Menselijk temporeel botonderzoek biedt een onschatbare bron om de pathologie en pathofysiologie van het binnen- en middenoor te bestuderen. Vóór de 19e eeuw was er weinig bekend over otologische ziekte 1,2,3. Om otologische ziekten beter te begrijpen en "auditieve chirurgie uit de handen van kwakzalvers te redden", ontwikkelde Joseph Toynbee (1815-1866) methoden om histologische delen van het menselijke temporale bot te bestuderen3. Dit werk werd bevorderd door Adam Politzer (1835-1920) in Wenen en anderen in heel Europa gedurende de rest van de 19e eeuw, die temporale botsecties gebruikte om de histopathologie van veel voorkomende aandoeningen van het oor te beschrijven 2,3,4.
Het eerste laboratorium voor menselijke temporale botten in de Verenigde Staten werd in 1927 geopend in het Johns Hopkins Hospital, waar Stacy Guild (1890-1966) methoden ontwikkelde voor temporale botsectie 5,6. De door Guild ontwikkelde methoden bestonden uit een proces van 9-10 maanden met postmortale oogst, fixatie, ontkalking in salpeterzuur, dehydratie in ethanol, celloidine-inbedding, sectie, kleuring en montage. Wijzigingen in deze techniek werden later aangebracht door Harold Schuknecht (1917-1996)7; de basiscomponenten van dit proces blijven echter in wezen ongewijzigd.
De aanzienlijke middelen die nodig zijn om een laboratorium voor temporale botten te onderhouden, hebben een uitdaging gevormd voor tijdelijk botonderzoek en hebben waarschijnlijk bijgedragen aan de afnemende populariteit ervan in de afgelopen 30 jaar 4,8. Een aanzienlijk deel van de temporale botlaboratoriumbronnen moet worden besteed aan het 9-10 maanden durende proces van temporele botvoorbereiding. Een van de meest tijdrovende stappen in de voorbereiding is de ontkalking van het temporale bot, het dichtste bot in het menselijk lichaam. Ontkalking wordt meestal uitgevoerd in salpeterzuur of ethyleendiaminetetra-azijnzuur (EDTA) en duurt weken tot maanden, terwijl de frequente verandering van oplossingenvereist 7,9. Verder kunnen tijdgevoelige studies van het menselijk oor, zoals die met betrekking tot de COVID-19-pandemie, worden belemmerd door dit langzame voorbereidingsproces. Dit artikel beschrijft een techniek voor snelle temporele botsectie die een diamantmicrozaag gebruikt om dikke secties te genereren die snelle ontkalking en weefselanalyse mogelijk maken binnen 10-14 dagen na de tijdelijke botoogst.

Histopathologische analyse van menselijke temporale botsecties is een fundamentele techniek voor het bestuderen van binnen- en middenoorpathologie. Temporale botsecties worden voorbereid door postmortale temporale botoogst, fixatie, ontkalking, inbedding en kleuring. Vanwege de dichtheid van het temporale bot is ontkalking een tijdrovend en grondstofintensief proces; volledige weefselvoorbereiding kan gemiddeld 9-10 maanden duren. Dit vertraagt otopathologisch onderzoek en belemmert tijdgevoelige studies, zoals die relevant zijn voor de COVID-19-pandemie. Dit artikel beschrijft een techniek voor de snelle voorbereiding en ontkalking van temporale botsecties om de weefselverwerking te versnellen.
Temporale botten werden postmortaal geoogst met behulp van standaardtechnieken en gefixeerd in 10% formaline. Een precisie microzaag met dubbele diamantbladen werd gebruikt om elke sectie in drie dikke secties te snijden. Dikke temporale botsecties werden vervolgens gedurende 7-10 dagen ontkalkt in ontkalkingsoplossing voordat ze werden ingebed in paraffine, gesneden in dunne (10 μm) secties met behulp van een cryotoom en gemonteerd op niet-geladen dia's. Weefselmonsters werden vervolgens gedeparaffineerd en gerehydrateerd voor antilichaamkleuring (ACE2, TMPRSS2, Furin) en in beeld gebracht. Deze techniek verkortte de tijd van oogst tot weefselanalyse van 9-10 maanden tot 10-14 dagen. Snelle temporele botsectie kan de snelheid van otopathologisch onderzoek verhogen en de middelen verminderen die nodig zijn voor weefselvoorbereiding, terwijl ook tijdgevoelige studies zoals die met betrekking tot COVID-19 worden vergemakkelijkt.

Hematoxyline- en eosinekleuring van het middenoorslijmvlies en de buis van Eustachius toonde behoud van het middenoorslijmvlies en submucosaal middenoorweefsel na verwerking (figuur 1). Dit protocol verkort de tijd voor temporele botverwerking van 9 tot 10 maanden tot 10 tot 14 dagen. Het belangrijkste voordeel van deze techniek is dat het de snelle beoordeling van otopathologie mogelijk maakt, wat in ons geval relevant is voor de beoordeling van COVID-19-geassocieerde middenoorpathologie. Deze techniek kan bijzonder nuttig zijn voor tijdgevoelige studies van het temporale bot.In de toekomst kan het verder worden geoptimaliseerd voor gebruik bij het bestuderen van binnenoorpathologie. Begin de oogst tijdens de autopsie en verdeel de hersenzenuwen scherp naar de interne gehoorgang. Maak de eerste benige incisie met de osteotome parallel aan en net mediaal aan het plaveiselgedeelte van het temporale bot.Voer vervolgens de tweede benige incisie parallel aan de eerste uit aan de mediale rand van het temporale bot. Maak de derde incisie op drie tot vier centimeter voorste en evenwijdig aan de petrousrug. Maak vervolgens de vierde incisie ongeveer evenwijdig aan de derde op twee centimeter na de petrousrug.Gebruik scherpe dissectie om het zachte weefsel dat langs de inferieure rand van het monster is bevestigd te bevrijden en verwijder het temporale bot. Dompel na de oogst het weefsel onmiddellijk onder in 200 tot 300 milliliter van 10% gebufferd formaline. Zet vervolgens een precisiemicrozaag met dubbele diamantbladen op een afstand van vijf millimeter.Voor snelle weefselontkalking snijdt u elk monster in drie secties, zodat het centrale deel van het temporale bot vijf millimeter dik is en die aan beide uiteinden drie tot vijf millimeter dik zijn. Plaats de secties in 200 tot 300 milliliter 0,37-molaire EDTA gedurende zeven tot 10 dagen bij kamertemperatuur om te verzachten. Hematoxyline- en eosinekleuring van het middenoorslijmvlies en de buis van Eustachius toonde aan dat het weefsel bewaard blijft na verwerking met behulp van dit protocol.Immunohistochemische analyses toonden de aanwezigheid aan van ACE2, transmembraanprotease serine en furine-eiwitten in deze weefsels, die een mogelijke wortel vormen voor SARS-CoV2-infectie in het respiratoire epitheel van het middenoor. Het is belangrijk om het temporale botweefsel in de diamantzaag op de juiste manier te positioneren. Alternatieve technieken die met deze methode kunnen worden gecombineerd, zijn immunohistochemie voor verschillende eiwitten, verschillende weefselvlekken en transcriptomica.Deze techniek stelde ons in staat om snel te beoordelen op de aanwezigheid van de eiwitten die betrokken zijn bij SARS-CoV2-infectie in het middenoor tijdens de COVID-19-pandemie.
