Dit protocol beschrijft een knaagdiermodel van DAI. In DAI veroorzaakt rotatieversnelling op de hersenen een afschuifeffect dat axonale en biochemische veranderingen teweegbrengt die leiden tot verlies van axonale functie in een progressief proces. Traumatisch hersenletsel (TBI) is een belangrijke oorzaak van overlijden en invaliditeit in de Verenigde Staten. Tberose dragen bij tot ongeveer 30% van alle schadegerelateerde sterfgevallen1,2. De belangrijkste oorzaken van TBI verschillen tussen leeftijdsgroepen en omvatten valpartijen, hoge snelheid botsingen tijdens sport, opzettelijke zelfbeschadiging, motorvoertuig crashes en aanvallen1,2,3.
Brain diffuse axonale letsel (DAI) is een specifiek type TBI veroorzaakt door rotatieversnelling, schudden of ontploffing letsel van de hersenen als gevolg van onbeperkte hoofdbeweging in het moment na letsel4,5,6,7,8. Dai gaat gepaard met wijdverbreide axonale schade die leidt tot langdurige neurologische stoornissen die gepaard gaan met een slechte afloop, omslachtige kosten voor gezondheidszorg en een sterftecijfer van 33-64%1,2,4,5,9,10,11. Ondanks een belangrijk recent onderzoek naar de pathogenese van DAI, is er geen consensus over de beste behandelingsopties11,12,13,14.
In de afgelopen decennia hebben talrijke experimentele modellen geprobeerd om verschillende aspecten van DAI11,12,15,16nauwkeurig te repliceren . Echter, deze modellen hebben beperkingen gezien de unieke presentatie van DAI in vergelijking met andere focale verwondingen. Deze eerdere modellen veroorzaken niet alleen axonale letsel in witte stof regio's, maar ook resulteren in focale hersenletsel. Klinisch gaat DAI gepaard met microbloedingen, die een belangrijke oorzaak van schade aan witte stof kunnen vormen.
Slechts twee diermodellen zijn aangetoond dat de belangrijkste klinische kenmerken van DAI repliceren. Gennarelli en collega's produceerden de eerste laterale hoofd rotatie apparaat in 1982, met behulp van non-impact hoofd rotatie versnelling coma te induceren met DAI in een niet-menselijke primaten model15. Dit primatenmodel gebruikte gecontroleerde enkele rotatie voor versnelling en vertraging om het hoofd door 60° binnen 10-20 ms te verplaatsen. Primatenmodellen zijn echter erg duur4,11,16. Deels gebaseerd op het vorige model, een varken model van rotatie versnelling hersenletsel werd ontworpen in 1994 (Ross et al.) met vergelijkbare resultaten14.
Deze twee diermodellen, hoewel ze verschillende presentaties van typische pathologie produceerden, hebben sterk bijgedragen aan de concepten van DAI pathogenese. Snelle hoofdrotatie wordt algemeen aanvaard als de beste methode voor het induceren van DAI, en knaagdieren bieden een minder duur model voor de snelle hoofdrotatie studies11,16. Hier valideren we een eenvoudig, reproduceerbaar en betrouwbaar knaagdiermodel van DAI dat wijdverspreide witte stofschade veroorzaakt zonder schedelbreuken of kneuzingen. Dit huidige model zal een beter begrip van de pathofysiologie van DAI en de ontwikkeling van meer effectieve behandelingen mogelijk maken.

induction of diffuse axonal brain injury in rats based on rotational acceleration

Traumatisch hersenletsel (TBI) is een belangrijke oorzaak van overlijden en invaliditeit. Diffuus axonal letsel (DAI) is het overheersende letselmechanisme bij een groot percentage TBI-patiënten die ziekenhuisopname nodig hebben. DAI gaat wijdverbreide axonale schade door schudden, rotatie of blast letsel, wat leidt tot snelle axonale stretch letsel en secundaire axonale veranderingen die worden geassocieerd met een langdurige impact op functioneel herstel. Historisch gezien zijn experimentele modellen van DAI zonder brandpuntsletsel moeilijk te ontwerpen. Hier valideren we een eenvoudig, reproduceerbaar en betrouwbaar knaagdiermodel van DAI dat wijdverspreide witte stofschade veroorzaakt zonder schedelbreuken of kneuzingen.

Tabel 1 illustreert de protocoltijdlijn. Het sterftecijfer in dit model van DAI was 0%. Een Mann-Whitney-test wees uit dat het neurologischtekort aanzienlijk groter was voor de 15 DAI-ratten in vergelijking met de 15 sham ratten op 48 uur na interventie (Mdn = 1 vs. 0), U = 22,5, p < 0,001, r = 0,78 (zie tabel 2). De gegevens worden gemeten in tellingen en worden gepresenteerd als mediaan en 25-75 percentiel bereik.

Traumatisch hersenletsel is een van de belangrijkste oorzaken dood en invaliditeit. Diffuus axonaal hersenletsel is een wijdverspreide axonale schade te ontwikkelen na TBI. In dit onderzoek wil ik ons model presenteren over inductie diffuse axale hersenletsel op basis van rotatieversnellingsmechanisme resultaat TBI.Selecteer volwassen mannelijke Sprague-Dawley ratten met een gewicht van 300 tot 350 gram. Zorg voor rat chow en water ad libitum. Voer alle experimenten uit tussen 06:00 en 12:00 uur. Het apparaat werd op een zware, stabiele laboratoriumtafel geplaatst.Het apparaat bestaat uit de volgende componenten: een cilinder, een ijzergewicht, een rotatiemechanisme bestaande uit een cilindrische buis en kopfixatiepennen. Het gewicht is bevestigd aan een string en verhoogd tot de hoogte van 120 centimeter. Het vrij dalende gewicht raakt de bout die het rotatiemechanisme activeert.De rat laterale hoofd rotatie apparaat werd gebruikt om het hoofd snel draaien van nul tot 90 graden. Na inductie, diffuus axonaal hersenletsel, werd de rat verplaatst naar de herstelkamer. F is de kracht die in kilogram op het hoofd van het dier wordt uitgeoefend.Capital M is het moment van de kracht, K voor kinetische energie. Weinig m is de massa van het dalende gewicht, g voor gravitatieversnelling, h voor hoogte in centimeters, en D is de afstand tussen de oorspelden in centimeters. Voor de beoordeling van neurologisch tekort en cijfermodeltekorten, gebruikten we Neurologische Ernst Score bij ratten.We evalueren mobiliteit, hemiplegie, balk lopen taak, falen in balk lopen taak. Storing van balk balanceren taak breed 1,5 centimeter. Stabiliteit en bundel balanceren, inspanning op bundel balans 1,5 centimeter breed en reflexen.48 uur na letsel, alle ratten, letsel en controlegroep, waren diep verdoofd en transcardieel geperfuseerd met 0,9% heparinized zoutoplossing gevolgd door 500 milliliter van 4% formaldehyde in 0,1 M fosfaatbuffer zoutoplossing. Na perfusie werd onthoofding geproduceerd en werden de hersenen onmiddellijk verwijderd, vervolgens gedurende 48 uur in een 4% formaldehydeoplossing bevestigd. De hersenen werden vervolgens geblokkeerd in vijf millimeter coronale secties van de olfactorische bol gezicht naar de visuele cortex, terwijl het cerebellum en hersenstam werden ontleed.Na paraffine inbedding, coronale en sagittale secties werden gemaakt van de thalamus. Produceren van de immunochemische kleuring van bèta-amyloïde precursor eiwit. De verschillende groepen ratten op verschillende tijdstippen worden op de regeling weergegeven.Diffuus axonaal hersenletsel aan het begin van het experiment. Op het moment 48 uur, Neurologische Ernst Score werd onderzocht. Op het moment 48 uur, immunochemische kleuring van beta-APP werd uitgevoerd op alle twee groepen.Geïllustreerd neurologisch tekort 48 uur na diffuus axonaal hersenletsel voor twee studiegroepen. Een Mann-Whitney test gaf aan dat neurologisch tekort aanzienlijk groter was voor 15 diffuse axonale hersenletsel ratten in vergelijking met 15 naïeve ratten op 48 uur na interventie. Representatieve fotomicrografen die axonale en neuronale immunoreactiviteit onthullen na geïsoleerde diffuse axale hersenletsel bij ratten 48 uur na het letsel.Kleinere cellulaire snaren worden gedetecteerd met bèta-amyloïde precursor eiwit. In sham group, werd het niet gedetecteerd. In ons werk ontwikkelden we een eenvoudig, reproduceerbaar en betrouwbaar nieuw model voor diffuus axonaal hersenletsel op basis van rotatieversnellingsmechanisme.Een dergelijk model zal een beter begrip van pathofysiologie van diffuus axonaal hersenletsel en de ontwikkeling van effectievere behandelingen mogelijk maken.
