La reparación de fracturas es un proceso dinámico y multicelular que es distinto del desarrollo y la remodelación del esqueleto embrionario. Durante este proceso, la inducción de señales de lesión del tejido dañado es seguida por el rápido reclutamiento, proliferación y posterior diferenciación de las células madre/progenitoras esqueléticas, que son críticas para la estabilidad general y la fijación de las fracturas1. En particular, las primeras etapas de la cicatrización de la fractura requieren la formación de callos blandos, que se atribuye principalmente a las células residentes del periostio2. Cuando los huesos se lesionan, un subconjunto de células periósticas responden rápidamente y contribuyen a los intermedios de cartílago y osteoblastos recién diferenciados dentro del callo3, lo que implica la presencia de una población distinta de células madre/progenitoras esqueléticas dentro del periostio. Por lo tanto, la identificación y caracterización funcional de las células madre esqueléticas residentes en el periostio (CSS) constituyen un enfoque terapéutico prometedor para las enfermedades óseas degenerativas y los defectos óseos4.
Se cree que las SSC residen en múltiples ubicaciones de tejidos, incluida la médula ósea. Al igual que en la médula ósea, también se han identificado CSS osteogénicas/condrogénicas en el periostio4,5,6.  Estas SSCs periósticas (P-SSCs) se pueden etiquetar con marcadores de linaje mesenquimal temprano (es decir, Prx1-Cre5, Ctsk-Cre7 y Axin2-CreER8) durante el desarrollo óseo fetal4,7,8,9,10. Una limitación significativa de estos modelos de rastreo de linaje genético único es que existe una heterogeneidad sustancial dentro de las poblaciones celulares marcadas. Además, no pueden distinguir las CSS etiquetadas de su progenie in vivo. Para abordar esta limitación, recientemente desarrollamos un ratón reportero dual (Mx1-Cre+Rosa26-Tomato+α SMA-GFP+) para visualizar claramente las P-SSCs de las SSCs de médula ósea (BM-SSCs)11. Con este modelo, se ha determinado que las P-SSC están marcadas por el etiquetado dual Mx1+αSMA+, mientras que las células Mx1+ más diferenciadas residen en la médula ósea, las superficies endodesteal y perióstica, y los huesos corticales y trabeculares11,12.
Se sabe que múltiples citoquinas y factores de crecimiento regulan la remodelación y reparación ósea y han sido probados para la mejora de la reparación esquelética en defectos críticos del segmento1,13. Sin embargo, debido a la complejidad celular de los modelos de lesión generados a través de la interrupción de la barrera física del compartimento óseo, los efectos directos de estas moléculas sobre la migración endógena de P-SSC y la activación durante la curación no están claros. Las características funcionales y la dinámica migratoria de las CSS a menudo se evalúan in vitro mediante la realización de un ensayo de transpozo o rasguño, en combinación con citoquinas o factores de crecimiento que se sabe que inducen la migración en otras poblaciones celulares. Por lo tanto, la interpretación de los resultados de estos experimentos in vitro para su aplicación en sus correspondientes sistemas in vivo es un desafío. Actualmente, la evaluación in vivo de la migración de células madre/progenitoras esqueléticas no se observa típicamente en tiempo real; más bien, se mide en puntos de tiempo fijos después de la fractura5,7,14,15,16.
Una limitación de este método es que la migración no se evalúa a nivel de una sola célula; más bien, se mide a través de cambios en las poblaciones celulares. Debido a los recientes avances en la microscopía intravital de animales vivos y la generación de ratones reporteros adicionales, ahora es posible el seguimiento in vivo de células individuales. Utilizando microscopía intravital de animales vivos, observamos la migración distinta de P-SSCs desde el nicho de sutura de calvaria a una lesión ósea dentro de las 24-48 h posteriores a la lesión en ratones Mx1 / Tomate / αSMA-GFP.
CCL5/CCR5 se identificó recientemente como un mecanismo regulador que influye en el reclutamiento y la activación de P-SSCs durante la respuesta temprana a la lesión. Curiosamente, no hubo detección de una migración sustancial de P-SSC en respuesta a una lesión en tiempo real. Sin embargo, el tratamiento de una lesión con CCL5 produce una migración robusta y direccionalmente distinta de P-SSC, que se puede capturar en tiempo real. Por lo tanto, el objetivo de este protocolo es proporcionar una metodología detallada para registrar la migración in vivo de P-SSC en tiempo real después del tratamiento con CCL5.

Las células madre esqueléticas periósticas (P-SSC) son esenciales para el mantenimiento y la reparación ósea de por vida, lo que las convierte en un foco ideal para el desarrollo de terapias para mejorar la curación de fracturas.  Las células periósticas migran rápidamente a una lesión para suministrar nuevos condrocitos y osteoblastos para la curación de fracturas. Tradicionalmente, la eficacia de una citoquina para inducir la migración celular solo se ha llevado a cabo in vitro mediante la realización de un ensayo de transpozo o rasguño. Con los avances en la microscopía intravital utilizando excitación multifotónica, recientemente se descubrió que 1) las P-SSC expresan el gen migratorio CCR5 y 2) el tratamiento con el ligando CCR5 conocido como CCL5 mejora la cicatrización de fracturas y la migración de P-SSCs en respuesta a CCL5. Estos resultados han sido capturados en tiempo real. Aquí se describe un protocolo para visualizar la migración de P-SSC desde el nicho de células madre esqueléticas (SSC) de sutura calvarial hacia una lesión después del tratamiento con CCL5. El protocolo detalla la construcción de una sujeción del ratón y un soporte de imágenes, la preparación quirúrgica de la calvaria del ratón, la inducción de un defecto de la calvaria y la adquisición de imágenes de lapso de tiempo.
