אנו מציגים מודל כירורגי אמין ושחזורי של השתלת אחורית הטרוטופית בחולדות. טכניקה זו אידיאלית למחקרים קצרי טווח על שימור או דחייה חיסונית ב- VCA. תיאור שלב אחר שלב יאפשר לחנכים לשלוט במהירות בניתוח.כדי להתחיל, לבצע חבטה היקפית על העור מעל הקרסול בשליש הדיסטלי של הרגל. שלד וצרוב את העורק הספני ואת הענף הסופי של העורק הפופליטי באמצעות מלקחיים דו קוטביים. לצרוב ולחתוך את gastrocnemius, סולאוס, tibialis, שרירי עצם הירך שרירים עד העצם tibial נחשף.הפוך 2.5 ס"מ חתוך בקמט מפשעתי ימין ולנתק את כרית השומן מפשעתי. השתמש במפסק קרס דגים כדי לתפוס את הרצועה המעשרית. החזק את כרית השומן מפשעתית distally עם מלקחיים הידוק ולבטל את כרית השומן כדי לחשוף את כלי הירך.לנתח את כלי הירך, להתאים אישית את ענפי מרפי, ולגבש עם קשרי ניילון 8/0. הפריניזציה של חולדת התורם על ידי הזרקת הפרין בווריד הפין תוך שימוש במחט 27.5 מד. השלם את רסיס העור סביב הירך.השתמש במלקחיים הדו קוטביים כדי לצרוב את שרירי הירך ואת שרירי gluteus superficialis, ולאחר מכן לצרוב ולחתוך את העצב הסיאטי באורך אמצע עצם הירך. לחשוף את עצם הירך בסמיכות ברמה של פסגת הירך האחורית ולכסות את כלי הירך עם קשרי ניילון ברמה של הרצועה המגינית. בצע עורקים על עורק הירך ממש מתחת לקשירה ולהרחיב כדי להכניס את אנגיוקטטר 24 מד.לצרוב ולחתוך את השריר הנותר מתחת לפדיקור כדי לחשוף את הצד האחורי של עצם הירך. השתמש בחותך העצם כדי לחתוך את השוקה בקרבת מקום ולחתוך את עצם הירך בצורה מנומרת ככל האפשר. לשטוף את אחורי החלקי עם שני מיליליטר של תמיסת מלח הפרין כדי להשיג זרימה ורידית ברורה.מניחים את החלק האחורי על הקרח בגאזה סטרילית עד להעברת המיקרו-וסקולר. לנתח את כרית השומן מפשעתי ולהישען כרית שומן מפשעתית distally לחשוף את כלי הירך. השתמש וו כדי לבטל את הרצועה המעשרית ואת מלקחיים הידוק להחזיק את כרית השומן מפשעתית distally.לנתח את כלי הירך, להתאים אישית את ענפי מרפי, ולגבש עם קשרי ניילון כפי שהודגם. ליגת את שני כלי הדם מעל כלי האפיגסטרי באמצעות קשרי ניילון. מניחים את מלחצי הקירוב בסמיכות, מרחיבים את עדשת כלי הדם, ושוטפים עם תמיסת מלח הפרין.לעשות חתך באגף השמאלי מעל הירך וליצור כיס תת עורי עם מנהרה תת עורית לקמט מפשעה. מניחים את החלק הפרוקסימלי של הגפה החלקית ואת כרית השומן המשתנה דרך המנהרה התת עורית להעברת מיקרו וסקולריים. בצע את anastomoses ורידים ועורקים באמצעות תפרי ניילון.הסר את שני מלחצי הקירוב ובחן את revascularization של האיבר. בצע בדיקת חליבה על שני כלי הדם כדי להעריך את הפטישות של כל anastomosis. לעשות קיסך עור אורך בצד המתיי של הגפה המושתלת ולהכניס את השתל.הסר את העור העודף של השתל ולסגור את הפצע עם תפרים נפרדים תפר ריצה באמצעות תפרים 4/0 נספג. תפרו יחד את רפידות השומן המשתנה של הגפה המושתלת ואת המטופל באמצעות שני תפרים נספגים נפרדים וסגרו את הקמט המגיתי לאחר הבדיקה האחרונה של אנסטומוזיס מיקרו וסקולרי. האבולוציה של המודל ההטרוטופי האחורי הייתה במעקב עד סוף המחקר.צמיחת השיער נצפתה במהלך השבוע הראשון שלאחר הניתוח, והנסיגה עורית הופיעה לאחר שבועיים. כשל הקצאה מרוכבים וסקולריים יכול להתרחש במהלך השבוע הראשון לאחר הניתוח עקב פקקת מיקרו וסקולרית. פקקת ורידים הייתה הגורם להמתת חסד מוקדמת ב -20% מהמקרים, שכולם התרחשו לפני היום החמישי לאחר הניתוח.העור נראה כחול ונעשה כהה יותר בכל יום. השחתה עצמית היא דאגה רצינית בהשתלות שטותיות ולעתים קרובות מתרחשת בין היום השני לשבע לאחר הניתוח. אם מוגבל לפחות משליש משטח השתל ומדאיג רק את העור, ניתן לדון בתפר כירורגי באמצעות תפרים שאינם נספגים עם הווטרינר של הצוות.מניעת השחתה עצמית מסתמכת על שימוש בצווארון אלקטרוני התפור לצוואר עד היום השביעי שלאחר הניתוח וניקוי כל דם או קרום על הפצעים הכירורגיים של החיה. אוטופגיה חמורה של שכבות מרובות של השתל מובילה להמתת חסד של החיה. ציסטות עור הופיעו לאחר יום לאחר הניתוח 14, לפעמים עם מרכז נמק עורי לפני פיסטולה.חשוב לוודא כי מיקום השתל לא ימנע את בעל החיים מלהיות אמבולטורי, וכי הפדיקור אינו קינק או הדוק במהלך השתל. הצוות שלנו ביצע מחקרי שימור בגפיים שנרכשו לפני ההשתלה. ההשפעות של משכי זמן שונים של אחסון קר סטטי הוערכו, כמו גם פרוטוקולי זלוף גפיים שונים.
