Hier beschrijven we benaderingen voor genbewerking CAR-T-cellen met behulp van CRISPR Cas9-technologie en produceren we producten om verder te testen op functie en werkzaamheid. Het bovenstaande protocol is geoptimaliseerd voor het uitvoeren van CRIPSR-genbewerking in primaire menselijke T-cellen in combinatie met technische T-cellen met chimere antigeenreceptoren. Chimere antigeenreceptor (CAR)-T celtherapie heeft een revolutie teweeggebracht op het gebied van adoptieve celtherapieën en kankerimmunotherapie. CAR-T-cellen zijn gemanipuleerde T-cellen die een synthetische immuunreceptor tot expressie brengen die een antigeenspecifiek antilichaamfragment met één keten combineert met signaaldomeinen afgeleid van de TCRzeta-keten en kostenimulatorische domeinen die nodig en voldoende zijn voor T-celactivering en co-stimulatie1,2,3,4 . De productie van CAR-T-cellen begint met het extraheren van de eigen T-cellen van de patiënt, gevolgd door ex vivo virale transductie van de CAR-module en uitbreiding van het CAR-T-celproduct met magnetische kralen die functioneren als kunstmatige antigeen presenterende cellen5. Geëxpandeerde CAR-T-cellen worden opnieuw in de patiënt geïnjecteerd, waar ze kunnen enten, doeltumorcellen kunnen elimineren en zelfs meerdere jaren na infusie kunnen aanhouden6,7,8. Hoewel CAR-T-celtherapie heeft geresulteerd in opmerkelijke responspercentages bij B-cel maligniteiten, is klinisch succes voor solide tumoren uitgedaagd door meerdere factoren, waaronder slechte T-celinfiltratie9, een immunosuppressieve tumor micro-omgeving10, antigeen dekking en specificiteit, en CAR-T cel disfunctie11,12 . Een andere beperking van de huidige CAR-T-celtherapie omvat het gebruik van autologe T-cellen. Na meerdere rondes chemotherapie en hoge tumorbelasting kunnen CAR-T-cellen van slechte kwaliteit zijn in vergelijking met allogene CAR-T-producten van gezonde donoren, naast de tijd en kosten die gepaard gaan met de productie van autologe CAR-T-cellen. Genbewerking van het CAR-T-celproduct door CRISPR/Cas9 vertegenwoordigt een nieuwe strategie om de huidige beperkingen van CAR-T-cellen te overwinnen13,14,15,16,17.
CRISPR/Cas9 is een tweecomponentensysteem dat kan worden gebruikt voor gerichte genoombewerking in zoogdiercellen18,19. De CRISPR-geassocieerde endonuclease Cas9 kan plaatsspecifieke dubbelstrengsbreuken induceren geleid door kleine RNA's door base-pairing met de doel-DNA-sequentie20. Bij afwezigheid van een reparatiesjabloon worden dubbelstrengsbreuken gerepareerd door de foutgevoelige niet-homologe eindvoeging (NHEJ) -route, resulterend in frameshiftmutaties of voortijdige stopcodons door insertie- en deletiemutaties (INDELs)19,20,21. Efficiëntie, gebruiksgemak, kosteneffectiviteit en de mogelijkheid voor multiplex genoombewerking maken CRISPR/Cas9 tot een krachtig hulpmiddel om de werkzaamheid en veiligheid van autologe en allogene CAR-T-cellen te verbeteren. Deze aanpak kan ook worden gebruikt om TCR-gerichte T-cellen te bewerken door de CAR-constructie te vervangen door een TCR. Bovendien kunnen allogene CAR-T-cellen met een beperkt potentieel om graft versus host-ziekte te veroorzaken, ook worden gegenereerd door genbewerking van de TCR-, b2m- en HLA-locus.
In dit protocol laten we zien hoe CRISPR-engineering van T-cellen kan worden gecombineerd met virale vectorgemedieerde levering van het CAR-Transgen om genoom-bewerkte CAR-T-celproducten te genereren met verbeterde werkzaamheid en veiligheid. Een schematisch diagram van het gehele proces is weergegeven in figuur 1. Met behulp van deze aanpak hebben we een zeer efficiënte gen knock-out aangetoond in primaire menselijke CAR-T-cellen. Figuur 2A beschrijft in detail de tijdlijn van elke stap voor het bewerken en vervaardigen van T-cellen. Strategieën voor gids RNA-ontwerp en knock-out validatie worden ook besproken om deze aanpak toe te passen op verschillende doelgenen.

Adoptieve celtherapieën met behulp van chimere antigeenreceptor T-cellen (CAR-T-cellen) hebben een opmerkelijke klinische werkzaamheid aangetoond bij patiënten met hematologische maligniteiten en worden momenteel onderzocht voor verschillende solide tumoren. CAR-T-cellen worden gegenereerd door T-cellen uit het bloed van een patiënt te verwijderen en ze te manipuleren om een synthetische immuunreceptor tot expressie te brengen die de T-cellen omleidt om doeltumorcellen te herkennen en te elimineren. Genbewerking van CAR-T-cellen heeft het potentieel om de veiligheid van de huidige CAR-T-celtherapieën te verbeteren en de werkzaamheid van CAR-T-cellen verder te verhogen. Hier beschrijven we methoden voor de activering, uitbreiding en karakterisering van menselijke CRISPR-engineered CD19-gerichte CAR-T-cellen. Dit omvat transductie van de CAR lentivirale vector en het gebruik van single guide RNA (sgRNA) en Cas9 endonuclease om genen van belang in T-cellen te targeten. De methoden die in dit protocol worden beschreven, kunnen universeel worden toegepast op andere CAR-constructies en doelgenen die verder gaan dan de genen die voor deze studie worden gebruikt. Bovendien bespreekt dit protocol strategieën voor gRNA-ontwerp, lead gRNA-selectie en doelgen knock-out validatie om reproduceerbaar een hoog-efficiënte, multiplex CRISPR-Cas9-engineering van klinische kwaliteit menselijke T-cellen te bereiken.

We beschrijven hier een protocol om T-cellen genetisch te manipuleren, dat kan worden gebruikt om zowel autologe als allogene CAR-T-cellen te genereren, evenals TCR-omgeleide T-cellen.

Ons laboratoriumprotocol maakt de combinatie van menselijke genoombewerking met CAR T-celtherapie mogelijk. De laboratoriumprocedure is de aanpak met CRISPR-Cas9 om maximaal drie genen met een hoge efficiëntie te richten. En ten slotte kan onze aanpak worden vertaald naar klinische proeven bij mensen.De beschreven methoden kunnen worden toegepast op andere CAR-constructen en doelgenen. De basis van de technieken is robuust genoeg om te worden vertaald naar grotere schaal en naar academische en industriële medewerkers voor verdere ontwikkeling. Wanneer u dit protocol voor de eerste keer probeert, beperk dan het aantal groepen in het gids-RNA-scherm om nauwkeurige incubatietijden mogelijk te maken.Het naleven van de kweekomstandigheden en zaaidichtheid zoals beschreven zijn in onze ervaring van cruciaal belang gebleken. Om te beginnen, verkrijg autologe perifere bloed mononucleaire cellen, of PBMC's, van gezonde vrijwillige donoren en isoleer CD4- en CD8-positieve T-cellen met behulp van commercieel verkrijgbare CD4- en CD8-selectiekits. Combineer CD4-positieve en CD8-positieve T-cellen in een één-op-één-verhouding en incubeer 3 miljoen cellen per milliliter in R10, aangevuld met 5 nanogram per milliliter van elke menselijke IL7 en menselijke IL15 's nachts bij 37 graden Celsius.Tel de volgende dag de T-cellen en centrifugeer 5 tot 10 miljoen cellen op 300 keer G gedurende vijf minuten. Gooi alle supernatant weg en was de celkorrel in gereduceerd serum minimale essentiële media. Suspensie de pellet opnieuw in 100 microliter nucleaire affectie-oplossing volgens de instructies van de fabrikant.Bereid tijdens het wassen van de cellen een ribonucleoproteïne of RNP-complex voor door 10 microgram Cas9-nuclease gedurende 10 minuten bij kamertemperatuur te incuberen met 5 microgram single guide RNA. Elektroporateer de cellen met behulp van pulscode EH111, incubateer vervolgens 5 miljoen cellen per milliliter in R10, aangevuld met 5 nanogram per milliliter menselijk IL7 en menselijk IL15 bij 30 graden Celsius gedurende 48 uur in 12 putplaten. Ga na de incubatie verder met T-celactivering en -expansie.Ontwerp de sequencing primers door de volgorde van het PCR-amplicon in te voeren in een standaard primerontwerpsoftware. De ontwerpsoftware zal meerdere primersequenties voorstellen die geschikt zijn voor Sanger-sequencing. Kies voorwaartse en omgekeerde primers die met het amplicon ten minste 150 basenparen stroomopwaarts of stroomafwaarts van de gRNA-snijplaats binden om een goede sequencingkwaliteit rond de indels te garanderen.Gebruik TIDE-analyse om knock-outefficiëntie op genomisch niveau te detecteren. Het algoritme reconstrueert nauwkeurig het spectrum van indels uit de sequentiesporen en berekent R-kwadraatwaarden. Na activering worden T-cellen uitgebreid in cultuur en gecryopreserveerd voor toekomstige studies.Tijdens de uitbreiding worden de populatieverdubbeling en volumeveranderingen in het hele protocol bijgehouden voor zowel mock- als bewerkte CAR T-cellen. Er werden geen significante veranderingen gedetecteerd in de proliferatie en activering tijdens de uitbreiding. Zodra de cellen waren gecryopreserveerd, werden niveaus van CAR-expressie bepaald voor verdere functionele studies voor zowel de mock-bewerkte als knock-out CAR T-cellen.Er werden geen significante veranderingen waargenomen. Knock-out efficiëntie kan worden bepaald met behulp van meerdere technieken. In deze representatieve flowcytometrieplots werden de PDCD1- en TRAC-loci gericht met behulp van single guide RNA, met een efficiëntie van 90% voor het PDCD1 single guide RNA en 98% voor het TRAC single guide RNA voor meerdere gezonde donoren.Het is belangrijk om ervoor te zorgen dat de molaire verhoudingen van Cas9 en geleideRNA's nauwkeurig zijn. Tijdens de procedure moeten de cellen altijd op vier graden Celsius worden gehouden en niet gedurende langere tijd in de elektroporatieoplossing worden achtergelaten. Na car-conservering kunnen de CRISPR-gemanipuleerde CAR T-cellen worden gebruikt voor in-vitro en in-vivo functionele assays, zoals cytotoxiciteitstests, cytokineproductie en syngenetische of xenografische tumormuismodellen.Onze aanpak met CRISPR Cas9-technologie wordt nu toegepast op klinische onderzoeken. De aanpak kan zowel voor kanker worden gebruikt, als voor een niet-kwaadaardige genetische aandoeningen, zoals hemoglobinofaties, die miljoenen kinderen en volwassenen wereldwijd treffen.
