DU is een veel voorkomende oorzaak van lagere urinewegsymptomen bij zowel mannen als vrouwen. Het is een complexe constellatie van symptomen met weinig behandelingsopties die de kwaliteit van leven (Qol) van de getroffenen aanzienlijk kunnen verminderen18. Hoewel wordt aangenomen dat DU multifactorieel is, blijft het begrip van de pathogenese rudimentair. Detrusor onderactiviteit (DU) is een typische lagere urinewegdisfunctie die onderbelicht is gebleven. Hoewel DU is gedefinieerd door de International Continence Society (ICS)1, worden tal van verschillende terminologieën gebruikt om naar deze ziekte te verwijzen, bijvoorbeeld "detrusorfalen", "acontractiele blaas", "detrusor areflexie"2. DU, zoals gedefinieerd door de International Continence Society (ICS) in 2002, is een samentrekking van verminderde kracht en duur, wat resulteert in een langdurige toename van de tijd voor blaaslediging, wat resulteert in het niet bereiken van volledige blaaslediging binnen een normale periode.
DU kan voorkomen bij 48% van de mannen en 12% van de vrouwen (in de leeftijd van >70 jaar)3 met symptomen van lagere urinewegen. Het lijkt multifactorieel te zijn en er bestaat geen effectieve behandeling. Er wordt gemeld dat DU alomtegenwoordig is bij patiënten met neurogene blaasdisfunctie, zoals multiple sclerose4, diabetes mellitus5, ziekte van Parkinson6 of cerebrale beroerte7. DU kan ook worden veroorzaakt door iatrogene zenuwbeschadiging, zoals laparoscopische hysterectomie, prostatectomie of andere chirurgische ingrepen in het kleine bekken8. De pathofysiologische veranderingen en beschikbare behandelingen van DU zijn nog steeds verwarrend vanwege het ontbreken van een geschikt diermodel voor studie.
De mictiereflex wordt gecontroleerd door spino-bulbospinale paden die het pontine mictiecentrum, sacrale parasympathische kern en meer senior cortexcentracombineren 9. Activering en onderhoud van de mictiereflex zijn voornamelijk afhankelijk van het regelmatige transport van sensorische signalen van de blaas naar meer senior cortexcentra. Er kan worden gepostuleerd dat sensorische disfunctie bijdraagt aan DU.
De meeste dierproeven met betrekking tot lagere urinewegdisfuncties hebben zich gericht op overactieve blaas (OAB) modellen10. Deze modellen bieden een redelijk begrip van OAB pathofysiologie en prognose. Er zijn echter slechts enkele DU-modellen gemeld, bijvoorbeeld supraspinale schade (lokale laesies, decerebratie en occlusie van de middelste hersenslagader), transsectie van het ruggenmerg of kneuzingsletsel, systemisch (bijv. cyclofosfamide) of intravesicale toediening van irriterende of inflammatoire middelen (bijv. zuur, acroleïne en lipopolysaccharide)11,12,13,14 . Van deze methoden kan alleen de methode voor ruggenmergtranssectie of kneuzingsletsel worden gebruikt bij het vaststellen van een diermodel van DU13. Pogingen met betrekking tot de verwonding van het pontine mictiecentrum en de hogere cortexcentra werden opgegeven vanwege het ernstige trauma. Er wordt dus meer aandacht besteed aan het vinden van een nauwkeurige locatie in het mictiereflexcentrum om de DU te induceren met minimale bijwerkingen.
Zoals eerder vermeld, is een van de mechanismen van het induceren van DU het verwonden van het ruggenmerg om de signaalroute van de mictiereflex te beschadigen. Allen's weight-drop methode werd ontwikkeld om proefdieren met gewonde ruggenmerg vast te stellen15. Er zijn echter geen verdere experimentele gegevens beschikbaar over deze methode. Bovendien, aangezien delen van de dieren de spinale functie herstelden na een beroerte zonder DU, kan het niet worden beschouwd als een perfecte methode voor het genereren van een DU-diermodel16.
In 1987 excogiteerde Bregman een proces van transectatie van het ruggenmerg voor het genereren van het DU-diermodel en verwierf experimentele gegevens17. Toch werd deze methode niet toegepast om het DU-diermodel vast te stellen. Op dat moment waren onderzoekers nog steeds in de war over de pathogenese van DU. Omdat locaties in het ruggenmerg geassocieerd met de inductie van OAB of DU naast elkaar liggen, waren ze niet in staat om de nauwkeurige plaats van schade aan het ruggenmerg te vinden om DU17 te induceren. OAB en DU werden samen of afzonderlijk geïntroduceerd door deze methode. Dus hoewel deze methode DU introduceerde, was deze onnauwkeurig en kon deze niet worden gebruikt voor het begrijpen van het voorkomen en verwerken van DU.
Zoals hierboven vermeld, is het ontbreken van een geschikt diermodel van DU een van de belangrijkste obstakels voor de studie van DU. Onderzoekers zijn voortdurend op zoek naar een nauwkeurig en beheersbaar model dat de pathologie van DU kan simuleren. Zelfs de behandelingsopties voor DU zijn de afgelopen 20 jaar niet significant verbeterd. Collectief is er een grote behoefte om een standaardprotocol te beschrijven voor het vaststellen van een diermodel van DU.
Daarom beschrijven we in dit artikel een methode om met succes een rattenmodel van DU vast te stellen door conus medullris-transsectie. Transsectie werd uitgevoerd op L4\u2012L5 niveau om de conus medullaris te scheiden. De maximale cystometrische capaciteit (MCC), detrusoropeningsdruk (DOP) en naleving van de blaas werden geregistreerd en geanalyseerd om het protocol te valideren. Het onderstaande protocol combineert zowel haalbaarheid als betrouwbaarheid op een gestandaardiseerde manier om het DU-diermodel vast te stellen, waarbij het voorkomen en de verwerking van DU wordt gesimuleerd. Het protocol kan worden gebruikt als een techniek voor verdere studie van DU.

Het doel van het gepresenteerde protocol was om een detrusor underactiviteit (DU) model in de rat vast te stellen door middel van conus medullris transection. Laminectomie werd uitgevoerd bij in totaal 40 vrouwelijke Wistar-ratten (controlegroep: 10 ratten; testgroep: 30 ratten) met een gewicht van 200-220 g, en de conus medullaris werd getransecteerd op het L4\u2012L5-niveau in de testgroep. Alle ratten werden gedurende zes weken gehuisvest en gevoed onder dezelfde omgevingsomstandigheden. In de testgroep werd gedurende zes weken tweemaal daags urineleeging uitgevoerd en werd het gemiddelde resterende urinevolume geregistreerd. In beide groepen werd een cystometrogram uitgevoerd. Maximale cystometrische capaciteit (MCC), detrusoropeningsdruk (DOP) en naleving van de blaas werden geregistreerd en berekend. De testgroep vertoonde een significante urineretentie na de operatie, zowel tijdens als na de spinale shock. Er werd echter geen afwijking waargenomen in de controlegroep. In vergelijking met de controlegroep was de MCC en de therapietrouw van de blaas in de testgroep significant hoger dan die van de testgroep (3,24 ± 2,261 ml versus 1,04 ± 0,571 ml; 0,43 ± 0,578 ml/cmH2O versus 0,032 ± 0,016 ml/cmH2O), terwijl DOP in de testgroep lager was dan de controlegroep (20,28 ± 14,022 cmH2O versus 35 ± 13,258 cmH2 O). Deze methode om een diermodel van DU vast te stellen door de conus medullaris transsectie biedt een uitstekende gelegenheid om de pathofysiologie van DU op een betere manier te begrijpen.

De gehele procedure van de conus medullaris transsectie kan binnen 45 minuten worden voltooid door ervaren chirurgen. Ons laboratorium heeft meer dan 100 gevallen van conus medullaris transsectie operaties uitgevoerd. Het slagingspercentage is meer dan 95%, zoals gedefinieerd door de overleving van de ratten en de succesvolle inductie van DU. De urodynamische test bevestigde de inductie van DU.

And carefully resect the muscles attached to the spine to expose the vertebral column. Resect the supraspinous, and interspinous ligaments.And use surgical scissors and forceps to expose the spine at L4 to L5.Use Kelly forceps to carefully remove the L4 to L5 vertebral spinous process and parts of the transverse process to expose the spinal cord. And completely expose the conus medullaris at L4 to L5.Use iridectomy scissors to completely transect the conus medullaris. And insert tissue packing to block the recovery of the spinal cord.Then use a 4-0 non-absorbable suture to close the overlying muscle and skin. And place the rat in a 37 degree Celsius incubator for the first hour post-procedure, with monitoring until the animal is sternal or actively moving. Urine retention is observed immediately after surgery, with the peak point of volume appearing on the second day post-operation, and gradually decreasing for about 10 days.Ten days after surgery, the retention volume reaches a steady level. Urodynamic testing reveals that the maximum cytometric capacity and bladder compliance are significantly higher in the test group, while the detrusor opening pressure in the test group decreases significantly compared to the control group. Correctly identifying the 13th rib is a key step for successful identification of the L4 to L5 vertebra.
