Kardiyovasküler doku mühendisliği (TE),1,2,3,4hastalarının büyük kohortları için yetersiz olan şu anda kullanılan kalıcı kardiyovasküler protezlere (örneğin, vasküler greftler, kalp kapak değiştirmeleri) alternatif bir tedavi seçeneği olarak takip edilmektedir. Çok aranan uygulamalar arasında doku mühendisliği yapılmış vasküler greftler (TEVG'ler) 5,6 ve kalp kapakçıkları (TEHV' ler)7,8bulunur. Çoğu zaman, kardiyovasküler TE metodolojileri, oluşacak yeni doku için öğretici bir iskele görevi gören yeniden sıralanabilir biyomalzemelerden (doğal veya sentetik) yararlanır. Yeni doku oluşumu tamamen in vitro olarak mühendislik yapılabilir, implantasyondan önce bir biyoreaktörde (in vitro TE) 9,10,11veya doğrudan in situ, sentetik iskelenin doğrudan vücutta yeni doku oluşumunu teşvik etmek için ön kültleme yapılmadan implante edildiği hücreli ve kültlü iskeleyi tohumlayarak (yerinde TE)12,13,14. Hem in vitro hem de in situ kardiyovasküler TE yaklaşımları için, başarılı fonksiyonel rejenerasyon baskın olarak hem implante yapıya konak immün yanıtına hem de uygun biyomekanik yüklemeye bağlıdır.
Kardiyovasküler TE için biyomekanik yüklemenin önemi iyi kabul edilir15. Kardiyovasküler implantlarda, iskeleyi dolduran hücreler hemodinamik ortamın bir sonucu olarak ortaya çıkan döngüsel esneme ve kesme streslerine maruz kalır. Çok sayıda çalışma, (döngüsel) esnemenin kollajen16 , 17 , 18,19,glikozaminoglisekanlar (GAG) 20 ve elastin21,22gibi matris bileşenlerinin oluşumu üzerindeki uyarıcı etkisini çeşitli hücre tiplerine göre bildirmektedir. Örneğin, Huang ve ark. biaksiyel streç bir vasküler biyoreaktör kullanarak in vitro TEVGs kollajen ve elastin birikimini ve organizasyonununu yükselttini göstermiştir23. Vurgu tipik olarak baskın yük olarak esnemeye dayansa da, bu çalışmalar genellikle numunenin kesme akışına da maruz kaldığı akış tahrikli biyoreaktörlerden yararlanır. Kesme gerilmelerinin 3D doku oluşumu ve iltihaplanma üzerindeki izole etkisi hakkında nispeten az şey bilinmesine rağmen, bazı veriler mevcuttur. Örneğin, Hinderer ve ark. ve Eoh ve arkadaşları, 3D iskele mikro yapısına ek olarak, in vitro model sistemde insan damar düz kas hücreleri tarafından olgun elastin oluşumu için önemli olduğunu göstermiştir24,25. Bu bulgular, kardiyovasküler TE için hem döngüsel esneme hem de kesme stresinin alaka düzeyini göstermektedir.
TE implantlarının başarısı veya başarısızlığı için bir diğer önemli belirleyici, konağın implante grefte immün yanıtıdır26. Bu, hücresel akın ve endojen doku oluşumu ve yeniden şekillendirme27sonraki süreçlerini başlatmak için iskeleye akut enflamatuar yanıta dayanan malzeme odaklı in situ TE stratejileri için özellikle önemlidir. Makrofaj,28,29,30gibi birden fazla çalışma ile gösterilen fonksiyonel doku yenilenmesinin kritik bir başlatıcısıdır. Yara iyileşmesine benzer şekilde, dokunun yenilenmesi makrofajlar ve fibroblastlar ve miyofibroblastlar31, 32,33 gibi doku üreten hücreler arasında parakrin sinyal ile yönetilir. Yeni doku birikimini koordine ederek, makrofajlar yabancı iskele malzemesi34,35'inaktif resorpsiyonunda yer almaktadır. Bu nedenle, bir biyomalzeme in vitro makrofaj yanıtı, implantların in vivo başarısı için tahmine dayalı bir parametre olarak tanımlanmıştır36,37,38.
İmplante edilmiş bir iskeleye makrofaj yanıtı, malzeme bileşimi ve mikroyapı35, 39,40gibi iskele tasarım özelliklerine bağlıdır. İskele özelliklerine ek olarak, bir iskeleye makrofaj tepkisi ve myofibroblasts ile çapraz konuşmaları da hemodinamik yüklerden etkilenmiştir. Örneğin, döngüsel streç makrofaj fenotip41 , 42,43,44ve sitokinlerinsalgılanması önemli bir modülatör olarak gösterilmiştir43,44,45,46 3D elektrospun iskelelerde. Makrofajlar ve vasküler düz kas hücrelerinden oluşan bir ortak kültür sistemi kullanan Battiston ve arkadaşları, makrofajların varlığının elastin ve GAG seviyelerinin artmasına yol açtığını ve orta derecede döngüsel esneme seviyelerinin (1.07-1.10) kollajen I ve elastin47'ninbirikmesini uyardığını göstermiştir. Önceki çalışmalarda, kesme stresinin 3D elektrospun iskeleleri48,49'a monosit alımı için önemli bir belirleyici olduğunu ve hem kesme stresinin hem de döngüsel esnemenin insan monositleri ve mezenkimal stromal hücreler arasındaki parakrin sinyali etkilediğini gösterdik50. Fahy ve arkadaşları, kesme akışının insan monositleri tarafından pro-enflamatuar sitokinlerin salgısını artırdığını göstermiştir51.
Birlikte ele alındığında, yukarıdaki kanıtlar hemodinamik yükler üzerinde yeterli bir anlayış ve kontrolün kardiyovasküler TE için çok önemli olduğunu ve bunu başarmak için enflamatuar yanıtın göz önünde bulundurulmasının önemli olduğunu göstermektedir. Daha önce in vitro52 , 53 , 54 ,55,56,57,58veya ex vivo59,60 ,61kardiyovasküler doku kültürü için çok sayıda biyoreaktör tanımlanmıştır. Bununla birlikte, tüm bu sistemler fizyolojik hemodinamik yükleme koşullarını mümkün olduğunca taklit etmek için tasarlanmıştır. Bu, kardiyovasküler dokuların in vitro olarak oluşturulması veya ex vivo kültürlerinin korunması amacıyla son derece değerli olsa da, bu tür sistemler bireysel ipuçlarının bireysel etkilerine sistematik çalışmalara izin vermez. Bunun nedeni, bu biyoreaktörlerde hem döngüsel esneme hem de kesme stresinin uygulanmasının, özünde onları bağlayan aynı basınçlı akış tarafından yönlendirilmesidir. Doğru çok işaretli mekanik manipülasyona izin veren mikrosistemler 2D substratlar62 veya 3D hidrojel kurulumları63,64için açıklanmış olsada,bu tür kurulumlar elastomerik 3D biyomalzeme iskelelerinin birleştirilmesine izin vermez.
Burada, kesme stresi ve döngüsel esnemenin ayrışmasını benzersiz bir şekilde sağlayan ve bireysel ve kombine etkilerini mekanistik olarak araştırmaya yardımcı olan bir tübüler biyoreaktör sisteminin uygulanmasını sunuyoruz. Bu sistem, çok çeşitli doku mühendisliği vasküler greftlerin (örneğin sentetik veya doğal kökenli, farklı mikro mimari, çeşitli gözeneklilikler) testine izin verir. Kesme gerilme ve germe uygulamasını etkili bir şekilde ayrıştırmak için, biyoreaktörün kullandığı temel kavramlar (1) ayrı pompa sistemleri kullanılarak kesme stresi ve esneme kontrolünün ayrılması ve (2) iskelelerin hesaplama odaklı boyutlara sahip 'içten dışa' bir şekilde uyarılmasıdır. Akış, borulu iskelenin dış yüzeyine bir akış pompası kullanılarak uygulanırken, iskelenin çevresel gerilmesi, iskelenin ayrı bir gerinim pompası kullanılarak monte edildiği silikon bir tüp genişletilerek indüklenmiştir. Silikon tüpün boyutları ve yapıyı içeren cam tüp, iskeledeki kesme stresinin (akış nedeniyle) ve çevresel germenin (tüp genişlemesi nedeniyle) birbirini önemli ölçüde etkilememesini sağlamak için hesaplamalı akışkan dinamiği simülasyonları kullanılarak özenle seçilir ve doğrulanır. Bu içten dışa tasarımın birkaç pratik mantığı vardır. Streç, ışık sıvısı basıncı (fizyolojik yüklemeye benzer) tarafından uygulanırsa, doğal olarak numune tasarımının sızdırmaz olmasını gerektirir. Ek olarak, numuneyi germek için gereken basınç, numuneler arasında ve zaman içinde bir numune içinde değişebilen numune sertliği tarafından tamamen belirlenir ve bu da esnemeyi kontrol etmeyi zorlaştırır. Bu biyoreaktör, doku mühendisliği yapılan grefti silikon bir tüpün etrafına monte eder ve greftin dış duvarında duvar kesme stresi (WSS) uygulamasına izin verir ve grefti içeriden basınçlandırır. Bu şekilde, numuneler arasında ve zaman içinde numuneler içinde eşit yükleme koşulları sağlanabilir ve dahası, gözenekli vasküler iskelelerde yaygın olduğu gibi numunelerin sızdırılmasına izin verilir19. Bu içten dışa biyoreaktör, geleneksel vasküler biyoreaktör kurulumlarının daha uygun olduğu doğal benzeri bir kan damarı in vitro mühendisliği yerine, kesme ve/ veya esnemenin etkileri üzerine sistematik çalışmalar için özel olarak tasarlanmıştır. Biyoreaktör kullanımı, grubumuz tarafından, yerinde kardiyovasküler doku için resorbable elektrospun iskelelerinde kesme stresi ve döngüsel streçlerin iltihaplanma ve doku oluşumu üzerindeki bireysel ve kombine etkilerini araştırdığımız bir dizi yeni çalışmaya dayanarak gösterilmiştir19,43,44. Bu amaçla, in situ rejeneratif çağlayanın çeşitli aşamalarını simüle etmek için mono veya ortak kültürde insan makrofajları ve miyofibroblastları kullandık. İnsan makrofajları tarafından sitokin salgılamanın hem döngüsel streç hem de kesme stresinden belirgin bir şekilde etkilendiğini, bu iskelelerdeki insan miofibroblastları tarafından matris birikimini ve organizasyonunu etkilediğini, hem parakrin sinyalizasyon hem de doğrudan temas yoluyla19,43,44. Özellikle, bu çalışmalar, kesme stresi ve esnemenin kombine uygulanması durumunda, doku oluşumu ve iltihabı üzerindeki etkilerin ya iki yüklerden birinin hakim olduğunu ya da her iki yükün sinerjik etkilerinin olduğunu ortaya koydu. Bu bulgular, mekanik ortamın TE proseslerine olan katkısını daha iyi anlamak için her iki yükün ayrıştırmasının alaka düzeyini göstermektedir. Bu anlayış, ilgili hemodinamik yükleme rejimlerinde iskele tasarım parametrelerini sistematik olarak optimize etmek için uygulanabilir. Buna ek olarak, bu tür iyi kontrol edilen ortamlardan elde edilen mekanistik veriler, yakın zamanda TEVG 65 veya TEHV66 için bildirildiği gibi, yerinde doku tadilatının seyrini tahmin etmek için geliştirilen sayısal modeller için girdi görevi görebilirsiniz.

Resorbe edilebilir biyomalzemelerin doğrudan vücutta yenilenmeyi teşvik etmek için kullanılması, çevirisel açıdan çekici bir stratejidir. A Multi-Cue Bioreactor to Evaluate the Inflammatory and Regenerative Capacity of Biomaterials under Flow and Stretch

So far, it has been really difficult to identify the cause and effect relationships between hemodynamics and vascular tissue regeneration. And this has because it's challenging to control the individual mechanical loads. This bioreactor enables us to mechanistically investigate the individual and combined effects of sheer stress and cyclic stretch on the regenerative potential of a broad variety of tissue engineered vascular grafts.Demonstrating this procedure will be Suzanne Koch, a PhD candidate in our group. And Dr.Tamar Wissing who is a postdoctoral researcher from our laboratory. To mount the electro spun scaffold onto the silicone tubing, thread a 4-0 prolene suture into one end of a piece of silicone tubing and out of the other.Make a small knot on both sides of the tube leaving approximately 10 centimeters of wire on both knots and make a third knot on top of the two. After removing a suture needle, trim the edges of the silicone tubing into a triangular shape and make a final knot at the end of the two 10 centimeter wires. Dip the electro spun scaffold in 30%ethanol and place the scaffold over one end of free suture wire.Next, gently stretch both sides of the silicone tubing and the 10 centimeter suture knot while second researcher uses tweezers with a smooth inner tip to gently slide the electro spun scaffold over the tubing. Slowly release the stretch on the silicone tubing while simultaneously smoothing the electro spun scaffold with the tweezers. And dip the scaffold and silicone tubing in ultra pure water two times.Construct the bottom compartment and make sure that the O-ring is properly placed. Place an adaptor bushing in the upper part of the bottom compartment and place the pressure conduit with holes through the bottom compartment. Lace the silicone O-ring around the lower end of the pressure conduit to prevent leakage.And screw the lower part of the bottom compartment to the upper part of the bottom compartment to secure the pressure conduit. Make sure that the lower engraved groove of the pressure conduit is approximately three to five millimeters above the edge of the adapter bushing of the bottom compartment. Pull the silicone tube with the electro spun scaffold over the pressure conduit and make a nod and the suture wire at the lower end of the electro spun scaffold, at the location of the engraved groove on the pressure conduit.Make a second nod at the opposite side to tightly secure the silicone tubing with the electro spun graft. And placing a scissor clamp equipped with a rule at the upper end of the silicone tubing, pull the scissor clamp upwards in a consistent manner and gently pull on the electric spawn scaffold to remove any wrinkles. Use a suture wire to make two knots at both ends of the scaffold at the upper engraved groove of the pressure conduit.When both knots have been made, release the scissor clamp and use a knife to remove any excess silicone tubing. Screw the nose cones on the screw thread of the pressure conduits for the samples that will be dynamically loaded. Dip the pressure conduit, tubing, and scaffold one time in 30%ethanol and two times in ultrapure water to pre-wet the setup.Place the glass tube over the pressure conduit and push gently on the bottom compartment to secure the glass tube in place. Then place the flow straightener, a silicone O-ring, and adapter bushing in the top compartment. And secure the compartment over the open end of the glass tube.Remove the male lure plug from the flow outlet, work in a sterile laminar flow cabinet. Open the white lure cap and place an ethanol soak tissue in front of the flow outlet. Deconstruct the flow culture chamber by taking off the glass tube with the top compartment.Place a vacuum pasture pipe add onto the electro spun scaffold to remove as much medium as possible. And add fibrinogen solution at a one-to-one ratio with the thrombin supplemented cell suspension. Pipette the solution up and down one time and immediately drip the solution over the full length of the scaffold.When all of the cells have been delivered, slowly move the scaffold from left to right and up and down to achieve an even distribution of the cells. When both sides of the scaffold have been seeded, carefully reconstruct the flow culture chamber by putting the glass tube and top compartment back. And place the flow culture chamber into the incubator to let the fiber solidify.To couple the bioreactor to the pump system, place the flow culture chamber on one of the eight screw threads on the bioreactor base. And place a host clip on the medium tubing. Remove the white lure cap, covering the flow inlet of the top compartment of the flow culture chamber and remove the female lure coupler of the medium tubing.Connect the medium tubing with one side of the flow inlet on the top compartment. And the other side with the flow outlet at the bottom compartment. Transfer the complete setup from the laminar flow cabinet to the incubator.And connect a fluidic units and the bioreactor base to the air pressure tubing and electric cable. Start the software and initialize the pumps. Start the medium flow for the samples one by one.Then change the strain pump parameters to the desired settings and start the strain. Monitor the stretch applied to the scaffold every other day. The monitoring of stretch and wall shear stress over longterm culture periods shows that these values can be maintained at relatively constant levels over a period of up to 20 days.After three days of hemodynamic loading, immunofluorescent staining reveals a homogenous distribution of monocyte derived macrophages and myofibroblasts throughout the scaffold. After 20 days of co-culture, cyclic stretch results in the deposition of more numerous and thicker collagen type 1 fibers. While in the combined hemodynamic load group, the cyclic stretch effect is overruled by sheer stress resulting in a less pronounced collagen type 1 deposition.After eight days of macrophage monoculture, fiber erosion and fiber cleavage are observed in all of the hemodynamic loading regimes with the most pronounced resorption observed in the static group, and the least pronounced resorption observed in the sheer stress group. Both cyclic stretch and a sheer stress impact the cytokine secretion profile of the co-culture setup. Interestingly, the combined effects of both loads show either dominance of one of the two loads or synergistic effects of both loads.Co-culture experiments also show that the mechanical environment and resulting loading dependent inflammatory environments modulate the phenotype of the myofibroblasts. Furthermore, the gene expression patterns of contractile marker alpha smooth muscle actin correlate with protein synthesis. The most important thing when performing this protocol is that's what the application of stretch, it's essential that your setup is leak-free.
