die Diagnose schwierig ist und die chirurgische Eradikation im Widerspruch zur mechanischen Stabilität der Wirbelsäule steht. Der Zweck dieser Methode ist es, ein neuartiges Mausmodell für spinale Implantatinfektionen (SII) zu beschreiben, das entwickelt wurde, um ein kostengünstiges, schnelles und genaues In-vivo-Werkzeug zum Testen potenzieller Therapeutika und Behandlungsstrategien für Wirbelsäulenimplantatinfektionen bereitzustellen.
In dieser Methode stellen wir ein Modell der posterioren Wirbelsäulenchirurgie vor, bei dem ein K-Draht aus Edelstahl in den Dornfortsatz L4 von 12 Wochen alten C57BL/6J-Wildtyp-Mäusen eingedrungen und mit 1 x 103 KBE eines biolumineszierenden Stammes des Staphylococcus aureus Xen36-Bakteriums inokuliert wird. Die Mäuse werden dann an den postoperativen Tagen 0, 1, 3, 5, 7, 10, 14, 18, 21, 25, 28 und 35 in vivo longitudinal für Biolumineszenz abgebildet. Biolumineszenz-Bildgebungssignale (BLI) aus einem standardisierten Sichtfeld werden quantifiziert, um die bakterielle Belastung in vivo zu messen.
Um Bakterien zu quantifizieren, die an Implantaten und periimplantärem Gewebe haften, werden Mäuse euthanasiert und das Implantat und das umgebende Weichgewebe entnommen. Die Bakterien werden durch Beschallung vom Implantat gelöst, über Nacht kultiviert und dann werden koloniebildende Einheiten (KBE) gezählt. Zu den Ergebnissen, die mit dieser Methode gewonnen werden, gehören longitudinale Bakterienzahlen, gemessen durch in vivo S. aureus-Biolumineszenz (mittlerer maximaler Fluss) und KBE-Zählungen nach Euthanasie.
Während frühere Tiermodelle für instrumentierte Wirbelsäuleninfektionen eine invasive Ex-vivo-Gewebeanalyse beinhalteten, nutzt das in dieser Arbeit vorgestellte Mausmodell der SII die nicht-invasive, optische Echtzeit-In-vivo-Bildgebung von biolumineszierenden Bakterien, um statische Gewebeuntersuchungen zu ersetzen. Die Anwendungen des Modells sind breit gefächert und können die Verwendung alternativer biolumineszierender Bakterienstämme, die Einbeziehung anderer Arten von gentechnisch veränderten Mäusen zur gleichzeitigen Untersuchung der Immunantwort des Wirts und die Bewertung aktueller oder die Untersuchung neuer diagnostischer und therapeutischer Modalitäten wie Antibiotika oder Implantatbeschichtungen umfassen.

Das hier vorgestellte Verfahren wurde verwendet, um die Wirksamkeit von Antibiotikatherapien in einem in vivo Mausmodell der SII zu bewerten. Insbesondere wurde die Wirksamkeit einer kombinierten Antibiotikatherapie mit Vancomycin und Rifampicin mit einer Vancomycin-Monotherapie und unbehandelten infizierten Kontrollen verglichen.
