Gezien de toenemende belangstelling voor NO-gastherapie voor niet-geïntubeerde patiënten, inclusief patiënten met COVID-198,beschrijft dit rapport een nieuw aangepast apparaat en hoe de componenten ervan kunnen worden geassembleerd om NO te leveren bij concentraties tot 250 ppm. In verschillende klinische omgevingen 1 ,2,3wordt regelmatig geen inhalatietherapie gebruikt als levensreddende behandeling . Naast het bekende pulmonale vaatverwijdende effect4vertoont NO een breed antimicrobieel effect tegen bacteriën5,virussen6en schimmels7, vooral als het in hoge concentraties wordt toegediend (>100 ppm). Tijdens de uitbraak van het Severe Acute Respiratory Syndrome (SARS) in 2003 vertoonde NO krachtige antivirale activiteit in vitro en toonde het therapeutische werkzaamheid aan bij patiënten die besmet waren met het SARS-Coronavirus (SARS-CoV)9,10. De stam van 2003 is structureel vergelijkbaar met SARS-Cov-2, de ziekteverwekker die verantwoordelijk is voor de huidige coronavirusziekte-2019 (COVID-19) pandemie11. Er lopen drie gerandomiseerde gecontroleerde klinische onderzoeken bij patiënten met COVID-19 om de potentiële voordelen van het inademen van no-gas met hoge concentratie te bepalen om de resultaten12,13,14te verbeteren . In een vierde lopende studie wordt de profylactische inhalatie van hoge concentraties NO onderzocht als preventieve maatregel tegen de ontwikkeling van COVID-19 bij zorgverleners die zijn blootgesteld aan SARS-CoV-2-positieve patiënten15.
De ontwikkeling van een effectieve en veilige behandeling van COVID-19 is een prioriteit voor de gezondheidszorg en wetenschappelijke gemeenschappen. Om de toediening van NO-gas bij doses > 80 ppm bij niet-geïntubeerde patiënten en vrijwillige gezondheidswerkers te onderzoeken, werd de noodzaak om een veilig en betrouwbaar niet-invasief systeem te ontwikkelen duidelijk. Deze techniek is bedoeld om hoge NO-concentraties toe te dienen bij verschillende fracties van geïnspireerde zuurstof (FiO2) aan spontaan ademende proefpersonen. De hier beschreven methodologie wordt momenteel gebruikt voor onderzoeksdoeleinden bij spontaan ademende COVID-19-patiënten in het Massachusetts General Hospital (MGH)16,17. Volgens de richtlijnen van de ethische commissie voor menselijk onderzoek van MGH wordt het voorgestelde systeem momenteel gebruikt om een reeks gerandomiseerde gecontroleerde proeven uit te voeren om de volgende effecten van hoge concentraties NO-gas te bestuderen. Ten eerste wordt het effect van 160 ppm NO-gas onderzocht bij niet-geïntubeerde personen met licht-matige COVID-19, toegelaten op de spoedeisende hulp (IRB-protocol #2020P001036)14 of als intramurale (IRB-protocol #2020P000786)18. Ten tweede wordt de rol van hoge dosis NO onderzocht om SARS-CoV-2-infectie en de ontwikkeling van COVID-19-symptomen bij zorgverleners die routinematig worden blootgesteld aan SARS-CoV-2-positieve patiënten (IRB-protocol nr. 2020P000831)19te voorkomen.
Dit eenvoudige apparaat kan worden geassembleerd met standaard verbruiksartikelen die routinematig worden gebruikt voor ademhalingstherapie. Het voorgestelde apparaat is ontworpen om niet-invasief een mengsel van GEEN gas, medische lucht en zuurstof (O2) af te leveren. Inademing van stikstofdioxide (NO2)wordt geminimaliseerd om het risico op toxiciteit van de luchtwegen te verminderen. De huidige no2 veiligheidsdrempel die door de Amerikaanse Conferentie van Overheids industriële hygiënisten wordt vastgesteld is 3 ppm over een tijdgewogen gemiddelde van 8 uur, en 5 ppm is de blootstellingslimiet op korte termijn. Omgekeerd beveelt het Nationaal Instituut voor veiligheid en gezondheid op het werk 1 ppm aan als kortdurende blootstellingslimiet20. Gezien de toenemende belangstelling voor hoge dosis NO-gastherapie, geeft dit rapport de nodige beschrijving van dit nieuwe apparaat. Het legt uit hoe de componenten te monteren om een hoge concentratie van NEE te leveren voor onderzoeksdoeleinden.

a novel inhalation mask system to deliver high concentrations of nitric oxide gas in spontaneously breathing subjects

Stikstofmonoxide (NO) wordt toegediend als gas voor inademing om selectieve long vasodilatatie te induceren. Het is een veilige therapie, met weinig potentiële risico's, zelfs als het in hoge concentratie wordt toegediend. Ingeademd NO-gas wordt routinematig gebruikt om systemische oxygenatie in verschillende ziekteomstandigheden te verhogen. De toediening van hoge concentraties NO oefent ook in vitro een virucidaal effect uit. Vanwege de gunstige farmacodynamische en veiligheidsprofielen, de bekendheid met het gebruik ervan door kritische zorgverleners en het potentieel voor een direct virucidaal effect, wordt NO klinisch gebruikt bij patiënten met coronavirusziekte-2019 (COVID-19). Toch is er momenteel geen apparaat beschikbaar om ingeademde NO gemakkelijk toe te dienen bij concentraties hoger dan 80 delen per miljoen (ppm) bij verschillende geïnspireerde zuurstoffracties, zonder dat er speciale, zware en dure apparatuur nodig is. De ontwikkeling van een betrouwbare, veilige, goedkope, lichtgewicht en ventilatorvrije oplossing is cruciaal, met name voor de vroege behandeling van niet-geïntubeerde patiënten buiten de intensive care (IC) en in een scenario met beperkte middelen. Om een dergelijke barrière te overwinnen, werd een eenvoudig systeem voor de niet-invasieve NO-gastoediening tot 250 ppm ontwikkeld met behulp van standaard verbruiksartikelen en een opruimkamer. De methode is veilig en betrouwbaar gebleken bij het leveren van een gespecificeerde NO-concentratie en het beperken van stikstofdioxideniveaus. Dit artikel is bedoeld om clinici en onderzoekers te voorzien van de nodige informatie over hoe een dergelijk systeem te assembleren of aan te passen voor onderzoeksdoeleinden of klinisch gebruik bij COVID-19 of andere ziekten waarbij GEEN toediening gunstig kan zijn.

Ingeademd stikstofmonoxide kan de oxygenatie verbeteren bij patiënten met ernstige hypoxemie. Bij hoge concentraties vertoont stikstofmonoxide brede antimicrobiële effecten. Verschillende onderzoeken testen het effect van stikstofmonoxide op COVID-19.Tot op heden is er geen apparaat beschikbaar dat in staat is om ingeademd stikstofmonoxide toe te dienen bij concentraties hoger dan 80 delen per miljoen zonder dat er speciale zware en dure apparatuur nodig is. Met dit apparaat kan hooggedoseerd ingeademd stikstofmonoxide buiten de ziekenhuisinstelling worden afgeleverd. Patiënten met longkolonisatie als gevolg van multiresistente bacteriën, zoals cystische fibrosepatiënten, kunnen baat hebben bij deze behandeling.De demonstranten zijn Stefano Gianni en Bijan Safaee Fakhr, twee postdoc-onderzoekers van mijn laboratorium. Om de patiëntinterface in te stellen, sluit u de ingebouwde elleboogpoort van een nauwsluitend standaard niet-invasief ventilatiemasker van de juiste grootte voor het onderwerp aan op een hoog rendement deeltjesluchtfilter via de 22 millimeter buitendiameter 15 millimeter binnendiameter connector. Om de opruimkamer voor te bereiden, sluit u een 22 bij 22 millimeter flexibele siliconenrubberconnectoradapter aan op het proximale uiteinde van een aaseterkamer met 100 gram calciumhydroxide en sluit u nog een flexibele siliconenrubberconnectoradapter van 22 bij 22 millimeter aan op het distale uiteinde van de aaseter en gebruikt u standaard knikbestendige vinylgasslangen met universele adapters aan beide uiteinden om de zuurstofbron op de inspiratoire ledemaat aan te sluiten.Om het stikstofmonoxidereservoirsysteem voor te bereiden, sluit u een aerosol T-stuk aan in een 90 graden ventilatorelleboogconnector zonder poorten en sluit u het andere uiteinde van de elleboog aan op een latexvrije ademhalingsreservoirzak van drie liter. Bevestig nog een eenrichtings inspiratoire klep met de pijl die naar boven wijst aan het distale uiteinde van twee opeenvolgende 15 millimeter buitendiameter door 22 millimeter buitendiameter 15 millimeter binnendiameter connectoren met 7,6 millimeter bemonsteringspoorten en flip top caps en sluit een 15 tot 22 millimeter tweestapsadapter aan op het proximale uiteinde van het systeem en sluit de proximale tweestapsadapter aan op de resterende vrije inlaat van het groene T-stuk van het stikstofmonoxide reservoirsysteem. Om de lucht- en stikstofmonoxidegasstroomleidingen te bevestigen, gebruikt u standaard knikbestendige Star Lumen vinyl zuurstofgasslangen om de medische luchtstroom aan te sluiten op de meest distale gasinlaatpoort en de stikstofmonoxidegasstroom van een 800 onderdelen per miljoen medische stikstofmonoxidetanks stroomafwaarts naar de volgende poort stroomafwaarts aan te sluiten.Om stikstofmonoxide toe te dienen aan een spontaan ademend subject, stelt u de luchtstroom, zuurstof en stikstofmonoxidegasstroom in volgens de tabel en plaatst u het nauwsluitende masker op het gezicht van het onderwerp, vergelijkbaar met een niet-invasieve ventilatie-interface-opstelling. Start vervolgens de inhalatiesessie voor de gewenste duur. De stikstofmonoxide- en stikstofdioxideconcentraties kunnen worden getraceerd om de stabiliteit van de gastoevoer tijdens de behandeling te beoordelen.Een 33-jarige ademhalingstherapeut die op de IC van het Mass General Hospital werkte tijdens de piek van de opname op de IC voor COVID-19 bood zich aan om stikstofmonoxide te ontvangen als onderdeel van de studie waarbij gezondheidswerkers betrokken waren. De resulterende stikstofmonoxideconcentratie was 160 delen per miljoen bij een totaal gasdebiet van 19,5 liter per minuut, gemeten door drie standaard 15 liter per minuut debietmeters en bleef stabiel gedurende de hele periode van inademing. Stikstofdioxide bereikte een piek van 0,77 delen per miljoen en lag daarom veilig onder de aanbevolen toxiciteitsdrempel.De mogelijkheid om veilig hoogconcentratie ingeademd stikstofmonoxide te leveren, opent het veld voor de toepassing van deze therapie op andere ziekten waarbij stikstofmonoxide een effectieve behandeling kan zijn.
