急性MIが発生すると、梗塞領域の心筋細胞は虚血と低酸素症のために急速に死滅します。いくつかの炎症因子は細胞死と破裂の後に放出されますが、炎症細胞は梗塞部位に浸潤して炎症を引き起こします1。重要なことに、線維芽細胞およびコラーゲンは、いずれも収縮性および導電性を伴わず、梗塞部位の心筋細胞に取って代わり、瘢痕組織を形成する。成体哺乳動物における心筋細胞の再生能力が限られているため、大面積の梗塞後に形成される生組織は、通常、十分な心拍出量を維持するのに十分ではありません2。MIは心不全を引き起こし、心不全の重症例では、患者は正常な心機能を維持するために心臓移植または補助人工心臓のみに頼ることができます3,4。
MI後の理想的な治療戦略は、死んだ心筋細胞を新しく形成された心筋細胞に置き換え、健康な組織との電気機械的結合を形成することです。ただし、治療オプションは通常、置換ではなく心筋サルベージを採用しています。現在、幹細胞および前駆細胞ベースの治療法は、MI5後の心筋修復を促進するための最も有望な戦略の1つです。しかし、これらの細胞の移植にはいくつかの問題があり、主に成体幹細胞が心筋細胞に分化できないことと、それらの寿命が短いことです6。
胚性幹細胞(ES)細胞の使用に関連する倫理的問題は、有望な細胞源であるiPS細胞によって回避することができます。さらに、iPS細胞は強力な自己複製能力を持ち、心筋細胞に分化することができます7。研究は、MI部位に移植されたhiPSC-CMが生存し、宿主細胞とギャップ結合を形成することができることを示しています8,9。しかし、これらの移植細胞は虚血や炎症の微小環境にあるため、生存率は極めて低い10,11。
移植細胞の低酸素症およびヒートショックの前処理12,13、遺伝子改変14,15、および細胞および毛細血管の同時移植16など、移植細胞の生存率を改善するためのいくつかの方法が確立されている。残念ながら、ほとんどの方法は複雑さと高コストによって制限されています。したがって、本研究では、再現性があり、便利で、比較的侵襲的で、効果的なhiPSC-CM送達方法を提案する。
超音波ガイド下心筋内細胞注入は、部位を問わず、高解像度の小型動物用超音波装置とマイクロインジェクターのみで行うことができます。超音波ガイダンスの下で、剣状突起下の細胞を心膜からマウスの心筋に直接送達することは、肝臓と肺の損傷を回避する安全なプロトコルです。この方法は、他の技術と同時に組み合わせて、移植細胞の生存率を大幅に向上させることができます。

ultrasound-guided induced pluripotent stem cell-derived cardiomyocyte implantation in myocardial infarcted mice

心エコーガイド下心筋細胞への経皮的心筋内注射がヒトiPS由来心筋細胞の修復特性を有意に高めることを実証した最初の報告です。この方法は、ヒトiPS由来心筋細胞治療を実施するための実行可能で、満足のいく、再現性の低い侵襲性および有効な手順である。経皮的心筋内送達は、虚血性心疾患、心筋梗塞、心不全などの疾患に対する局所薬物送達のための安全で効率的な方法であると考えています。このプロジェクトは、Jinfu Yang教授とJun Peng教授によって指導されています。手順を実演するのは、私の研究室のXun Wu、Kele Qin、Kun Xiang医学博士号、および湖南省心臓血管研究の主要研究所の博士号であるDi Wangです。まず、hiPSC-CMの60日間培養液から培養上清を真空吸引します。細胞をPBSで3回洗浄する。次に、細胞剥離酵素混合物を摂氏37度で3〜5分間使用して、hiPSC-CMを個々の細胞に解離させます。5ミリリットルのRPMIとB27培地を含む15ミリリットルのチューブに細胞を集めて混合します。セルをカウントし、セルの総数を計算します。細胞懸濁液を300Gで室温で2分間遠心分離する。上清を廃棄し、ペレットを心筋細胞支持培地に1マイクロリットル当たり6×10の4番目の細胞の濃度まで再懸濁する。注入ステップまでチューブを摂氏37度に置きます。マウスを十分に麻酔した後、手足と尾をテープで固定します。次に、マウスの切歯を2/0の絹糸とテープで固定します。正中頸部と左胸部をベタインとそれに続くアルコールの3ラウンド交互に消毒します。細かい解剖学的ハサミと細かい解剖鉗子を使用して、中央頸部を切開します。いくつかの前頸部の筋肉を切り取って、気管を安定させ、完全に露出させます。20ゲージのカテーテル穿刺針を気管チューブとして口に挿入します。気管チューブを人工呼吸器に接続し、胸部の動きをチェックして、両方の肺が十分に換気されていることを確認します。呼吸パラメータを調整します。次に、左後肢を右下側に固定し直し、左上肢周りのテープを緩めます。細かい解剖ハサミと鉗子で、左胸部の3〜4肋間腔で開胸術を行います。鉗子を使用して、心膜をはがし、左前下行冠状動脈を見つけます。6/0の絹糸を使用して、動脈の近位端を結紮します。急性心筋梗塞モデルを確実に確立するには、結紮部位の動脈の遠位側が赤から白に変わるまで待ちます。胸部を層ごとに閉じ、4/0の糸で皮膚を縫合します。偽の動物群の場合は、結紮を除くすべての外科的処置を行います。心筋梗塞後の最初の2週間で、マウスを5%イソフルオランで接続された麻酔ボックスに入れ、気管挿管を行います。hiPSC-CMを投与する3日前に、シクロスポリンAを1日1キログラムあたり10ミリグラムの用量で腹腔内に注射する。脱毛クリームでマウスの胸部と上腹部から髪を取り除きます。心拍数が毎分400〜500拍に維持されるまで、イソフルオランを吸入する。マウスの手足と尾を検出コンソールのテープで固定し、プラットフォームの温度を摂氏37度に設定します。上腹部に超音波ゼリーを適用します。高解像度の獣医用超音波プローブを使用して、マウスの肝臓画像を取得します。5マイクロリットルのマイクロシリンジを使用して、約3 x 10を5番目のhiPSC-CMに引き込みます。注射器を持ち、左傍キシフォイド突起に3ミリメートルの針を挿入します。超音波ガイダンスの下で、横隔膜の上端に従います。呼吸リズムと範囲を観察します。呼気円の終わりに心膜に入り、肝臓と肺への損傷を避けます。次にBモード心エコー検査によりマウス心臓の胸骨傍長軸画像を取得する。超音波ガイダンスの下で、5番目のhiPSC-CM細胞に1 x 10を梗塞部位の3つの辺縁領域のそれぞれに注入します。超音波プローブを取り外し、ゼリーを洗い流します。マウスを麻酔から離し、ケージに戻します。移植後1ヶ月間、シクロスポリンAを連続して注射する。左心室心エコーでは,複数回投与群では心筋梗塞による損傷が逆転していた。hiPSC-CMの移植はまた、駆出率の増加と画分の短縮をもたらしました。シリウスレッド染色とファストグリーン染色を用いた心臓パラメータの測定では、移植後に梗塞サイズとコラーゲン体積分率が減少したのに対し、左心室前壁の厚さは増加しました。緑で非特異的アルファアクチン、青でDAPI、赤色でヒト特異的トロポニンTの標識は、梗塞および辺縁梗塞領域に移植細胞の存在を示しました。移植細胞の数と割合は、単回投与と比較して複数回投与群で高かった。心筋梗塞後28日目、複数回投与群では内皮細胞マーカーI-B4を発現する細胞数の増加が認められた。心筋細胞上の小麦胚芽凝集素およびサルコメアアルファアクチニン染色は、最小繊維径の減少を示した。複数回投与群におけるトンネル陽性心筋細胞核の数が少ないことは、アポトーシスの減少を示した。PCR分析では、心臓でのみヒトミトコンドリアDNAが検出され、特定の移植が示されました。針を挿入するときは、肝臓や肺の損傷を避けるために、横隔膜の上縁に沿って、呼気円の端で心膜腔に入ることを確認してください。
