ラットにおける異所性後肢移植の信頼性と再現性のある外科モデルを紹介する。この技術は、VCAにおける保存または免疫拒絶反応に関する短期研究に最適です。ステップバイステップの説明は、研修生が迅速に手術を習得することができます。まず、脚の遠位 3 分の 1 で足首の上の皮膚に周回の切開を行います。双極鉗子を用いて、伏在性動脈および大窩動脈の末端枝を骨格化し、焼灼する。発灼化し、脛骨の骨が露出するまで、胃腸、ソレウス、脛骨前、および上腕二頭筋の筋を切断する。右のりぐるしで2.5センチメートルの切開を行い、インギナル脂肪パッドを解剖します。魚のフックレトラクターを使用して、インジュナル靭帯を把握します。鉗子を締め付け、脂肪パッドを引き込んで大腿骨の血管を露出させるため、うるさい脂肪パッドを遠ざけます。大腿骨の血管を解剖し、マーフィーの枝を個別化し、8/0ナイロンタイでリゲートします。27.5ゲージ針を用いて陰茎の側静脈にヘパリンを注入することにより、ドナーラットをヘパリン化する。股関節の周りの皮膚切開を完了します。双極鉗子を使用して上腕二頭筋と大腿骨筋を焼灼し、大腿骨の中枢で坐骨神経を焼灼して切る。大腿骨を後腿骨の紋章のレベルで近位に露出させ、結膜靭帯のレベルでナイロンの結びつきを持つ大腿骨の血管をリゲートする。合字のすぐ下の大腿動脈に動脈管内術を行い、24ゲージの血管カテーテルを挿入するために拡張する。大腿骨の前側を露出させるためにペディクルの下に残りの筋肉を焼灼し、切断します。骨カッターを使用して脛骨を近位に切断し、可能な限り遠位に切ります。ヘパリン生理食塩水の2ミリリットルで部分的な後肢を洗い流し、明確な静脈流出を得る。微小血管移動まで滅菌ガーゼで氷の上に後肢を置きます。りうる脂肪パッドを解剖し、大腿骨の血管を露出させるために遠位にイングイナル脂肪パッドをリクライニングする。フックを使用して、イングイナル靭帯とクランプ鉗子を引き込み、イングイナル脂肪パッドを遠位に保持します。大腿骨の血管を解剖し、マーフィーの枝を個別化し、実証されているようにナイロンの結びつきでリゲートします。ナイロンタイを使用して、上頭血管の上に両方の容器をリゲートします。近似クランプを近位に置き、容器レンズを拡張し、ヘパリン生理食糸ですすいます。腰の上の左側面に切開を行い、皮下の折り目に皮下トンネルを持つ皮下ポケットを作成します。微小血管移動のために、部分的な四肢の近位部分と皮下脂肪パッドを皮下トンネルに入れる。ナイロン縫合糸を使用して静脈および動脈のアナストモーゼを行います。両方の近似クランプを取り外し、四肢の再血管化を観察します。両血管に搾乳試験を行い、各吻合のステイシーを評価します。移植した四肢の内側側に縦方向の皮膚切開を行い、移植片を挿入する。移植片の余分な皮膚を取り除き、吸収可能な4/0縫合糸を使用して別々の縫合糸とランニング縫合糸で創傷を閉じます。移植された四肢のうりご脂肪パッドとレシピエントを2つの別々の吸収性縫合糸を使用して縫合し、微小血管吻合の最後の検査後に、脳の折り目を閉じる。異所性後肢モデルの進化は、研究の終わりまで監視された。術後1週間目に毛髪の再成長が観察され、2週間後に皮状の引き込みが現れた。血管化複合同種移植不全は、微小血管血栓症による術後第1週に起こり得る。静脈血栓症は、20%の症例で早期安楽死の原因であり、そのすべてが術後5日目の前に起こった。肌は青く見え、毎日暗くなりました。自己突然変異は無感覚移植片の深刻な懸念事項であり、術後2日目から7日目の間にしばしば起こる。移植片表面の3分の1以下に制限され、皮膚のみに関わる場合、非吸収性縫合糸を用いた外科的脱花剤および縫合糸は、スタッフ獣医師と議論することができる。自己突然変異の予防は、術後7日目まで首に縫い付けた電子首輪を使用し、動物の外科的傷に血液または地殻を洗浄することに依存する。移植片の複数の層の重度のオートファジーは、動物の安楽死につながる。皮膚嚢胞は術後14日目以降に現れ、時には瘻孔の前に皮膚壊死中心を有する。移植片の配置が動物が歩行するのを妨げず、移植片の発症時にペディクルがキンクまたはタイトでないことを確認することが重要です。我々のチームは、移植前に調達した手足の保存研究を行った。静的なコールド ストレージの異なる持続時間の影響は、異なる四肢灌流プロトコルと同様に評価されています。
