尿中ATPは、排尿の制御において重要な感覚的役割を果たすと考えられています1。例えば、ATPは膨満に応答して尿路上皮から放出され、膀胱求心性神経の受容体に作用して興奮性を高め、満腹感をもたらすと考えられています2。したがって、尿中ATPは膀胱病変の発症において重要なプレーヤーである可能性があるとも考えられています。この仮説を支持するために、尿中ATP濃度は、過活動膀胱(OAB)3、膀胱痛症候群/間質性膀胱炎(BPS / IC)4、または尿路感染症(UTI)5,6に苦しむ患者で有意に増加します。逆に、膀胱を空にすることができないことを特徴とし、時には膀胱の満腹感を感知する能力の低下を含むことがある低活動膀胱(UAB)に苦しむ患者は、尿中ATP濃度が低下していることが示されています7。実験的には、尿中ATP濃度の操作はラットの膀胱反射を変化させる可能性があります。膀胱内腔の内因性ATPアーゼを遮断することによってATP濃度を増加させると排尿頻度を増加させることができ、一方、外因性ATPアーゼを膀胱に注入することによってATP濃度を低下させると排尿頻度が低下する8。したがって、膀胱機能に対する尿中ATPの重要性は明らかである。
膀胱病理に対する尿中ATPの明らかな重要性を考えると、尿路上皮ATP放出の正確な測定は、放出を制御するメカニズムを理解する上で重要なステップです。尿路上皮ATP放出を測定するために、異なる実験モデルを用いて多くの研究が完了している。これらの中で最も重要なのは、初代培養または細胞株のいずれかの細胞培養です。しかし、培養尿路上皮細胞の使用は、特殊な透過性膜(Transwell技術[ウェルインサート]など)で増殖させない限り、尿路上皮細胞が生理学的分極形態をとらないという事実によって複雑になります9。したがって、測定された任意のATP放出を生理学に関連付けることは困難である。ウェルインサート上で増殖した尿路上皮細胞は分極し、 in vivoで見られるものと同様の障壁を形成する可能性があります。ただし、完全に分化した尿路上皮の成長には数日から数週間かかる場合があります。さらに、ウッシングチャンバーにウェルインサートを取り付け、頂端側に圧力を加えて伸張させることは可能ですが、病理学中に膀胱内の状態を模倣するのに十分な圧力(すなわち、30 cm H2O以上の圧力)を適用することは困難です。膀胱組織全体をストレッチ実験のためにUssingチャンバーに取り付けることもできますが、これにより、尿路上皮細胞の健康、したがって尿路上皮バリア機能を維持する栄養因子とともに、膀胱が生物から除去されます。したがって、伸張または圧力に応答した尿路上皮からのATPの放出を研究するための最も生理学的に関連性のある方法は in vivoである。実験のセットアップに必要な外科的技術は、動物の膀胱測定で一般的に使用されるものと同じであるため、その技術に精通している人なら誰でも簡単に実行できるはずです。
このプロトコルでは、以下で説明する経尿道的カテーテル法は女性の方がはるかに簡単であるため、体重約200〜250 gの雌Sprague Dawleyラットの管腔ATPを調べるために使用される技術について説明します。しかしながら、経尿道的カテーテル法は、雄のげっ歯類10においても行うことができる。経尿道的カテーテル法は現在、男女両方のマウスでも行われているため11、これらの実験は、研究チームのニーズに応じて、性別またはさまざまなサイズのマウスまたはラットに簡単に適応させることができます。

私たちのプロトコルは、膀胱の重要な伝達物質であるATPを測定し、その放出を制御するメカニズムをさらに調べることを可能にします。私たちの技術は、膀胱内腔から直接ATPを測定するため、培養細胞や膀胱組織を使用する技術よりも生理学的に関連性があります。手順を実演するのは、私の研究室の研究技術者であるKeara HealyとStephanie Daughertyです。まず、動物を酸素中の4%〜5%イソフルランでガスを放出した密閉ボックスに入れることによって初期麻酔を誘発する。ウレタンを両側皮下投与し、次に動物をケージに入れて、ウレタンがほぼ2時間有効になるようにします。ウレタンが効くのを待った後、鉗子を使って動物の足をつまんで適切な麻酔面をテストします。実験中の膨張に反応して膀胱の収縮を防ぐために、動物に神経節遮断剤であるヘキサメトニウムを注射します。.次に、実験中の乾燥を防ぐために、眼科用軟膏を動物の目に塗布します。次に、動物の腹部を剃り、正中線開腹術を行って膀胱を露出させます。炎を使用して、短い長さのPE50イントラメディックチューブの一方の端をフレアし、チューブのもう一方の端に22ゲージの針を配置し、クレブス溶液で満たします。膀胱のドームの上に3-0絹縫合糸の小さなループを置き、小さな膀胱瘻造設術を行ってカテーテルのフレア端を挿入します。片手でカテーテルを持ちながら、縫合ループを締めて、もう一方の手でカテーテルを所定の位置に固定します。縫合糸に2つの結び目を結び、カテーテルの固定を終了します。フレアヘッドが膀胱壁に接触するまでカテーテルを引き戻します。カテーテルを通して少量のクレブス溶液を注入することにより、漏れのセットアップをテストします。IVカテーテルの端を外科用潤滑剤に浸します。一対の鉗子で外尿道口をそっと持ち、原稿に記載されているように、カテーテルの先端を尾の方向に尿道開口部に挿入します。カテーテルを90度回転させ、ゆっくりと前進させます。ルアーロックハブが外尿道開口部から5ミリメートル離れるまでカテーテルを挿入します。カテーテルを固定し、外尿道口の周りに短い長さの3-0シルク縫合糸をループさせ、しっかりと結ぶことにより、カテーテルの周りの漏れを防ぎます。誤って引き抜かれないように、カテーテルをテープで尾に固定します。カテーテル挿入したら、恥骨上膀胱カテーテルからクレブス溶液を膀胱に静かに注入し、液体が尿道カテーテルの周囲ではなく尿道カテーテルから流出することを確認します。3-0シルク縫合糸を使用して膀胱の腹部切開を閉じます。吸収性アンダーパッドを加熱パッドに配置して、体温を維持し、液体を吸収します。動物を傾斜したボードに固定して、尿道カテーテルから膀胱内液を排出するのを助けてから、加熱パッドをオンにします。恥骨上カテーテルを、カテーテルをシリンジポンプと圧力トランスデューサに接続する三方ストップコックに接続します。圧力トランスデューサをアンプとデータ収集システムを介してコンピュータに接続します。恥骨上カテーテルにクレブス溶液を毎分0.1ミリリットルの速度で注入し、液体を尿道カテーテルから1時間排出して、カテーテル移植中に放出された残留ATPを洗い流します。このウォッシュアウト期間の後、ルアーロックプラグを使用して尿道カテーテルにキャップをします。膀胱内の圧力を測定し、膀胱内圧が急激な上昇なしに30センチメートルの水圧までゆっくりと上昇し、膀胱収縮を示すのを探します。膀胱の損傷を防ぐために、圧力が30センチメートルの水に達したら、尿道カテーテルからプラグを取り外します。膀胱を注入し、尿道カテーテルから溶離液を採取します。溶離液の100マイクロリットルアリコートをATPについてすぐにテストするか、後でバッチ定量するために凍結します。管腔ATP濃度に対する膀胱膨満の影響をテストするには、尿道カテーテルをプラグでキャップし、膀胱圧を所望のレベルに達するまで監視してから、尿道カテーテルのキャップを外し、上記のようにATP測定または凍結用の溶離液を収集します。各膨張の後、追加のサンプルを採取する前に、膀胱を休ませて10〜15分間洗い流します。ATPの放出に対する薬物の効果をテストするには、膀胱を注入するクレブス溶液を、選択した薬物を含むクレブスに切り替えます。効果を得るために10〜15分間薬を灌流し、前に示したように、膨張していない膀胱と膨張した膀胱からサンプルを収集します。100マイクロリットルの灌流液サンプルと50マイクロリットルのアッセイミックスをルミノメーターに入れて読み取ります。相対光単位からATP濃度に変換するには、クレブス溶液中のATPを1マイクロモルから10ピコモルの範囲で10倍希釈して標準曲線を作成し、ルミノメーターで読み取ります。結果の読み取り値をグラフにプロットし、非線形回帰を実行して、動物から採取したサンプルから濃度を推定します。尿道カテーテルが詰まった状態で溶液を膀胱に注入すると、膀胱が収縮するにつれて膀胱内圧が急激に上昇します。ヘキサメトニウム投与後、膀胱内圧のより緩やかな上昇が観察され、収縮することなく30センチメートルの水まで圧力が上昇した。ブリリアントブルーFCFは、検量線の傾きを有意に減少させ、サンプル中のATP濃度の計算値を変更するのに十分である。ブリリアントブルーFCFを含むクレブス標準を使用してサンプル中のATP濃度を計算すると、ATP濃度が50%過小評価される可能性があります膀胱の膨張は管腔ATP濃度を増加させ、NTPDaseアプラーゼの存在下で減少させるか、内因性NTPD ACEがARL67156で阻害されると増加する可能性があります。尿路上皮からの伸張誘発ATP放出を分析した。膨張誘発ATP放出は、パネクチンチャネルアンタゴニスト、ブリリアントブルーFCF、およびカルボノキソロンによってブロックされますが、コネクソンチャネルアンタゴニストである18α-グリチルレチン酸ではブロックされません。ルシファールシフェラーゼ反応は多種多様な異なる薬剤による干渉の影響を非常に受けやすいため、すべてのルミノメーターの読み取り値を適切な標準曲線を使用してATPの濃度に変換することが不可欠です。私たちの技術は、膀胱の内腔内の任意の分子の膨張誘発放出を研究するために使用できます, その分子に適したアッセイが存在すると仮定します.
