石灰化大動脈弁疾患(CAVD)は、大動脈弁尖の炎症、線維症、および肉眼石灰化を特徴とする慢性病理です。リーフレットの進行性のリモデリングと石灰化(大動脈硬化症と呼ばれる)は、血流の閉塞(大動脈弁狭窄症)を引き起こし、脳卒中の一因となり、心不全を引き起こす可能性があります。現在、CAVDの唯一の治療法は、外科的または経カテーテル的大動脈弁置換術(それぞれSAVRおよびTAVR)です。CAVDの進行を停止または逆転させる非外科的選択肢はなく、弁置換術を行わないと、死亡率は2〜3年以内に50%に近づきます1,2,3。この病態を駆動する根本的なメカニズムを定義することで、潜在的な新しい治療アプローチが特定されます。
健康な成人では、大動脈弁尖の厚さは約1ミリメートルであり、その主な機能は左心室4からの一方向の血流を維持することです。3つのリーフレットのそれぞれは、リーフレットの外面を裏打ちし、バリアとして機能する弁内皮細胞(VEC)の層で構成されています。VECは、透過性、炎症性細胞接着、およびパラクリンシグナル伝達を調節することにより、弁の恒常性を維持します5,6,7。弁間質細胞(VIC)は、弁尖8内の細胞の大部分を構成する。VICは、リーフレットの3つの特徴的な層に配置されています。これらの層は、心室、海綿状筋、および線維症として知られています9。心室は左心室に面しており、コラーゲンとエラスチン繊維を含んでいます。中間層である海綿状菌には、心周期中にせん断の柔軟性を提供する高いプロテオグリカン含有量が含まれています。外側の線維層は大動脈側の流出面の近くに位置し、拡張期10,11,12の間に共離力を維持する強度を提供するI型およびIII型繊維状コラーゲンが豊富です。VICは静止状態にありますが、炎症、細胞外マトリックス(ECM)のリモデリング、機械的ストレスなどの要因により、VICの恒常性が破壊される可能性があります8、9、13、14、15、16。恒常性が失われると、VICは細胞外ミリエウをリモデリングするタンパク質の増殖、収縮、分泌が可能な筋線維芽細胞様表現型を活性化して獲得します17。活性化されたVICは、間葉系幹細胞(MSC)の骨芽細胞への分化を連想させる石灰化細胞に移行することができる15、17、18、19、20、21、22、23、24、25。
石灰化は、VECとVICの両方の寄与からコラーゲンに富む線維症層で開始されるように見えますが、リーフレット8の他の層を拡大して侵入します。したがって、VECとVICの両方が骨形成遺伝子の発現をアップレギュレートする刺激に応答することは明らかですが、骨形成遺伝子の活性化を引き起こす正確なイベント、および細胞とリーフレットの細胞外マトリックスとの間の複雑な相互作用は、明確に定義されていないままです。マウスはCAVD de novo26,27を発症しないため、マウスモデルはCAVD病因の非遺伝的ドライバーを研究するための理想的な情報源ではなく、したがって、初代ヒト組織およびこれらの組織から分離された初代細胞株の使用が必要である。特に、3D細胞培養やオルガノイドモデリングの分野が拡大しており、マウスモデルに代わるex vivoヒトベースの代替となる可能性が高いため、これらの細胞を大量かつ高品質で取得することが不可欠です。

ヒト一次弁内皮細胞および間質細胞の単離は、石灰化大動脈弁疾患の病因の根底にあるメカニズムを理解するために重要です。弁が天然組織から切除されると、細胞の生存率が低下します。そこで、細胞生存率を延長する方法を特定しました。抽出した弁組織サンプルを受け取った後、大動脈根を抽出し、滅菌すすぎ液を入れた50ミリリットルの円錐管に組織を沈めます。ロッカーのアイスバケットで10分間インキュベートした後、70%エタノールをチューブにスプレーします。滅菌組織培養フードで、組織をペトリ皿に移し、2つの弁尖を切除する。1枚のリーフレットを極低温バイアルに入れ、組織を液体窒素でスナップ凍結して摂氏マイナス80度で保管します。カルシウム含有量染色のために組織を固定するには、2番目のリーフレットを結節からヒンジまで半分に切り、両方の組織片を4%パラホルムアルデヒドに浸したカセットに入れます。次に、セットアップ全体を室温で最低2時間、4時間以内ロッカーに置きます。弁間質細胞を単離するには、氷冷PBSを含む新しい50ミリリットルの円錐管にリーフレットを移し、室温で2分間ロッカーにチューブを置きます。混合後、組織を5〜7ミリリットルの冷たいコラゲナーゼ溶液を含む60ミリメートルの皿に移します。鉗子を使用してリーフレットの両面を溶液に3〜4回浸してから、細胞培養インキュベーター内で組織を摂氏37度で5〜10分間インキュベートし、2分ごとに3〜4回組織を穏やかに揺らします。インキュベーションの終わりに、2ミリリットルの溶液を皿から滅菌15ミリリットルの円錐管に移します。リーフレットの結節に鉗子を配置し、滅菌綿棒を使用して、リーフレットに沿って綿棒を回転させながら、鉗子からヒンジまで組織をスワイプします。スワイプした後、コラゲナーゼ溶液のチューブで綿棒をすすぎ、先ほど示したように組織の反対側を綿棒で拭きます。すすぎ後、組織の両側で綿棒を繰り返し、1ミリリットルのピペットとコラゲナーゼ溶液を使用して、リーフレット表面の両側からディッシュに細胞を取り除きます。すすぎ後、皿から綿棒からの細胞を含むチューブにすべての溶液を移し、残りの弁組織を7ミリリットルの滅菌コラゲナーゼ溶液を含む新しい15ミリリットルの円錐管に入れる。遠心分離によって単離された弁内皮細胞をペレット化し、細胞ペレットを3ミリリットルの血管内皮細胞増殖培地に再懸濁する。2回目の遠心分離後、細胞を計数用の1ミリリットルの新鮮な増殖培地に再懸濁し、コラーゲンコーティングされた6ウェルプレートに5 x 10〜5細胞/平方センチメートルの濃度で細胞を播種し、3〜4日ごとに培地をリフレッシュします。弁内皮細胞パッチがプレートの80%以上を覆っている場合は、増殖速度に応じて、細胞を1平方センチメートルあたり1.3 x 10から4番目の細胞の濃度で分割します。細胞が増殖したら、目的の弁内皮細胞マーカーについて免疫蛍光染色により弁内皮細胞の表現型を確認します。弁間質細胞を単離するには、スワブとリーフレット組織を含むチューブを、キャップを少し開いた状態で12〜18時間細胞培養インキュベーターに入れます。インキュベーションの最後に、血清学的ピペットを使用して組織を穏やかに解離させ、弁間質細胞の放出を確実にし、細胞懸濁液を70ミクロンのストレーナーを通して50ミリリットルの円錐形チューブにろ過します。7ミリリットルのバルブ間質細胞培地をチューブに加え、遠心分離によって細胞を回収します。計数用の1ミリリットルのフレッシュバルブ間質細胞増殖培地にペレットを再懸濁し、直径60ミリメートルの組織培養処理皿に1平方センチメートルあたり1.3 x 10の4番目の細胞濃度で細胞を播種します。1〜2日後、培養液をPBSで2回洗浄し、細胞に新鮮な培地を加えます。細胞が90%コンフルエントに達したら、培養物をDPBSで2回洗浄し、2〜3ミリリットルの予め温めた解離試薬で細胞を摂氏37度で2〜3分間処理します。細胞が剥離したら、細胞懸濁液を遠心分離用の円錐管に移し、カウントのために3〜4ミリリットルの予熱した弁間質細胞増殖培地にペレットを静かに再懸濁します。次に、元の培養密度に対して1対2の比率で細胞を播種し、実証されたように免疫蛍光染色によって弁間質細胞増殖表現型を評価します。石灰化大動脈弁疾患組織サンプルは、対照の非石灰化組織サンプルと比較して、石灰化の重い結節を伴う変化した形態を示します。石灰化をフォンコッサ染色によって評価すると、罹患したリーフレット組織に暗褐色または黒色の沈殿が観察される。低温保存溶液は、切除された弁組織細胞を大幅に安定化させ、弁摘出後61時間まで、両方のタイプの回収細胞で約40%の生存率が観察されます。弁内皮細胞の形態学的解析により、丸石のような増殖接触阻害細胞が詰まっていることが明らかになり、弁間質細胞は筋線維芽細胞と同様の紡錘形の形態を示します。免疫染色により、拡張された弁内皮細胞および弁間質細胞の大部分が予想される組織特異的マーカーを発現することが確認される。内皮細胞層の放出には、リーフレットの穏やかでしっかりとした拭き取りが重要であり、密な組織内から間質細胞集団を放出するには、コラゲナーゼとの一晩のインキュベーションが必要であることを忘れないでください。細胞株が確立されると、疾患の発症、進行、治療など、特定の大動脈弁疾患の病因に関する機械的研究に使用できます。
