Denne protokollen beskriver en gnagermodell av DAI. Traumatisk hjerneskade (TBI) er en viktig årsak til død og funksjonshemning i USA. TBIer bidrar til ca 30% av alle skaderelaterte dødsfall1,2. De viktigste årsakene til TBI varierer blant aldersgrupper og inkluderer fall, høyhastighetskollisjoner under sport, forsettlig selvskading, motorvognkrasj og angrep1,2,3.
Hjernediffus aksonal skade (DAI) er en bestemt type TBI indusert av rotasjonsakselerasjon, risting eller blast skade av hjernen som følge av ubegrenset hodebevegelse i umiddelbar tid etter skade4,5,6,7,8. DAI innebærer omfattende aksonal skade som fører til langvarig nevrologisk svekkelse som er forbundet med dårlig resultat, tyngende helsekostnader og en 33-64% dødelighet1,2,4,5,9,10,11. Til tross for betydelig nyere forskning på patogenesen av DAI, har det ikke vært en konsensus om beste behandlingstilbud11,12,13,14.
I løpet av de siste tiårene har mange eksperimentelle modeller forsøkt å nøyaktig gjenskape ulike aspekter ved DAI11,12,15,16. Disse modellene har imidlertid begrensninger gitt den unike presentasjonen av DAI sammenlignet med andre fokusskader. Disse tidligere modellene forårsaker ikke bare aksonalskade i hvite substansregioner, men resulterer også i fokale hjerneskader. Klinisk er DAI ledsaget av mikroblødninger, noe som kan utgjøre en viktig årsak til skade på hvit substans.
Bare to dyremodeller har vist seg å gjenskape de viktigste kliniske egenskapene til DAI. Gennarelli og kolleger produserte den første laterale hoderotasjonsenheten i 1982, ved hjelp av rotasjonsakselerasjon for ikke-støt for å indusere koma med DAI i en ikke-menneskelig primatmodell15. Denne primatmodellen brukte kontrollert enkeltrotasjon for akselerasjon og retardasjon for å fortrenge hodet gjennom 60° innen 10-20 ms. Denne teknikken var i stand til å etterligne nedsatt bevissthet og utbredt aksonal skade som lignet effekten av alvorlig TBI observert i menneskelige hjerner. Men primat modeller er svært dyrt4,11,16. Basert delvis på den forrige modellen, ble en grismodell av rotasjonsakselerasjon hjerneskade designet i 1994 (Ross et al.) med lignende resultater14.
Disse to dyremodellene, selv om de produserte forskjellige presentasjoner av typisk patologi, har lagt sterkt til begrepene DAI patogenese. Rask hoderotasjon er generelt akseptert som den beste metoden for å indusere DAI, og gnagere gir en billigere modell for de raske hoderotasjonsstudiene11,16. Her validerer vi en enkel, reproduserbar og pålitelig gnagermodell av DAI som forårsaker omfattende skade på hvit materie uten kraniebrudd eller kontusjoner. induction of diffuse axonal brain injury in rats based on rotational acceleration

Traumatisk hjerneskade (TBI) er en viktig årsak til død og funksjonshemning. Diffus aksonal skade (DAI) er den dominerende skademekanismen hos en stor andel TBI-pasienter som krever sykehusinnleggelse. DAI innebærer omfattende aksonalskade fra risting, rotasjon eller blastskade, noe som fører til rask aksonal strekkskade og sekundære aksonale endringer som er forbundet med en langvarig innvirkning på funksjonell gjenoppretting. Historisk har eksperimentelle modeller av DAI uten fokalskade vært vanskelig å designe. Her validerer vi en enkel, reproduserbar og pålitelig gnagermodell av DAI som forårsaker omfattende hvit materieskade uten kraniebrudd eller kontusjoner.

Tabell 1 illustrerer protokolltidslinjen. Dødeligheten i denne modellen av DAI var 0%. En Mann-Whitney-test indikerte at nevrologisk underskudd var signifikant større for de 15 DAI-rottene sammenlignet med de 15 sham-rottene ved 48 timer etter intervensjon (Mdn = 1 vs. 0), U = 22,5, p < 0,001, r = 0,78 (se tabell 2). Dataene måles i tellinger og presenteres som median og 25–75 persentilområde.

Traumatisk hjerneskade er en av de viktigste årsakene til død og funksjonshemning. Diffus aksonal hjerneskade er en utbredt aksonal skade utvikles etter TBI. I denne forskningen vil jeg presentere vår modell om induksjon diffus axonal hjerneskade basert på rotasjonsakselerasjonsmekanisme resultat TBI.Velg voksne hann Sprague-Dawley rotter som veier 300 til 350 gram. Gi rotte chow og vann ad libitum. Utfør alle eksperiment mellom 06:00 og 12:00. Enheten ble plassert på et tungt, stabilt laboratoriebord.Enheten består av følgende komponenter:en sylinder, en jernvekt, en rotasjonsmekanisme bestående av et sylindrisk rør og hodefikseringspinner. Vekten er festet til en streng og forhøyet til høyden på 120 centimeter. Den fritt fallende vekten treffer bolten som aktiverer rotasjonsmekanismen.Rotte lateral hode rotasjon enheten ble brukt til å snu hodet raskt fra null til 90 grader. Etter induksjon, diffus axonal hjerneskade, ble rotte flyttet til gjenopprettingsrommet. F er kraften som påføres dyrets hode i kilo.Capital M er øyeblikket av kraften, K for kinetisk energi. Liten m er massen av den fallende vekten, g for gravitasjonsakselerasjon, h for høyde i centimeter, og D er avstanden mellom ørepinnene i centimeter. For vurdering av nevrologisk underskudd og klassemodellunderskudd, brukte vi nevrologisk alvorlighetsgradscore hos rotter.Vi evaluerer mobilitet, hemiplegi, strålegangsoppgave, svikt i strålevandringsoppgave. Svikt i strålebalanseringsoppgaven bred 1,5 centimeter. Stabilitet og strålebalansering, innsats på strålebalanse 1,5 centimeter bred og reflekser.48 timer etter skade, alle rotter, skade og kontrollgruppe, ble dypt bedøvet og transkarardielt perfundert med 0,9% heparinisert saltvann etterfulgt av 500 milliliter med 4% formaldehyd i 0,1 M fosfatbuffersvann. Etter perfusjon ble halshugging produsert og hjernen ble umiddelbart fjernet, deretter løst i en 4% formaldehydløsning i 48 timer. Hjernen ble deretter blokkert i fem millimeter koronar seksjoner fra olfaktorisk pære ansikt til den visuelle cortex mens lillehjernen og hjernestammene ble dissekert.Etter parafin innebygging, koronar og sagittal seksjoner ble laget av thalamus. Produser immunokjemisk farging av beta-amyloid forløperprotein. De ulike gruppene av rotter til forskjellige tider er vist på ordningen.Diffus aksonal hjerneskade i begynnelsen av forsøket. På tidspunktet 48 timer ble Neurological Severity Score undersøkt. På tidspunktet 48 timer ble immunokjemisk farging av beta-APP utført i alle to grupper.Illustrert nevrologisk underskudd 48 timer etter diffus axonal hjerneskade for to studiegrupper. En Mann-Whitney-test indikerte at nevrologisk underskudd var signifikant større for 15 diffuse axonal hjerneskaderotter sammenlignet med 15 naive rotter ved 48 timer etter intervensjon. Representative fotomikrografer som avslører aksonale og nevronale immunoreaktiviteter etter isolert diffus axonal hjerneskade hos rotter 48 timer etter skade.Mindre cellulære strenger oppdages med beta-amyloid forløperprotein. I sham-gruppen ble det ikke oppdaget. I vårt arbeid utviklet vi en enkel, reproduserbar og pålitelig ny modell for diffus axonal hjerneskade basert på rotasjonsakselerasjonsmekanisme.En slik modell vil muliggjøre en bedre forståelse av patofysiologi av diffus aksonal hjerneskade og utvikling av mer effektive behandlinger.
