Epstein-Barr-virus (EBV), een γ-herpesvirus ook bekend als humaan herpesvirus type 4, is alomtegenwoordig in de menselijke bevolking en vestigt levenslange latente infectie bij meer dan 90% van de volwassen bevolking1. De meeste PRIMAIRE EBV-infecties treden op tijdens de kindertijd en adolescentie, waarbij een fractie van de patiënten zich manifesteert met infectieuze mononucleosis (IM)2, met karakteristieke immunopathologie, waaronder een geactiveerde immuunrespons met CD8+ T-cellen in het bloed en een voorbijgaande proliferatie van EBV-geïnfecteerde B-cellen in de orofarynx3. Het verloop van IM kan 2-6 weken duren en de meerderheid van de patiënten herstelt goed4. Sommige personen ontwikkelen echter aanhoudende of terugkerende IM-achtige symptomen met een hoge morbiditeit en mortaliteit, die is geclassificeerd als chronische actieve EBV-infectie (CAEBV)5. Verschillende studies hebben de moleculaire handtekeningen voor de immunopathologie van EBV-infectie onderzocht door transcriptoomsequencing. Zhong et al. analyseerden volledige transcriptoomprofilering van perifere mononucleaire bloedcellen (PBMC's) van Chinese kinderen met IM of CAEBV om te ontdekken dat CD8 + T-celexpansie voornamelijk werd gevonden in de IM-groep7, wat suggereert dat CD8 + T-cellen een belangrijke rol kunnen spelen bij IM. Al Tabaa et al. vonden dat B-cellen polyclonaal geactiveerd en gedifferentieerd waren inplasmablasten (CD19+, CD27+ en CD20−, en CD138− cellen) en plasmacellen (CD19+, CD27+ en CD20−, en CD138+) tijdens IM9. Bovendien vonden Zhong et al. dat monocytmarkers CD14 en CD64 werden geupreguleerd in CAEBV, wat suggereert dat monocyten een belangrijke rol kunnen spelen in de cellulaire immuunrespons van CAEBV via antilichaamafhankelijke cellulaire cytotoxiciteit (ADCC) en hyperactieve fagocytose7. Alka et al. karakteriseerden het transcriptoom van MACS gesorteerde CD56dim CD16+ NK cellen van vier patiënten van IM of HL en vonden dat NK-cellen van zowel IM als HL gedownreguleerde aangeboren immuniteit en chemokine signaleringsgenen hadden, die verantwoordelijk zouden kunnen zijn voor de hyporesponsiviteit van NK-cellen10. Daarnaast analyseerden Greenough et al. genexpressie van gesorteerde CD8 + T-cellen van 10 PBMC's van personen met IM. Ze meldden dat een groot deel van de CD8 + T-cellen in IM virusspecifiek, geactiveerd, delend en klaargestoomd waren om effectoractiviteiten uit te oefenen11. Zowel T-cel-gemedieerde, EBV-specifieke responsen als NK-cel-gemedieerde, niet-specifieke responsen spelen essentiële rollen tijdens primaire EBV-infectie. Deze studies onderzochten echter alleen de transcriptoomresultaten van het diverse mengsel van immuuncellen of alleen een bepaalde subpopulatie van lymfocyten, wat niet voldoende is voor de uitgebreide vergelijking van de moleculaire kenmerken en functies van verschillende immuuncelsubpopulaties bij kinderen met IM in dezelfde ziektetoestand.
Dit artikel beschrijft een methode die immunomagnetische kralen en fluorescentie-geactiveerde celsortering (FACS) combineert om gedefinieerde immuuncelsubpopulaties van PBMC's (monocyten, CD4 + T-cellen, CD8 + T-cellen, B-cellen en NK-cellen) te isoleren en te analyseren. Infectieuze mononucleosis (IM) is een acuut syndroom dat meestal wordt geassocieerd met primaire Epstein-Barr-virus (EBV) -infectie. De belangrijkste klinische symptomen zijn onregelmatige koorts, lymfadenopathie en significant verhoogde lymfocyten in perifeer bloed. Het pathogene mechanisme van IM is nog onduidelijk; er is geen effectieve behandelmethode voor, waarbij voornamelijk symptomatische therapieën beschikbaar zijn. De belangrijkste vraag in EBV-immunobiologie is waarom slechts een kleine subgroep van geïnfecteerde personen ernstige klinische symptomen vertoont en zelfs EBV-geassocieerde maligniteiten ontwikkelt, terwijl de meeste personen asymptomatisch zijn voor het leven met het virus.
B-cellen zijn voor het eerst betrokken bij IM omdat EBV-receptoren op hun oppervlak worden gepresenteerd. Natural killer (NK) cellen zijn cytotoxische aangeboren lymfocyten die belangrijk zijn voor het doden van EBV-geïnfecteerde cellen. Het aandeel CD4+ T-cellen neemt af, terwijl dat van CD8+ T-cellen dramatisch uitbreidt tijdens acute EBV-infectie, en de persistentie van CD8+ T-cellen is belangrijk voor levenslange controle van IM. Separation of Immune Cell Subpopulations in Peripheral Blood Samples from Children with Infectious Mononucleosis

We beschrijven een methode die immunomagnetische kralen en fluorescentie-geactiveerde celsortering combineert om gedefinieerde immuuncelsubpopulaties van perifere mononucleaire bloedcellen (monocyten, CD4 + T-cellen, CD8 + T-cellen, B-cellen en natural killer-cellen) te isoleren en te analyseren. Met behulp van deze methode kunnen magnetische en fluorescerend gelabelde cellen worden gezuiverd en geanalyseerd.

Deze methode zal de karakterisering van verschillende immuuncellen in dezelfde ziektetoestanden van personen met IM mogelijk maken, wat ons begrip van het mechanisme van EBV-infectie zal vergroten. De procedure wordt gedemonstreerd door Linlin Zhang, een arts van ons laboratorium. Neem om te beginnen de PBMC's die eerder zijn geïsoleerd in een microcentrifugebuis van 1,5 milliliter en draai deze gedurende 10 minuten op 300 G bij kamertemperatuur.Gooi het supernatant weg en suspensieer de cellen opnieuw met 80 microliter van de voorbereide buffer. Voeg vervolgens 20 microliter CD 14 microbeads toe aan de celsuspensie, goed gemengd door pipetteren, en incubeer de buis gedurende 15 minuten op ijs. Was vervolgens de PBMC's met één milliliter buffer en centrifugeer op 300 G gedurende 10 minuten bij kamertemperatuur.Gooi het hele supernatant weg en suspensieer de cellen opnieuw met 500 microliter van de buffer. Plaats voor magnetische scheiding een kolom op de magnetische kraalscheider en prime de kolom door deze te wassen met drie milliliter van de buffer. Voeg vervolgens de celsuspensie toe aan de kolom.Laat het door en verzamel de niet-gelabelde cellen in een centrifugebuis van 15 milliliter. Was de kolom drie keer met drie milliliter van de buffer en verzamel het volledige effluent in een centrifugebuis van 15 milliliter. Plaats de kolom nu in een nieuwe centrifugebuis van 15 milliliter en voeg vijf milliliter buffer toe aan de kolom.Duw de zuiger stevig in de kolom om de magnetisch gelabelde cellen in de buis te spoelen. Centrifugeer de magnetisch gelabelde cellen op 300 G gedurende vijf minuten. Na het verwijderen van het supernatant, suspensie de cellen opnieuw in 500 microliter PBS en breng de celsuspensie over naar een microcentrifugebuis van 1,5 milliliter voor daaropvolgende transcriptoomsequencing.Pelleteer de verzamelde niet-gelabelde cellen door gedurende vijf minuten te centrifugeren op 300 G en suspensieer de cellen opnieuw in 100 microliter PBS in een microcentrifugebuis van 1,5 milliliter. Voeg vervolgens twee microliter van elk gelabeld antilichaam toe aan de celsuspensie en incubeer gedurende 30 minuten op ijs, beschermd tegen licht. Vervolgens aliquot 100 microliter van de PBMC-celsuspensie in microcentrifugebuizen van 1,5 milliliter om een negatief controlemonster CD 3, CD 4, CD 8 en CD 19, enkele kleuringsmonsters en een CD 56- of CD 16-kleuringsmonster op te zetten.Voeg vervolgens twee microliter van het overeenkomstige fluorescerend gelabelde antilichaam toe aan elke buis en incubeer gedurende 30 minuten op ijs, beschermd tegen licht. Was na incubatie alle cellen met PBS zoals eerder aangetoond en suspensie opnieuw in 500 microliter PBS. Open het celsorteersysteem en voer het stroomprogramma uit.Installeer vervolgens het mondstuk van 85 micrometer en open de sorteerstroom. Stel de sorteerspanning in op 4.500 volt en de frequentie op 47. Stel de opening in op 8 in het afbreekvenster en schakel de automatische sweet spot-sorteermodus in om de amplitudewaarde van de druppel automatisch te bepalen.Stel ten slotte de druppelvertraging in op 30,31 in het zijstroomvenster. Teken met behulp van een voorwaartse versus zijwaartse spreidingspuntplot de polygoonpoort P1 om de intacte lymfocytenpopulatie te identificeren. Teken vervolgens met behulp van een voorwaarts spreidingsgebied versus hoogtepuntplot de polygoonpoort P2 om de enkele cellen te identificeren en doubletten uit te sluiten.Gebruik vervolgens een CD 19 FITC-gebied versus CD 3 per CPCY 5.5-gebiedspuntenplot, teken de rechthoekige poort P3 om CD 3-positieve T-cellen en CD 19-positieve B-cellen te selecteren. Teken vervolgens met behulp van een CD 4 APCCY 7-gebied versus CD 8 PE-gebiedspuntplot een rechthoekige poort om CD 4- en CD 8-positieve T-cellen te selecteren. Teken ten slotte met behulp van een zijverstrooiingsgebied versus CD 56 of CD 16 APC-gebiedspuntplot een rechthoekige poort om CD 56- of CD 16-positieve natural killer-cellen te selecteren.Klik voor de negatieve controlebuis op laden in het acquisitiedashboard en selecteer de cytometerinstellingen in de software. Klik in het controlevenster op het tabblad Parameters en pas de spanningen van voorwaartse verstrooiing, zijverstrooiing en verschillende fluorescerende kleurstoffen aan. Nadat u de enkele gekleurde buizen in de cytometer hebt geladen, klikt u op laden in het acquisitiedashboard en klikt u vervolgens op het tabblad compensatie om de compensatie aan te passen, zoals beschreven in de tekst.Vortex de celsuspensie voorzichtig, voordat de buis in de cytometer wordt geladen. Voeg 200 microliter FBS toe aan vier verzamelstroombuizen en vortex de buizen. Plaats ze in de verzamelkamer van de cytometer.Verzamel de verschillende soorten cellen afzonderlijk in de vier stromingsbuizen. Draai de gescheiden cellen gedurende vijf minuten op 300 G en voeg 200 microliter RNA-isolatiereagens toe aan het celpalet voor transcriptoomsequencing. In de huidige studie werden immuuncellen subpopulaties uit perifeer bloed van verschillende monsters efficiënt gesorteerd door de technieken van immunomagnetische kralen en fluorescentie geactiveerde celsortering te combineren.Belangrijk in dit protocol is dat de celsorteerstap is geoptimaliseerd om de levensvatbaarheid van cellen te verbeteren. De gesorteerde immuuncellen vertoonden een verhoogd aandeel CD 3, CD 8 positieve T-cellen bij patiënten met infectieuze mononucleosis in vergelijking met zowel de gezonde Epstein-Barr-virusdragers als niet-geïnfecteerde monsters. Daarentegen werden verminderde proporties van CD 3, CD 4 positieve T-cellen en CD 19 positieve B-cellen waargenomen bij patiënten met infectieuze mononucleosis in vergelijking met gezonde EBV-dragers en EBV niet-geïnfecteerde kinderen.Bovendien werden verlaagde proporties van CD 56 of CD 16 positieve natural killer cellen waargenomen bij patiënten met infectieuze mononucleosis in vergelijking met EBV niet-geïnfecteerde kinderen. Het handhaven van de levensvatbaarheid van cellen is van vitaal belang voor latere experimenten. Het bewaren van cellen op ijs of in de koelkast tijdens het sorteerproces is gunstig voor de levensvatbaarheid van cellen.
