Assisterad befruktning (ART) har i allt högre grad använts för behandling av infertilitet. Framgångsfrekvensen för ART, såsom IVF, har dock varit begränsad, och missfallsfrekvensen är betydligt högre än för normalbefolkningen1. Den främsta orsaken till dessa problem är kromosomavvikelser, som vanligtvis förekommer i preimplantatoriska mänskliga embryon2. PGT-A är en effektiv metod för att screena embryon för kromosombalans före implantation 3,4. Vissa studier har visat att PGT-A kan minska antalet aborter och förbättra graviditetsfrekvensen 5,6,7,8. PGT-A kräver dock komplex teknisk expertis som kräver specifik utbildning och erfarenhet. Den invasiva embryobiopsiproceduren kan också potentiellt orsaka skador på embryona9. Även om den långsiktiga biosäkerhetsfrågan för embryobiopsi ännu inte har utvärderats grundligt hos människor, har djurstudier visat dess negativa inverkan på embryoutvecklingen11,12,13.
Tidigare rapporter indikerade att spårmängder av DNA-material utsöndrades i odlingsmediet under embryoutvecklingen, och ansträngningar har gjorts för att utföra omfattande kromosomscreening (CCS) med hjälp av använt embryoodlingsmedium 14,15,16,17,18. Detektionsgraden och noggrannheten i testerna har dock inte uppfyllt kraven för omfattande klinisk användning. Den aktuella studien rapporterade en förbättring av NICS-analysen för att öka detektionsgraden samt noggrannheten hos NICS-testet19. Under de senaste åren har blastocoelevätska (BF) studerats som ett analytiskt prov av minimalt invasiv PGT-A. Andelen framgångsrik amplifiering av hela genomet och detekterbart DNA i blastocystvätskeprover varierar dock från 34,8 % till 82 %20,21,22. Volymen BF som rapporterats i olika studier varierar från 0,3 nL till 1 μL. Med tanke på den låga mängden DNA i BF är det möjligt att öka mängden cellfritt DNA genom att blanda blastocystvätska och odlingsmedium för att förbättra framgångsfrekvensen och konsistensen i detektionen. Kuznyetsov et al.23 och Li et al.24 behandlade zona pellucida med laser och släppte ut blastocystvätska i odlingsmediet för att förbättra den totala mängden embryonalt DNA, och amplifieringshastigheten för de kombinerade medium/BF-proverna efter WGA var 100 % respektive 97,5 %. Jiao et al.25 uppnådde också en 100% förstärkningsframgångsfrekvens genom att använda samma metod.
Den aktuella studien rapporterar ett detaljerat protokoll som inkluderar provberedning av förbrukade medier, NGS-beredning och dataanalys. Genom att försiktigt avlägsna cumulusceller från oocyter utförde den aktuella studien intracytoplasmatisk injektion av enstaka spermier (ICSI) och blastocystodling. Dagen 4-dagars 5/dag 6 spenderade medium samlades in för WGA och NGS bibliotek förberedelse. Genom att använda NICS-teknik effektiviserade den aktuella studien WGA- och NGS-bibliotekens förberedelsesteg på cirka 3 timmar och erhöll CCS-resultat icke-invasivt på cirka 9 timmar.

chromosome screening of human preimplantation embryos by using spent culture medium: sample collection and chromosomal ploidy analysis

Vid klinisk provrörsbefruktning (IVF) kräver den rådande metoden för PGT-A biopsi av ett fåtal celler från trofektodermet (TE). Detta är den härstamning som bildar moderkakan. I den aktuella studien tillämpades den föreslagna metoden på en patient. IRB-godkännande och informerat samtycke erhölls före tillämpningen av NICS-analys. Den aktuella studien erhöll 6 blastocyster från patienter och utförde NICS på alla 6 embryon på dag 4 till dag 5 medium. Protokollet är att normalisera samlingen av odlade medier och kromosomal ploidianalys baserad på icke-invasiva kromosomscreenings-NICS av humana preimplantatoriska embryon. Den största fördelen med NICS är att det är lätt att använda snarare än TiDieR-mikromanipulation, tidsbesparande och minskar effektivt den invasiva biopsiprovförlusten. Provtagningsmetoden är mycket flexibel.Här erbjuder vi dig två olika alternativ, ett för rutinmässig sekventiell embryoodling och ett annat för ett steg. Börja med att förbereda reagenserna och verktygen.Förvärm och jämvikt 20 till 30 mikroliter odlingsmedium och hyaluronidas före användning. Placera sedan odlingsmediet och hyaluronidas på en arbetsyta i ett dragskåp. Överför snabbt de smälta OCCC:erna i odlingsskålen för ägghantering och täck med mineralolja i varje brunn.Aspirera försiktigt och släpp oocyterna fem till 10 gånger för att ta bort kvarvarande granulosaceller runt oocyterna. Överför varje embryo till en unik förvärmd enda mikrodroppe av odlingsmedium för provtagning. Odla blastocystembryot före kryokonservering.För att utföra vitrifieringen före kryokonservering av embryot, förbered odlingens mikrodroppar och överför embryot till förvärmda mikrodroppar på 10 till 15 mikroliter odlingsmedium. Tvätta försiktigt varje embryo i tre mikrodroppar genom pipettering på morgonen den femte dagen. Som tidigare visats, överför varje embryo till en unik förvärmd enda mikrodroppe av odlingsmedium för provtagning.Och odla blastocystembryot vid 37 grader Celsius och 5 % koldioxid före kryokonservering. Justera sedan försiktigt ICM på ett betydande avstånd från laserstrålens målpunkt som fokuserar på trofektodermets cellkorsning för att generera ett litet hål i trofektodermet för att frigöra vätskan från blastocoelhålan. Blanda reaktionen noggrant och centrifugera som tidigare visats.Tillsätt 60 mikroliter av bibliotekets reaktionsblandning och två mikroliter av streckkodsprimern till varje förbiblioteksprodukt. Blanda sedan reaktionen noggrant och centrifugera enligt bilden. Efter centrifugering, placera PCR-röret i termocyklern.Kör bibliotekets förberedelseprogram och håll sedan på fyra grader Celsius. Förbered reagenserna och verktygen. Sex blastocyster erhölls från patienter och NICS utfördes på alla sex embryon från dag fyra till dag fem medium. Kromosomavvikelserna som orsakades av föräldrarnas balanserade translokation upptäcktes i fem av kromosomerna med hjälp av NICS-analysen och därför användes de inte för överföring.NICS-resultaten för de två embryona visade samma karyotyp 45 XN och kromosom 18 deletioner var båda kromosom 18 deletioner. Karyotyp 46 XN korta armen av kromosomen en region deletion var bara den korta armen av kromosomregionen deletion. NICS-resultaten visade karyotyp 46 XN med den långa armen av deletion av kromosom 18-region och den korta armen av kromosom en regionduplikation.Även om karyotyperna var kromosom fem duplikationer och visade åtta mosaikskillnader, kunde NICS-analysen screena alla 24 kromosomer för aneuploidi. Denna process ger en ny metod för överföring av enstaka normala blastocyster av karyotyp. När du försöker med denna procedur är det viktigt att komma ihåg att helt ta bort de kumulativa cellerna för att undvika kontaminering av moderns ursprung och samla in odlingsmediet och det förklarade blastocyststadiet när DNA-innehållet är tillräckligt för amplifiering.Genom att använda NICS-teknik effektiviserade den aktuella studien WGA- och WGS-bibliotekets förberedelsesteg på cirka tre timmar och opt-in CCS-resultat icke-invasivt på cirka nio timmar. Efter utvecklingen gav tekniken kliniker inom ART ett nytt sätt att kombinera morfologisk bedömning med NICS för att överföra aploida embryon med god morfologi till livmodern och förbättra den pågående graviditetsfrekvensen och antalet levande födslar.
