駆出率が保たれた心不全(HFpEF)は、心不全症例の半数以上を占めており、世界的な公衆衛生上の問題となっています1。臨床観察では、HFpEFのいくつかの重要な特徴が示されています:(1)収縮期硬化の増加を伴う心室拡張期機能障害、(2)運動能力の低下を伴う安静時の正常駆出率、および(3)心臓リモデリング2。提案されたメカニズムには、ホルモン調節不全、全身性微小血管炎症、代謝障害、およびサルコメアおよび細胞外マトリックスタンパク質の異常が含まれます3。しかし、実験的研究は、駆出率の低下を伴う心不全(HFrEF)がこれらの変化を引き起こすことを示しています。臨床研究では、アンジオテンシン受容体阻害剤とHFpEFのHFrEF治療薬の治療効果が調査されています4,5。しかし、HFpEFには独自の治療法が必要です。臨床症状の理解と比較すると、HFpEFの病理学、生化学、および分子生物学の変化は十分に定義されていません。
HFpEFの動物モデルは、そのメカニズム、診断マーカー、および治療アプローチを探求するために開発されています。ブタ、イヌ、ラット、マウスなどの実験動物はHFpEFを発症する可能性があり、高血圧、糖尿病、老化など多様な危険因子を誘導因子として選択しました6,7。例えば、酢酸デオキシコルチコステロン単独または高脂肪/高糖質飼料と組み合わせると、豚にHFpEFが誘導されます8,9。心室圧過負荷は、大動物および小動物モデルでHFpEFを発症するために使用される別の技術です10。また、欧州心臓病学会のガイドライン、米国心臓病学会財団/米国心臓協会11、日本循環器学会/日本心不全学会12に見られるように、近年、HFpEFを定義するための特定のEFカットオフ値が大陸を超えて採用されています。したがって、臨床基準が採用されれば、以前に確立された多くのモデルがHFpEF研究に適している可能性があります。例えば、Youselfiらは、遺伝子改変マウス系統Col4a3-/-が有効なHFpEFモデルであると主張した。この株は、拡張期機能障害、ミトコンドリア機能障害、心臓リモデリングなどの典型的なHFpEF心臓症状を発症した13。以前の研究では、高エネルギー食を使用して、代謝障害、線維症、および心筋のアクトミオシンMgATPaseの減少を特徴とする高齢のサル14で中程度のEFによる心臓リモデリングを誘発しました。マウス横大動脈狭窄術(TAC)は、高血圧誘発性心室性心筋症を模倣するために最も広く使用されているモデルの1つです。左心室は、EFの増加を伴う同心円状の肥大から、EF15,16の減少を伴う拡張型リモデリングへと進行する。これら2つの典型的な病期の間の移行表現型は、大動脈狭窄法を使用してHFpEFを研究できることを示唆しています。
