深部低体温循環停止(DHCA)は、1953年以来心臓外科で使用されています1。DHCAには、体への血流を世界的に遮断する前に、患者の深部温度を深刻な低体温レベル(15〜22°C)に下げることが含まれます2。循環停止は、比較的無血の手術野を提供することができます。深部低体温は、特に脳および心筋における代謝を低下させ、これは虚血に対する効果的な保護方法である3。DHCAは、複雑な先天性心疾患、大動脈弓疾患、さらには大静脈血栓を伴う腎腫瘍または副腎腫瘍の手術中に一般的に適用されます4,5。したがって、DHCA動物モデルの確立は、手順の改良と臨床現場での合併症の予防のための重要な参考資料を提供します。

深部低体温循環停止(DHCA)は、複雑な先天性心疾患および大動脈弓疾患の手術中に日常的に適用されます。本研究は、ラットにおいてDHCAを確立する方法を提供することを目的とする。バイタルサインに対するDHCAプロセスの影響を評価するために、循環停止のない常温心肺バイパス(CPB)ラットモデルを対照として使用しました。予想通り、DHCAは体温と平均動脈血圧の有意な低下をもたらしました。血液ガス分析の結果,DHCAは乳酸濃度を上昇させたが,血中pHおよびヘモグロビン,ヘマトクリット,Na+,Cl-,K+,グルコース濃度には影響しなかった。さらに、常温CPBラットと比較して、透過型電子顕微鏡観察の結果、DHCAラットにおける海馬オートファゴソームの軽度の増加を示した。

ラットにおける深部低体温循環停止の確立。深部低体温循環停止であるDHCAは、複雑な先天性心疾患および大動脈弓疾患の手術中に日常的に適用されます。本研究は、ラットにおいてDHCAを確立する方法を提供することを目的とする。プロトコルは治験審査を受け、中国医学科学院不破病院の施設動物管理および使用委員会から承認を得ました。準備作業。CPB回路に、モルフドロッパー、ローラーポンプ、熱交換器、膜型酸素供給器、接続チューブ、および回路を接続した水タンクから変更されたリザーバーが含まれていることを確認し、1ミリリットルのヘパリンナトリウムと1ミリリットルの生理食塩水で12ミリリットルのヒドロキシデンプンを作りますローラーポンプを穏やかに回転させて40ミリリットルのプライミング溶液で回路をプライミングします。第二に、麻酔とカニューレ挿入。チューブを人工呼吸器とサイドパラメータに接続します。電気加熱毛布と右側を置き、テープで右向きにします。眼科用軟膏を目に塗り、乾燥を防ぎます。シェーバーで右鼠径部、右頸部、尾部の毛を剃ります。その後、アルデヒドとアルコールで皮膚を消毒します。皮膚と左鼠径部を切断し、筋肉と組織の軟骨を解剖して左大腿静脈と動脈を露出させます。動脈を慎重に分離します。22ゲージの静脈内カテーテルを左大腿動脈にカニューレ挿入します。絹で結紮された動脈とカテーテル。凝固を避けるためにカニューレを洗い流すためにヘパリンを含む生理食塩水を使用してください。カテーテルをプレスセンサーに接続して血圧を監視します。尾の皮膚を切り取り、メスを使って尾動脈の表在性を切断し、術野の中央にある尾動脈を露出させます。22ゲージの静脈内カテーテルで尾動脈をカニューレ挿入します。絹で動脈とカテーテルを結睭します。凝固を避けるためにカテーテルを洗い流すためにヘパリンを含む生理食塩水を使用してください。静脈の右頸静脈の皮膚を切り取り、筋肉と組織を分離して静脈を露出させます。16ゲージの自家製マルチエイジ静脈カテーテルを静脈の右外頸静脈に挿入し、右下大静脈または右心房に慎重に入れます。汚染を避けるために、右外静脈が湿ったボールで各カニューレ領域を覆う場所にヘパリンナトリウムを投与します。3つ目は、回路を尾動脈のカテーテルに接続し、ポンプを毎分1〜2ミリリットルの低レートに保つことです。次に、リザーバーを右外頸静脈のカテーテルに接続します。リザーバーには常に約1センチメートルの血中濃度があることを確認してください。水タンクの電源を入れ、最初に水温を摂氏37度に設定します。血圧が安定したら、ポンプを毎分80〜100ミリリットルまで静かに増やします。4、冷却。室温を摂氏20度前後に設定します。この使い捨て手袋に角氷を入れてから、右の頭と側面に置きます。タンクの温度は、米の直腸温度に応じてリアルタイムで提案されました。血液ガス分析の結果に応じて、人工呼吸器の関連パラメータを適切に変更してください。5、深い低体温循環停止。その後、直腸温は摂氏15〜20度に下がります。アイスパックを交換し、手袋を廃棄して、循環停止中の深い低体温症を確実に維持します。ローラーポンプを停止します。リザーバーを環境と接触させ、外頸静脈からリザーバーにゆっくりと血液を排出します。血圧改革に注意してください。血圧と心拍がなくなったら、排水を停止し、貯水池を閉じたままにします。人工呼吸器をオフにします。6、ウォームアップと再灌流。使い捨て手袋をすべて取り外し、室温を摂氏25度に上げます。静脈ドレナージチューブのクリッピングを維持しながら、メモリ酸素インターベンチレーションを復元します。ローラーポンプをオンにして、リザーバー内の血液がゆっくりと正しい体に戻ることを確認します。貯水池の血中濃度が1センチメートルのままになったら、人工呼吸器の電源を入れます。ドレナージチューブを使用して、右心房からリザーバーに血液をゆっくりと排出します。加熱ランプ、加熱パッド、水タンクをオンにします。水タンクの温度を摂氏25度に設定します。次に、タイムリーに左の温度に調整します。直腸温度が摂氏34度に戻ったら、加熱ランプを取り外します。7、CPBから離乳します。ポンプ流量のローラーをゆっくりと徐々に減らし、大静脈属の速度を毎分1ミリリットルに減らす流量に調整します。貯水池を環境と接触させてください。残りの血液を毎分1ミリリットルの流量で回路に注入します。メモリー酸素化とローラーポンプを停止します。機械的換気と深い麻酔の期間の後に安楽死させた。代表的な結果。対照群としては、常温CPB、NtCPB、ラットが挙げられる。循環停止なしでは、処置全体を通して安定した平均動脈血圧MAPおよび体温を示した。一方、DHCAラットのMAPは心停止中に減少した。DHCAラットの温度は冷却段階で急速に低下し、回転段階で徐々に回復しました。DHCA回路の離乳により、DHCAラットの温度は正常に戻った。上記のデータは、DHCA手順が診療所の患者の実際の体温とMAPの変化を模倣していることを示しました。DHCA処理装置および上昇の影響を血液ガス分析により検討した。一次溶液との全血接触後、ヘモグロビンの濃度は両方の群でデシリットル当たり6グラムより高かった。DHCA表面からの離乳により、CPB回路内の残りの血液が右側に再注入されたため、デシリットルあたり9グラムに増加しました。ヘマトクリット値HCTはHBと同様の傾向を示した。DHCA再灌流後、乳酸酸は急速に増加し、DHCA群でより顕著であった。DHCA手順後にpHが低下しましたが、これは乳酸の蓄積の結果である可能性が最も高いです。実験全体を通して、ナトリウム、塩化物、カリウムイオン、およびグルコースの濃度は、各10点で有意差を示さなかった。この結果から,DHCAは乳酸への増加を誘導しただけで,血中pHやヘモグロビン,ヘマトクリット,ナトリウムイオン,塩化物イオン,カリウムイオン,グルコース濃度には影響しないことが示唆された。神経細胞のオートファジーに対するDHCAの影響を評価するために、透過型電気顕微鏡を使用し、驚くべきことに、DHCAラットの海馬でオートファゴソームの数を発見し、増加させました。上記のデータは、DHCAモデルが診療所の患者の実際の体温とMAPの変化を模倣していることを示しました。このモデルは、全身性炎症反応症候群、神経炎症などを調査するために適用することができる。
