주산기와 신생아 시기는 종종 부모에게 스트레스를 줍니다. 이는 일본과 같은 국가에서 특히 문제가 되는데, 일본은 저출산, 고령화, 핵가족 생활 경향을 감안할 때 자녀 양육에 대한 직접적인 노출이 제한적입니다. 이러한 상황은 자녀 양육에 관한 제도적 지원 교육의 부족으로 인해 더욱 악화되고 있다1. 이러한 제한된 경험과 빈곤과 관련된 사회적 문제를 배경으로 볼 때, 예를 들어 젊은 부부의 경우 부모의 스트레스 관련 학대 및 방치가 있는 경우 모유 수유 문제와 관련된 산후 성장 실패의 위험이 높다2.
"분만 후 모유수유와 분리된 산모가 모유수유를 하는 것은 애착 형성에 매우 중요하며, 모유수유와 분유수유를 비교한 결과 모유수유가 신생아의 풍부한 감정 발달에 영향을 미친다는 것을 알 수 있다"며 모유수유와 애착형성 사이에 강한 인과관계가 있음을 시사한다3. 그러나 영아나 산모에게 의학적 문제가 있는 경우 조기 모유 수유의 성공과 출생 후 성장 및 발달을 개선하기 위해 신생아의 적절한 영양 관리를 위해 신생아와 산모를 분리해야 하는 경우가 많습니다. 신생아가 안정화된 후에는 모유 수유를 성공적으로 시작할 수 있도록 산모에게 지도와 지원을 제공해야 합니다. 아버지, 친밀한 파트너 및 기타 지원 개인이 이 지침에 포함되어 자녀를 양육하고 모유 수유에 대한 적절한 지원을 제공하는 방법을 배워야 합니다4. 대부분의 문헌(예: "분만 직후부터 모유수유 확립까지의 환경 및 간호 중재")은 주로 산모를 위한 중재와 지원에 초점을 맞추고 있다5. 조산사 및 간호학과 학생들의 인식과 특정 중재 기술을 더 잘 가르쳐 부모와 자녀 모두를 참여시키고 둘 사이의 상호 작용을 장려하는 모유 수유를 지원할 것으로 예상됩니다.
이전에 우리는 주산기 및 신생아 중재의 정신생물학적 기초를 탐구하기 위해 작은 비인간 영장류 모델로 일반적인 마모셋(Callithrix jacchus)을 개발했다6. 우리는 다양한 환경에서 감각(시각, 청각, 촉각, 후각) 지각 제어의 발달 변화를 정량적으로 식별하기 위해 사회적 장면에따른 변화7,8,9 및 생체 분자 활동(10,11,12)을 사용하는 시스템을 설계했습니다. 주성분 분석(PCA)을 사용하여 여러 설명 행동 요인을 추출할 수 있었고, 이를 통해 이러한 동물의 사회 심리적 기능을 담당하는 복잡한 신경망을 재현할 수 있었습니다. 이러한 정량적 감정상태 번역(BOUQUET) 방법을 위한 행동산출 분석의 결과는 성인 남성과 여성 간의 사회적 상호작용이 동일한 평가 시스템에서 친숙도에 따른 특징을 기반으로 정량적으로 시각화될 수 있음을 시사하였다11. 우리는 일반적인 마모셋 행동의 차이가 과거에 각 개인의 심리인지 학습을 반영한다는 가설을 세웠다.
이 연구에서 우리는 세 가지 조건, 즉 아이들이 서로 상호작용하는 사회적 환경에 있는 형제자매, 부모와의 사회적 상호작용만 경험한 한 아동, 그리고 새끼가 유전적 가족 구성원으로부터 분리되는 인간 위탁 보호와 유사한 상황의 동물을 비교했다. BOUQUET 분석은 집단 간 사회적 반응 행동 발달의 시간 경과에 양적 차이가 있음을 보여주었으며, 이는 발달 중 가족 상호작용(즉, 부모와 형제자매)이 사회심리적 기능 형성에 영향을 미친다는 것을 나타낸다13. 또한, 연구진이 분유를 제공한 위탁 양육을 재현하는 상황에서 지속적인 적외선 영상을 사용하여 마모셋의 출생 후 15일째부터 45일까지의 심리사회적 발달을 분석했습니다. 체표면 온도, 활동, 위치 선호도를 포함한 데이터는 연령에 따른 고유한 패턴의 존재를 시사했습니다. 추가 분석 결과, 초기 수유 기간의 경험은 발달 1개월 후 행동 리듬에 유의한 영향을 미쳤을 수 있다14. 일주기 리듬과 그 혼란은 점점 더 복잡해지는 사회에 대한 인간의 적응에 필요한 정신 기능과 밀접한 관련이 있는 것으로 알려져 있으며, 그 형성은 어린 시절의 경험에 의해 영향을 받습니다.
이 보고서는 이전 연구 결과 11,13,14에 이어 아버지의 학대와 유사한 행동과 어머니의 방치와 같은 행동을 보이는 일반적인 마모셋 가족의 모유 수유 실패 사례에 개입하는 타당성을 탐구합니다. 마모셋은 일반적으로 다접합 배수를 생산하고 어미뿐만 아니라 아버지와 형제 자매의 협력을 통해 자손을 공동으로 키우는 것으로 알려져 있지만, 이 영장류 모델은 핵가족, 부모, 그리고 그들의 유일한 자손을 포함하며, 이는 인간 사회 조건의 현재 상태를 모방하는 것을 목표로 합니다. 처음 마모셋을 낳은 부모는 모유 수유를 중단하고 아기를 유기한 후 억제되지 않은 행동을 보였고, 이로 인해 아기가 잘 자라지 못했습니다. 사회적 섭식 영장류 모델에서 애착 기능 장애의 경우 개입 및 회복 과정은 비디오 기록을 통해 평가되었습니다. 증상 및 치료 효과는 생존한계에 비해 체중의 증가 또는 감소로 표시되며, 이는 이전에 발표된 기준15,16을 참조한다. 이 연구의 더 큰 목표는 모유 수유를 시행하는 어머니와 아버지 및 이를 지원하는 조산사의 교육을 위한 인간 임상 개입의 효능을 결정하는 것입니다. 자연 선택에 의해 몇 가지 문제가 있는 취약한 자손을 버리는 것은 일반적인 마모셋의 정상적인 행동으로 분류될 수 있지만, 이 임상 모델은 모든 생명의 존엄성을 위해 건강과 웰빙을 지원하는 인간 진화 전략을 탐구하는 것을 목표로 합니다.
