임상적으로 이용가능한 현재의 생체보철 판막은 이종이계 돼지 또는 소 조직(13)에 대한 이식편 대 숙주 반응으로 인해 필연적으로 시간이 지남에 따라 분해된다. 밸브-관련 석회화, 분해 및 불충분은 몇 년 후, 특히 현재의 생체보철 물질(14)에 내재된 특성인 판막의 성장 부족으로 인해 일생 동안 다수의 폐판막 교체를 겪어야 하는 젊은 환자에서 반복적인 개입을 필요로 할 수 있다. 또한, 현재 이용가능한, 본질적으로 비재생성, TPVR 밸브는 혈전색전성 및 출혈 합병증과 같은 주요 한계뿐만 아니라, 리플릿 수축 및 보편적인 판막 기능장애로 이어질 수 있는 불리한 조직 리모델링으로 인한 제한된 내구성을 갖는다(15,16).

경피적 폐판막 치환술은 우심실 유출관 또는 생체 보철 판막 기능 장애를 앓고 있는 환자를 위한 실행 가능한 대안 접근법으로 확립되었으며, 우수한 초기 및 후기 임상 결과를 갖는다. 그러나 스텐트 심장 판막 악화, 관상 동맥 폐색, 심내막염 및 기타 합병증과 같은 임상 적 문제는 평생 적용, 특히 소아 환자에서 해결되어야합니다. 환자를 위한 평생 용액의 개발을 용이하게 하기 위해, 경피적 자가 폐판막 치환술을 성인 양 모델에서 수행하였다. 자가 심낭은 환기와 함께 전신 마취하에 왼쪽 전측 하부 흉부 절제술을 통해 양으로부터 수확되었습니다. 심낭을 2일 및 21시간 동안 비독성 가교결합을 위해 3D 성형 심장 판막 모델 상에 배치하였다. 심장 내 심초음파(ICE) 및 혈관조영술을 수행하여 천연 폐판막(NPV)의 위치, 형태학, 기능 및 치수를 평가하였다. 트리밍 후, 가교된 심낭을 자체 팽창성 니티놀 스텐트 상에 꿰매어 자체 설계된 전달 시스템으로 압착시켰다. 자가 폐 판막 (APV)을 좌측 경정맥 카테터 삽입을 통해 NPV 위치에 이식하였다. ICE와 혈관조영술을 반복하여 APV의 위치, 형태학, 기능 및 치수를 평가하였다.
